    Notes: Abstract Since the summer of 1996, scientists from China and Japan have conducted a joint observation of natural cloud-to-ground lightning discharges in the Zhongchuan area that is located close to Qinghai-Xizang (Tibet) Plateau, China. It has been found that the long-duration of intracloud discharge processes, just before the first return stroke, lasted more than 120 ms for 85% of cloud-to-ground flashes in this area, with a mean duration of 189.7 ms and a maximum of 300 ms. We present the results of charge sources neutralized by four ground flashes and two intracloud discharge processes, just before the first return stroke, by using the data from a 5-site slow antenna network synchronized by GPS with 1 μs time resolution. The result shows that the altitudes of the neutralized negative charge for three negative ground flashes were between 2.7 to 5.4 km above the ground, while that of neutralized positive charges for one positive ground flash and one continuing current process were at about 2.0 km above the ground. The comparison with radar echo showed that the negative discharges initiated in the region greater than 20 dBZ or near the edge of the region with intense echoes greater than 40 dBZ, while positive discharge initiated in the weak echo region.

    Notes: Abstract Scrapie is a transmissible neurodegenerative disease which shares some characteristics with Alzheimer disease (AD). Recent studies show abnormal enlargement of the adrenal glands and kidneys in 139H-affected hamsters. Using immunocytochemical techniques with antibodies to corticotropin-relasing factor (CRF) and vasopressin (VP), we observed the following: (1) a significantly higher number of CRF-immunostained neurons in the preoptic nucleus of hypothalamus of 139H-affected hamsters than controls; (2) the area of VP-immunostained (ir-VP) neurons in the lateral hypothalamus, which includes the internuclear group of magnocellular neurons and the nucleus circularis, was significantly lower for 139H-affected hamsters than for controls; and (3) no significant difference between 139H-affected and control hamsters with regard to the number of ir-VP neurons in the dorsalmedial hypothalamus (DMH), including the paraventricular hypothalamus, or the supraoptic nuclei. However, the population of ir-VP neurons in the DMH shifted to the anterior part of the hypothalamus in 139H-affected hamsters. Three-dimensional models of the immunostaining were prepared and these provide clear depictions of the changes noted. The changes in the CRF and VP systems in 139H-affected hamsters suggest that the neuroendocrine system can be affected by unconventional slow infections.

    Notes: Summary Nimodipine pharmacokinetics was investigated in 12 Chinese patients with acute subarachnoid haemorrhage receiving an IV infusion of 1.6 or 2 mg/h (based on estimated body weight) for 10 days. Peripheral venous blood samples were collected for up to 4 days and plasma nimodipine was assayed by GC/ECD. The mean value was taken as the steady state concentration (Css) and Clearance (CL) (hourly dose/Css) was calculated. Eight survivors were given oral nimodipine (60 or 90 mg) every 6h (based on body weight), blood was sampled over 6 h and the plasma nimodipine level determined. The values for Css, CL and CL·kg−1 were 33.5 μg·l−1, 58 l·h−1 and 1.0 l·h−1·kg−1 respectively; in survivors receiving the drug orally, bioavailability of the 30 mg tablet was 9%. In one very sick patient given crushed tablets by naso-gastric tube, the AUC was very low; in vitro studies indicated that adsorption of nimodipine by the tubing was unlikely to have been the cause. The pharmacokinetic findings in Chinese patients are comparable to previously reported values in Caucasians.

    Notes: The two-photon, laser-assisted reaction between Xe and Cl2 has been used to prepare XeCl (B,C) molecules in Xe and in Xe mixtures with lighter rare gases for kinetic studies of the coupled XeCl (B,C) states. The XeCl (B–X) spectra were used to assign the conditions for which the vibrational temperature was nearly 300 K. Relative intensity and coupled radiative lifetime measurements provide independent data which support an energy separation between the XeCl(B) and XeCl(C) states of ∼100 cm−1. The two-body quenching rate constant in Xe for XeCl (B,C) was measured as 5+3−2×10−12 cm3 s−1, and a three-body quenching rate constant of 13±4×10−31 cm6 s−1 is indicated. Experiments also were done with added Kr, Ar, Ne, and He to establish two-body and mixed (e.g., Xe+Ar) three-body quenching rate constants. The three-body quenching rate constants are nearly equal to the Xe2Cl* formation rate constants and three-body dark quenching of XeCl(B,C) is negligible.

    Notes: Objective  To determine the effectiveness of laparoscopic uterine nerve ablation (LUNA) for chronic pelvic pain in women with endometriosis and women with no laparoscopic evidence of endometriosis.Design  A prospective double-blind randomised controlled trial (RCT).Setting  Single-centre, secondary-level gynaecology outpatient service and tertiary-level pelvic pain and endometriosis outpatient service in Auckland, New Zealand.Population  One hundred and twenty-three women undergoing laparoscopy for investigation and management of chronic pelvic pain, 56 with no laparoscopic evidence of endometriosis and 67 with endometriosis.Methods  Women were randomised from the two populations, firstly those with no evidence of endometriosis and secondly those undergoing laparoscopic surgical treatment for endometriosis, to receive LUNA or no LUNA. Participant and assessor blinding was employed. Follow up for pain outcomes was undertaken at 24 hours, 3 months and 12 months.Main outcome measures  Changes in non-menstrual pelvic pain, dysmenorrhoea, deep dyspareunia and dyschezia were assessed primarily by whether there was a decrease in visual analogue score for these types of pain of 50% or more from baseline and additionally whether there was a significantly different change in median visual analogue score. The numbers requiring further surgery or starting a new medical treatment for pelvic pain and complications were also measured.Results  There was a significant reduction in dysmenorrhoea at 12 month follow up in women with chronic pelvic pain in the absence of endometriosis who underwent LUNA (median change in visual analogue scale (VAS) from baseline −4.8 versus−0.8 (P= 0.039), 42.1%versus 14.3% experiencing a successful treatment defined as a 50% or greater reduction in visual analogue pain scale for dysmenorrhoea (P= 0.045). There was no significant difference in non-menstrual pelvic pain, deep dyspareunia or dyschezia in women with no endometriosis undergoing LUNA versus no LUNA. The addition of LUNA to laparoscopic surgical treatment of endometriosis was not associated with a significant difference in any pain outcomes.Conclusions  LUNA is effective for dysmenorrhoea in the absence of endometriosis, although there is no evidence of effectiveness of LUNA for non-dysmenorrhoeic chronic pelvic pain or for any type of chronic pelvic pain related to endometriosis.
