    Description / Table of Contents: Summary Huntington’s disease (HD) may cause considerable diagnostic problems because of the highly variable clinical features, especially at the beginning of the illness. We report the case of a 21-year-old woman with definite HD whose main symptoms were psychopathological changes that were misinterpreted as schizophrenia for 7 years. One reason for this misdiagnosis was the unusual, very early onset and the remarkable earlier age of onset of HD compared with that of other affected relatives. The phenomenon of earlier onset within one affected family is well documented in the literature in cases with the paternally transmitted mutated gene. Diagnostic landmarks were consideration of the initially mild disturbances of movement and the once again repeated detailed examination of the family history.
    Notes: Zusammenfassung Die Diagnose einer Chorea Huntington kann aufgrund der hohen Variabilität des klinischen Bildes besonders zu Beginn der Erkrankung erhebliche Probleme bereiten. Wir stellen eine 21jährige Patientin mit einer gesicherten Chorea Huntington vor, bei der über 7 Jahre die psychopathologischen Veränderungen die Hauptsymptomatik bildeten und als Schizophrenie fehldiagnostiziert wurden. Außergewöhnlich und somit für die Fehldiagnose maßgeblich verantwortlich waren der sehr frühe Beginn der Erkrankung und die im Vergleich zu anderen erkrankten Familienmitgliedern deutliche Vorverlegung des Krankheitsbeginns. Das Phänomen der Vorverlegung des Krankheitsbeginns ist jedoch für den Fall der Übertragung des mutierten Gens vom Vater in der Literatur gut dokumentiert. Differentialdiagnostisch wegweisend waren die Beachtung der zunächst dezenten Auffälligkeiten des Bewegungsmusters der Patientin und die daraufhin nochmals detaillierte Erhebung der Familienanamnese.

    Notes: Abstract Primary infections with herpes simplex virus type 2 (HSV-2) suppress the antibody response to secondary HSV-1 and -2 infections in the BALB/c mouse. In contrast, a challenge by the i.p. route using a vaccinia virus-HSV-1 glycoprotein B (VV gB1) recombinant induces a significant enhancement of the antibod response. This booster reaction is also observed if a challenge with a formalin-inactivated HSV-1 vaccine is performed. Although no or low humoral and vaginal antibodies are detectable after a single i.p. infection with the VV gB1 recombinant or the HSV-1 vaccine, protection against vaginal challenge with HSV-2 is induced. This points to the important role of cellular immunity for vaginal infections.

    Notes: Abstract Reinforcement effects in composites are widely influenced by fibre coatings. A detailed understanding of their microstructure and chemical composition is of great interest. Boron nitride films were deposited continuously on fibre rovings of various ceramics in CVD reactors of vertical as well as horizontal position. XPS depth profilings show that the film compositions are close to stoichiometric BN with carbon and oxygen impurities in the range of 10 at%. Cross-sections of separated fibres were investigated by HREM and TEM diffraction. All BN films are hexagonal turbostratic. The (002) layers with an increased distance (about 0.36 nm) showed a mean stacking sequence near to graphite and a characteristic orientation to the fibre in the interface region. We assume the gas flow type and hence the exchange rate of matter and energy determines the film structure in this region. With increasing film thickness the (002) layers fold randomly in all directions or form nanocrystals at elevated temperatures.

    Description / Table of Contents: Résumé Le cas d'une jeune femme de 19 ans porteuse d'un lymphome à cellules béta primitif est présenté. Une imagerie médicale complète avec scanner, IRM ainsi qu'une étude histologique ont été nécessaires au diagnostic, qui a d'abord été erroné avec suspicion d'ostéomyélite chronique. Une résection du fémur distal comprenant le genou a été réalisée et une distraction bipôlaire du cal fut ensuite appliquée. Après une inflammation locale, le diagnostic de lymphome à cellules béta put être posé et une étude immunohistochimique sur un prélèvement de tissus mous fut demand'ee. Une amputation de cuisse dut être effectuée à cause d'une grave nécrose et d'une surinfection des tissus mous. Des problèmes diagnostiques survinrent à cause de symptomes aspécifiques, d'incertitudes radiologiques et d'une absence de diagnostic histologique. Les facteurs de croissance ont probablement stimulé la croissance tumorale après la mise en route de la distraction du cal.
    Notes: Summary A case of a 19-year-old female patient with a primary B-cell lymphoma of bone is presented. Although extensive diagnostic procedures including CT and MRI scans as well as bone histology were used, the case was initially misdiagnosed as chronic osteomyelitis. Resection of the distal femur including the knee joint was performed and bisegmental callus distraction was initiated. After local exacerbation a B-cell lymphoma was diagnosed histologically and immunohistochemically in a soft tissue specimen. Above-knee amputation had to be carried out due to severe local soft tissue necrosis and superinfection. Diagnostic problems arose from non-specific clinical symptoms, uncertain radiographic signs and non-diagnostic histology. Growth factors may have stimulated tumor growth after initiation of callus distraction.

    Notes: Summary Mid- and long term stability is still the major challenge in total hip replacement (THR). A consecutive series of cemented 250 cases and of uncemented 250 cases was analyzed in a prospective follow-up study. The femoral component of the BiCONTACT hip stem system with the same design for cemented and uncemented implantation was used. While the cemented stems had a smooth surface, the cementless implants were proximally coated with a microporous titanium plasmaspray (Plasmapore®). Follow-up time averaged 7 years for cemented and 6.5 years for uncemented THRs. Follow-up rate was 88.7% in cases treated with cemented stems and 93.1% in cases treated with an uncemented stem. The rate of aseptic loosening was 1.2% for the cemented version and 0.4% for the uncemented one. Radiological loosening could be observed in another 1.2% of the cemented stems but was not found in the uncemented cases. Our data suggest excellent midterm stability of both the cemented and the uncemented version of the BiCONTACT hip stem system. Early aseptic loosening especially of the cemented titanium alloy straight stem has not occurred in our series as reported recently. The prosthesis design and follow-up time may therefore play an important role. The long-term results for both cemented and uncemented BiCONTACT prostheses of our series are still to be evaluated.
