    Notes: Summary Experimental cerebral vasospasm was produced in a “two-hemorrhage” canine model and examined by immunohistochemistry for leukotriene C4 (LTC4). The immunostain for LTC4 showed a strong positivity in intima and adventitia and a scattered reaction in media of normal basilar artery. The immunoreactivity after subarachnoid hemorrhage (SAH) was little changed in intima and media. Inflammatory cells which were characterized histochemically as neutrophils and macrophages, were shown to infiltrate from the adventitia of basilar artery to the periphery of blood clot after SAH and were markedly immunoreactive for LTC4. Also the neutrophils increased in number with the lapse of time after SAH. Thus, it would be reasonable to conclude that the LTC4 responsible for the development of vasospasm would most likely be produced from the infiltrating neutrophils and macrophages. In addition, neurons in hypothalamus, median eminence, and pons, as well as ependymal and arachnoid cells were immunoreactive for LTC4 both in the control and after SAH, whereas astrocytes and oligodendrocytes were not immunoreactive for LTC4 in either case.

    Description / Table of Contents: Résumé Cette étude analyse les causes possibles de pseudarthrose et de retard de consolidation après tentative d'arthrodèse antérieure du rachis cervical et précise les mesures préventives. Les symptômes créés par ces problèmes sont également évalués. Parmi les 443 cas d'arthrodèse antérieure du rachis cervical faits dans notre Institution entre 1965 et 1988, on note 15 pseudarthroses ou fusions retardées. Les causes de ces troubles de consolidation peuvent être divisées en deux catégories principales: défaut d'alignement des corps vertébraux et contention post-opératoire inappropriée. Certains de ces cas étaient symptomatiques, probablement à cause d'une mobilité anormale, d'une compression médullaire secondaire à un glissement postérieur ou une rotation du corps vertébral supérieur, d'une déformation en cyphose, ou d'une anomalie au niveau vertébral adjacent. Les 15 cas ont été traités par une reprise de l'arthrodèse qui a conduit à une fusion osseuse complète. Quatre des cinq cas symptomatiques ont eu une récupération complète après la chirurgie secondaire.
    Notes: Summary Anterior cervical spine fusion was carried out on 443 cases between 1965 and 1988 with pseudarthrosis or delayed union in 15 (3.4%). The causes of these complications were malalignment and faulty postoperative care. Symptoms resulted from abnormal mobility, cord compression due to posterior slip or rotation of the upper vertebral body, kyphotic deformity or a problem at the adjacent level. Four out 5 cases with symptoms recovered successfully after a second operation.
