Notes: Summary Hyperinsulinaemia is a reported feature of the inherited multisystem disorder myotonic dystrophy. This phenomenon has been attributed to a compensatory beta cell response to tissue insulin resistance. In this study, circulating concentrations of insulin, proinsulin, and split proinsulin molecules were determined after an overnight fast in ten patients with myotonic dystrophy using two-site monoclonal antibody-based immunoradiometric assays. Results were compared with ten healthy control subjects matched for age, gender, and body mass index. Oral glucose tolerance (75 g), as defined by World Health Organization criteria, was normal in all subjects. Fasting plasma immunoreactive insulin concentration, as determined using a conventional radioimmunoassay, was almost three times higher (p〈0.005) in the myotonic dystrophy patients than the healthy control subjects. By contrast, fasting concentrations (mean±SEM) of C-peptide (0.75±0.09 vs 0.52±0.03 nmol/l, p=0.07) and immunoradiometrically-determined insulin (60±12 vs 38±4 pmol/l, p=0.09) were not significantly different between the groups. Fasting concentrations of proinsulin (10.3±2.9 vs 1.6±0.3 pmol/l, p〈0.01), and 32–33 split proinsulin (7.8±2.5 vs 2.9±0.4 pmol/l, p〈0.05) were significantly elevated in the patients with myotonic dystrophy. Accordingly, the mean fasting proinsulin∶insulin ratio, expressed as a percentage, was significantly increased in the myotonic patients (20±5 vs 4±1%, p〈0.01). The overall C-peptide response to the oral glucose challenge was significantly greater in the myotonic patients compared with the healthy control subjects (p〈0.001). These results provide corroborative evidence of increased beta-cell secretion in myotonic dystrophy. In addition, myotonic dystrophy is characterised by elevated plasma concentrations of proinsulin-like molecules which may cross-react in insulin radioimmunoassays.

Notes: Twelve beef/pork meat blends were made from lean of mature beef and dark or mature pork carcasses. Meat blends containing about 20% beef fat were rated higher for appearance scores by a consumer panel than blends made with 20% pork fat. Visual acceptance of beef/pork patties was accomplished via use of all-beef fat and additionally by extending mature beef (MB) with mature pork (MP) or dark pork lean (DP). Agtron calorimetry scores indicate a minimum of 20% MB lean extended with 60% MP was needed to create the appropriate red color attractive to consumers. Use of 60% MP lean with about 20% beef fat resulted in patties acceptable to consumers for visual appearance and eating satisfaction.

Notes: Abstract: Seventeen analogues of l-methyl-4-phenyl-1,2,3,6-tetrahydropyridine were synthesized using three reaction pathways: condensation of phenols with 1-methyl-4-piperidone, reaction of Grignard reagents with 1-methyl-4-piperidone followed by dehydration of the product, and aminomethylation of olefins. The identity of the products of synthesis was established by nuclear magnetic resonance spectroscopy, mass spectroscopy. and elemental analysis. Thirteen analogues were shown to inhibit the oxidation of benzylamine by bovine plasma amine oxidase. Increasing the length of the aliphatic chain of N-substituted analogues resulted in increased inhibition. In 4-phenyl-substituted analogues, both the position and electronic character of the substituent group affected the degree of inhibition.

Notes: An unusual case of re-exploration of the site of previous ventricular aneurysmectomy is reported. The conduct of cardiopulmonary bypass was complicated by the presence of cold agglutinins with a wide thermal range active up to 30°C. There follows a brief description of cold agglutinins, and the difficulties they cause during cross-matching.

Notes: Summary Platelet MAO-B levels have been investigated in seventeen consecutively diagnosed and previously untreated patients with idiopathic Parkinson's disease using the non-hydroxylated catecholamine, β-phenylethylamine, as substrate. Patients with Parkinson's disease had MAO-B activity levels that were considerably higher than sex and age matched normal controls or patients with Motor neurone disease or Myasthenia gravis.

Notes: Abstract For a given optimization problem, P, considered as a function of the data, its marginal values are defined as the directional partial derivatives of the value of P with respect to perturbations in that data. For linear programs, formulas for the marginal values were given by Mills, [10], and further developed by the current author [16]. In this paper, the marginal value formulas are extended to the case of mixed integer linear programming (MIP). As in ordinary linear programming, discontinuities in the value can occur, and the analysis here identifies them. This latter aspect extends previous work on continuity by the current author, [18], Geoffrion and Nauss, [5], Nauss, [11], and Radke, [12], and work on the value function of Blair and Jeroslow, [2]. Application is made to model formulation and to post-optimal analysis.

Notes: Summary Enzymes involved with the metabolic transformation of xenobiotics have recently been studied in patients with the neurodegenerative diseases, Alzheimer's disease, Parkinson's disease and motor neurone disease. Defects were detected in sulphur pathways and also, in the case of Parkinson's disease, in monoamine oxidase B. The possibility exists that the ability to cope safely with endogenous and exogenous substances which have neurotoxic properties is important in the pathogenesis of these diseases. Potentially such individuals could be identified preclinically and these diseases postponed by reduction in the load of toxin or modification of the relevant enzymic activity.

Notes: Summary An extension of geometric programming to handle rational functions of posynomials is presented. The solution technique consists of successive approximations of posynomials and solution of ordinary geometric programs. An example of a multistage heat exchanger system optimization illustrates the computational method.
