Notes: Abstract  Hepatocellular carcinoma (HCC) occurs more frequently in subsaharan Africa and the Orient than in other geographical regions, but remains an uncommon tumour of childhood. We review six children with HCC (mean age 13 years) treated by the paediatric oncology unit at Tygerberg Hospital in Cape Town over an 8-year period (1983 – 1990). Patients presented with epigastric and right upper quadrant discomfort and hepatomegaly. The hepatitis B serum antigen (HbsAg) was positive in three patients; serum alpha-fetoprotein (AFP) levels were markedly elevated in three (range 100 – 453,000 μg/l). Age and sex did not differ significantly and all patients initially had irresectable advanced-stage tumours. Morphologically, three were highly malignant adult-type pleomorphic HCCs, two were differentiated tumours, and one a fibrolamellar subtype. The mean 2-year survival was 33% and the 5-year survival 16.6%. The biological behaviour and response to treatment of the tumours varied. Whereas three patients had a poor response to therapy, two with poorly-differentiated tumours, negative HbsAg, and normal serum AFP levels responded to doxorubicin/cisplatinum chemotherapy. This facilitated radical surgical excision. One patient of this group has survived for more than 75 months following surgical resection and remains well. HCC remains an uncommon tumour of childhood with a high mortality. Aggressive chemotherapeutic regimes in combination with surgical resection may lead to improved survival in some cases. Prevention of hepatitis B remains a priority.

Notes: Abstract A re-emergence of tuberculosis (TB) is occurring world wide in both developed and developing countries. The clinical picture caused by infection with M. tuberculosis may simulate many other disease entities and may result in unnecessary investigations with a delay in diagnosis and treatment. Skeletal TB tends to be isolated to one anatomical site. We report a 6-year-old boy with disseminated skeletal TB with dactylitis resembling sickle cell anaemia and lytic lesions similar to those which are often seen in neuroblastoma, Langerhans' cell histiocytosis and leukaemia. The clinician should be aware that TB can mimic almost any disease and recognise the radiographic appearances of skeletal tuberculous lesions.

Description / Table of Contents: β-diketone complexes of the type M(LL)4, where M = Zr(IV), Hf(IV), Th(IV) and U(IV), and LL = β-diketone with different acid strengths, viz. acetylacetone, dibenzoylmethane, thenoyltrifluoroacetone, trifluoroacetylacetone and hexafluoroacetylacetone were used to prepare adducts of the type M(LL)4 · S, where represents the neutral phosphorus esters with various basicities, viz. tributylphosphate, trioctylphosphine oxide, triphenylphosphine oxide and triphenylphosphate. It was found that no adducts of the Zr(IV), Hf(IV) and U(IV) complexes were formed Adducts of the Th(IV) complexes were formed only if the β-diketones were relatively strong acids, e. g. thenoyltrifluoroacetone, trifluoroacetylacetone and hexafluoroacetylacetone. Results of the attempted preparations showed that the possibility of adduct formation between β-diketone complexes and phosphorus esters in influenced by the acid strength od the β-diketone, the basicity of the phosphorus ester, the ionic radii, and the electron configuration of the metal.
Notes: β-Diketonkomplexe vom Typ M(LL)4, worin M = Zr, Hf, Th sowie U und LL = β-Diketone verschiedener Säurestärken, d. h. Acetylaceton, Dibenzoylmethan, Thenoyltrifluoroaceton, Trifluoroacetylaceton, und Hexafluoroacetylaceton bedeuten, wurden für die Darstellung von Addukten des Typs M(LL)4 · S, verwendet. Darin bedeutet S die neutralen Phosphorester TBP, TOPO, TPPO, und TPP (vgl. Abstract).

Notes: The crystal structure of the compound thorium(IV)tetrakis(trifluoro-acetylacetonate) was determined by means of a threedimensional X-ray analysis. The space group is C2/c and the cell dimensions are a = 25,05 Å, b = 6.43 Å, c = 21.3 Å, β = 125.4°, with Z = 4. The thorium atom is coordinated by 8 oxygen atoms in the form of an 1111 (D4-422) antiprism. The mean of the Th-O distances is 2.39 Å with a standard deviation ± 0.04 Å. The trifluoroacetylacetonate rings are approximately planar, except for the CH3 and CF3 groups which show significant deviations. The Th-O bonds form angles of approximately 72° and 50° with the theoretical 8-axis of the antiprism. The structure is stabilized by VAN DER WAALS contacts between neighbouring molecules. The refinement of the atomic positions, the anisotropic temperature parameter of thorium and isotropic temperature parameters of all the other atoms by the least squares method, has given a reliability index of 0.148.
