    Notes: Abstract. Objective: The purpose of this study was to analyze the biochemical characteristics of synovial fluids after treatment of patients with rheumatoid arthritis (RA) with intra-articularly injected hyaluronate (HA).¶Treatment: In a double-blind study, 13 patients received intra-articular injections of 1% HA in phosphate buffered saline (PBS) while 13 other patients were injected 0.01% HA in PBS, as the control group. Injections were administered once a week for five weeks.¶Methods: Clinical efficacy and characteristics of synovial fluid were compared between groups.¶Results: Significant clinical improvement was observed in the HA-treated group compared to the control group. The viscosity of retained synovial fluid increased. Stringing and HA concentration increased significantly whereas the concentrations of protein and chondroitin sulfate 4 and 6 decreased significantly. The HA molecular weight remained the same in both groups.¶Conclusion: Intra-articular HA injection altered the properties of synovial fluid and proved efficacious for patients with RA.

    Notes: Abstract. This study was performed to evaluate the correlation between serum biochemical marker levels of bone metabolism and bone mineral density (BMD) in patients with ossification of the posterior longitudinal ligaments (OPLL). In addition, the efficacy of these markers in the prediction of the progression of OPLL was also examined. In 40 patients with OPLL, and in age- and gender-matched controls (n= 36), the levels of C-terminal extension peptide of type I procollagen (PICP) and intact osteocalcin were quantitated by a sandwich enzyme-linked immunosorbent assay as serum markers of bone formation. BMD of the whole body was measured using dual-energy X-ray absorptiometry. Serum PICP and intact osteocalcin levels were significantly increased in patients with OPLL. There was a significant correlation between serum bone formation marker levels and BMD in both the OPLL and the control groups. Patients with radiological progression of OPLL showed a level higher than the mean + 2 SD of controls in PICP, intact osteocalcin, and BMD. In addition, the levels of serum PICP and intact osteocalcin increased in correspondence with the progression of OPLL without statistical significance. In patients without any growth of OPLL, however, all the parameters were below the mean + 2 SD of controls. Thus, aggravation of bone formation activity was suggested to be closely correlated with the development of OPLL. In addition, serial measurements of these parameters might be useful for estimating the progression of OPLL.

    Notes: Abstract Time-related changes of laminectomy-induced cauda equina adhesions were investigated by MRI in ten patients with degenerative spinal disease who underwent posterior surgery to the lumbar spine; seven had disc herniations and three spinal stenosis. Axial MRI was performed before and 3, 7, 21 and 42 days after surgery. Cauda equina adhesions were most severe at the laminectomised levels L3–4, L4–5 and L5–S1 (n = 16); partial adhesions were found in 9 of 16 levels at 6 weeks after surgery. At the L3–4 or L5–S1 levels (n = 14), the area of laminar exposure without laminectomy, the cauda equina adhesions continued 1 week after surgery, but thereafter resolved; only partial adhesions were seen at 5 of 14 levels 6 weeks after surgery. Shrinkage of the arachnoid sac was also found at the level of the laminectomy, but it re-expanded 3 weeks after surgery in all cases. Cauda equina adhesions and shrinkage of the sac were correlated closely with laminectomy, with or without discectomy, suggesting that an inflammatory process of deep wound healing may be involved in the mechanism of a laminectomy-induced arachnoradiculitis which may be correlated with postoperative leg symptoms.

    Description / Table of Contents: Résumé. Le cas d’un homme de 50 ans atteint d’histiocytome fibreux malin secondaire à une dysplasie fibreuse asymptomatique du fémur proximal est rapporté. Les anomalies caractéristiques de la radiographie était une région ostéolytique maldéfinie adjacente à la lésion avec une apparence de verre dépoli. L’histologie a montré l’histiocytome fibreux malin concomitant à la dysplasie fibreuse. Une résection de la tumeur et une chimiothérapie étaient pratiquées, mais le patient est mort de métastase hépatique neuf mois après l’intervention chirurgicale. La possibilité d’une malignité secondaire doit être considérée si un patient atteint d’une dysplasie fibreuse a des symptômes progressifs.
    Notes: Summary. A man of 50 years of age had a malignant fibrous histiocytoma secondary to asymptomatic fibrous dysplasia of the proximal femur. Radiographs showed ill-defined osteolytic changes near to the fibrous lesion which had a ground glass appearance. The histology was of a malignant fibrous histiocytoma concomitant with fibrous dysplasia. An extensive resection of the tumour was carried out and chemotherapy given, but the patient died from a liver metastasis 9 months later. The possibility of malignancy should be considered if a patient with fibrous dysplasia has any progressive symptoms.

    Description / Table of Contents: Résumé  L’armature MC a été mis au point pour le traitement de patients souffrant d’une arthrite rhumatoide (AR) qui se présente avec une protrusion acétabulaire, en tant que bague de renforcement métallique acétabulaire d’une cavité articulaire à revêtement poreux sans ciment Harris-Galante (HG). Depuis 1991, cette nouvelle bague de support avec greffon osseux autogène à paroi médiale a été utilisé pour traiter la protrusion acétabulaire de patients ayant une AR. Nous avons réexaminé 15 hanches de 13 patients AR sur une moyenne de 53,6 mois, s’étalant sur une plage de 24 à 84 mois après l’opération. Les résultats cliniques de l’arthroplastie totale de la hanche utilisant cette bague de support furent satistaisants pour tous les cas. Aucun cas ne montra de radiotransparence entre le greffon osseux et la paroi acétabulaire. Tous les greffons osseux se présentaient comme étant tomographiquement réunis, et la protrusion n’a pas progressé
    Notes: Abstract  The MC cup supporter is an acetabular reinforcement ring for the Harris-Galante (HG) cementless porous-coated socket. This supporting ring has been used with autogenous bone graft since 1991 in the management of protrusio acetabuli in patients with rheumatoid arthritis (RA). We have reviewed 15 hips in 13 patients at an average of 53.6 months (range 24–84 months) after operation. The clinical results of total hip arthroplasty (THA) in protrusio acetabuli using this ring were satisfactory. All grafts appeared to have united radiologically with no radiolucent lines between graft and acetabulum.
