    Notes: Summary We report two patients with thrombotic thrombocytopenic purpura who were subjected to plasma exchange. In one case, the plasma levels of platelet factor 4, measured shortly after plasma exchange, increased significantely during plasma exchange. This was followed, however, by a failure to respond to therapy. Repeated plasmapheresis over 3 weeks gave no therapeutic benefit and reversible deep coma occurred. This patient recovered completely after treatment with vincristine. In the second patient, a decline in platelet factor 4 was observed after plasma exchange. This was accompanied by improvement of the patient's condition and a slow rise in platelet count. Plasma exchange was again carried out in this patient because of a recurrence of thrombotic thrombocytopenic purpura 3 years later; again decreased platelet factor 4 plasma levels were observed after plasma exchange and again a therapeutic response followed. Platelet factor 4, therefore, seems to be an effective and early index for the therapeutic benefit of plasma exchange in thrombotic thrombocytopenic purpura.

    Notes: Summary Thirty-seven patients (15 female, 22 male; aged 46–89 years, mean age 68.8 years) with III or IV grade carotid sinus reflex carotid arteries were investigated by sonography (B-scan and CW-Doppler). It was demonstrated that stenoses or arteriosclerotic plaques, localized in the region of the carotid bulb or bifurcation, were present on both sides in 27 patients and on one side only in four patients. Five patients did not show these changes and four of them had thickening of common carotid artery endothelium. Thus, vascular changes occurred in 35 patients. Occlusions and stenoses 〉50% were visible in five patients. Primarily bilateral changes were shown as well as unilateral and bilateral pathologic reflex response; main localization was the posterior wall of the carotid bulb. Thus, arteriosclerotic changes seem to be important in the development of pathologic carotid sinus reflex. Cerebral complications (left-sided PRIND) occurred in one patient who did not have high degree carotid stenosis. In comparison to other authors who investigated only with CW-Doppler a much higher rate of arteriosclerotic wall changes and a greater occurrence of bilateral changes could be demonstrated by application of the highly sensitive B-scan sonography.

    Notes: Summary We report three patients with advanced breast cancer who received hyperthermia (43°C over 1 h, weekly ×4) and radiotherapy as palliative treatment for chest-wall recurrence. The case reports demonstrate situations where hyperthermia may be beneficial. These include tumours where radiotherapy alone is not expected to achieve control, such as large fungating tumours, and in previously irradiated areas. In these patients, hyperthermia with radiation achieves better local control of the tumour and delays the need for systemic chemotherapy. This may improve the quality of palliation.

    Notes: Summary A total of 419 real-time and duplex Doppler ultrasound examinations of renal allografts were carried out in 61 patients. Results were related to the clinical diagnoses based on histomorphological, clinical, and laboratory findings. The results of all sonographic examinations in terms of the diagnostic parameters of normal function, acute tubular necrosis, and interstitial and vascular rejection did not yield reliable criteria for distinguishing between the different forms of transplant dysfunction. The maximum difference was calculated for each ultrasound parameter between a time of normal transplant function and at a time of dysfunction. By means of this ‘maximum parenchyma-pyelon index difference’ it was possible to discriminate between acute tubular necrosis and vascular rejection (P〈0.05). Acute tubular necrosis could be distinguished from interstitial rejection using the maximum longitudinal renal diameter difference and the maximum parenchyma-pyelon index difference (P〈0.05). To discriminate between interstitial and vascular rejection the maximum pulsatility index difference was very useful (P〈0.05). Combined real-time and duplex Doppler sonography is most valuable in the diagnosis of transplant failure when it is performed not only in the case of dysfunction but also when transplant function appears to be normal.

    Description / Table of Contents: Summary Increased thrombin generation associated with resistance to activated protein C makes the latter a likely candidate for an increased risk of acute coronary events. Activated protein C resistance (factorV Leiden) on its own, however, appears to have no significant effect in this regard. We describe a case of recurrent myocardial infarction caused by coronary thrombosis in a patient with persistent thrombocytopenia who was found to have a coexistence of heterozygous factor V Leiden and primary antiphospholipid syndrome. Since both thrombophilic disorders interfere with the protein C anticoagulant system, the simultaneous existence of inherited and acquired resistance against activated protein C could account for an increased thrombophilia with manifestation in the coronary arteries. This case suggests that evaluation of patients who present with recurrent acute coronary events should also consider these coagulation defects.
    Notes: Zusammenfassung Eine verminderte Inhibitorwirkung des aktivierten Protein C und die daraus resultierende übermäßige Entstehung von Thrombin könnte zu einem erhöhten Risiko für akute koronare Ereignisse führen. Eine angeborene Resistenz gegen aktiviertes Protein C (Faktor-V-Leiden) allein scheint jedoch keinen Risikofaktor für Myokardinfarkte darzustellen. Wir berichten über einen Patienten mit chronischer Thrombozytopenie und rezidivierenden Myokardinfarkten infolge koronarer Thrombosen, bei dem eine Koexistenz von heterozygotem Faktor V Leiden und primärem Antiphospholipid-Syndrom diagnostiziert wurde. Da beide thrombophilen Erkrankungen mit dem Protein C System interferieren, könnte eine erhöhte Thromboseneigung mit Manifestation in den Koronargefäßen auf das gleichzeitige Vorliegen einer angeborenen und erworbenen Resistenz gegen aktiviertes Protein C zurückzuführen sein. Der Fallbericht weist darauf hin, dass Patienten mit rezidivierenden akuten Koronarereignissen auch hinsichtlich des Vorliegens dieser Gerinnungsstörungen untersucht werden sollten.

    Notes: Summary The carotid bifurcations and the common carotid arteries of 36 patients with the diagnosis hyperactive carotid sinus syndrome (HCSS) were investigated by continuous wave (CW) Doppler ultrasonography and high-resolution real-time B-scan. Using these non-invasive tests, the functional impact of luminal stenosis and the morphological changes resulting from arteriosclerotic deformities could be established. Significant differences were found in comparison with a reference group of 199 patients with a high risk of arteriosclerosis. In the HCSS group, 5 patients had a stenosis of more than 50% at the origin of the internal carotid artery on both sides, or on one side in combination with large plaques or a complete occlusion on the contralateral side. Seventy-five per cent of patients in the HCSS group, as compared to only 23.5% of the control group, had effective arteriosclerotic changes in the carotid bifurcation on both sides; 4 patients had such changes only unilaterally. Marked additional bilateral arteriosclerotic depositions were detected in the common carotid arteries of 17 patients (47.2%). In 5 patients no arteriosclerotic lesions were detectable in the carotid bifurcations, but marked changes were found in both common carotid arteries. These data indicate that bilateral arteriosclerotic changes in the carotid bifurcations and/or the common carotid arteries represent an important pathophysiological factor for the development of an HCSS.

    Notes: Summary The question as to whether the serum concentration of erythropoietin is relatively high for the degree of anemia in patients with erythrocytic hypoplasia has regained interest, since recombinant human-like erythropoietin has become available as a drug for replacement therapy. We have compared the concentration of serum immunoreactive erythropoietin in nonrenal anemic patients with erythrocytic hypoplasia (22 cases) or active erythropoiesis (82 cases). In both groups a negative correlation was determined between the blood hemoglobin concentration and the logarithm of the erythropoietin concentration. However, the two regression lines were not identical, and the serum erythropoietin concentration was significantly higher for the degree of anemia in the patients with erythrocytic hypoplasia. Additional measurements in four patients suffering from acute leukemia with marrow failure showed that the erythropoietin concentration decreased towards the values observed in anemic patients with active erythropoiesis when the erythron recovered in the early phase of complete remission. These data support the idea that, independent of the O2 offer, the proliferating erythrocytic progenitors by negative feedback lower the blood level of erythropoietin.

    Notes: Summary This article briefly describes the use of a photon counting system (ARGUS-100) in the detection of low levels of light. The ARGUS-100 was used in determining ATP in cell sections from tumor tissues and in measuring a luminescenceenhanced immunoluminometric assay, using ferritin as the analyte, based on the luminol-peroxide-4-iodophenol reaction with peroxidase as the enzyme. The aim is not so much the presentation of data, but rather to show the potentials of the photon counting camera in increasing our knowledge of the cellular and subcellular levels, as well as lowering the detection limits in already sensitive systems, such as immunoassays.
