    Notes: The treatise “De morbis artificum diatriba” (Modena, 1700) is considered to be the first text to specifically deal with occupational illnesses. It was also the last for over 150 years. Written by Bernardino Ramazzini (Carpi, 1633-Padua, 1714), a professor at the University of Padua from 1700 to 1714, the book highlights the importance given at the time to headache as an occupational symptom. Among the 69 professions described, accounting for the majority of the occupations of the period, 12 were found to lead to headache as an important symptom caused by work. Ramazzini appears to have paid more attention to this than we do today. Ramazzini's work opens up a wide view on social conditions in the 18th century, as his sensitivity for occupational hazards was exceptional. His remarks on headache are typical of his way of collecting first-hand experience of working conditions, and they underline the importance of occupational hazards in the assessment of headache, today just as in 1710.

    Notes: Abstract: Protein synthesis rates were measured (33 days postoperatively) in rats with portacaval shunts and in unoperated controls. In brain, no change in the rate of protein synthesis was evident in shunted rats. These data thus do not support the hypothesis that an inhibition of brain protein synthesis is a factor in the etiology of hepatic encephalopathy. The synthesis rate in forebrain at 82 days of age was 0.52%/h. Though brain wet weight was the same in both groups, rats with shunts grew relatively slowly, and their testicles probably decreased in weight. However, no inhibition of muscle, liver, or testicular protein synthesis could be detected. The mechanism of slower or negative growth in these tissues might thus involve an increase in the degradation rate, although a transient inhibition of synthesis at an earlier period is also possible.

    Notes: Abstract In the present investigation we studied the synaptosomal uptake of glutamate in brain omogenate of Mongolian gerbils submitted to bilateral common carotid occlusion, with and without subsequent return of blood flow. The results show that glutamate uptake after ischemia is reduced by about 35% The damage appears to be persistent, since return of blood flow restores uptake only slightly. The membrane alterations occurring in ischemia could explain the persistence of glutamate transporter impairment. Besides the blockade of NMDA receptors, the stimulation and/or the protection of the uptake systems for glutamate could be of help in preventing neuronal ischemic damage.

    Description / Table of Contents: Sommario Abbiamo esaminato il trattamento farmacologico dell'attacco acuto, usato da 200 pazienti sofferenti di emicrania senza aura (criteri IHS 1988), visitati nel periodo 1989–1991. È stata raccolta una dettagliata analisi farmacologica sulle terapie d'attacco usate dai pazienti nel corso della loro storia cefalalgica precedentemente alla prima visita presso il nostro ambulatorio. Sono state studiate le seguenti variabili: tipo di farmaco usato, dosaggio e via di somministrazione, efficacia del trattamento, frequenza e tipo degli effetti indesiderati; questi parametri sono stati confrontati con le linee guida della Società Italiana per lo Studio delle Cefalee (SISC, 1993). I farmaci più comunemente usati sono gli antiinfiammatori non steroidei; abbiamo osservato un analogo uso delle associazioni, in particolare propifenazone ed acido barbiturico. I pirazolonici propifenazone ed amidopirina, non consigliati nelle linee guida, sono anch'essi largamente usati. Il nostro studio evidenzia il fatto che l'utilizzo corrente dei farmaci differisce in molti aspetti da quello suggerito dalle linee guida.
    Notes: Abstract We assessed the attack drugs taken by 200 migraine without aura patients (International Headache Society criteria, 1988) between 1989 and 1991. A detailed pharmacological history regarding the acute attack therapy adopted up until our initial visit was gathered, including the type of drug used, dosage, administration route, the time of starting therapy, treatment efficacy, and the frequency and types of undesirable effects, all of which were subsequently compared with the guidelines (1993) of the Italian Society for the Study of Headache (SISC). The most commonly used are non steroidal anti-inflammatory drugs (NSAIDs). We observed a similar high frequency in the use of combinations, particularly prophyphenazone and barbituric acid. The pirazolones, such as noramidopyrine and prophyphenazone, are also widely used as single agents, even though they are not considered by the guidelines. Our study underlines the fact that current drug use differs in several respects from the guidelines.

    Description / Table of Contents: Sommario Si ripercorre la storia della Società Italiana di Neurologia (SIN), che “gemma” dalla Società Freniatrica Italiana, le cui origini risalgono a Biagio Gioacchino Miraglia, fondatore ad Aversa nel 1861 della Società Frenopatrica, la prima organizzazione neuropsichiatrica degli scienziati italiani. La Società Freniatrica Italiana rimane società comune ai neurologi e agli psichiatri fino al 1907, quando viene fondata la SIN, che si costituisce con un suo statuto, che sarà poi modificato nel 1971. All'epoca si scinde anche il legame con la psichiatria nella scuola di specialità, che da “Clinica delle Malattie Nervose e Mentali” viene divisa nelle branche distinte della “Neurologia” e della “Psichiatria”. Il primo congresso della SIN si tiene a Napoli nell'Aprile del 1908. Dall'analisi dei temi dei primi congressi si evince come gli interessi dei neurologi, fin dall'inizio, spazino in tutti i campi delle neuroscienze: neuroanatomia, istologia, fisiopatologia, clinica. Nel tempo si organizzano delle sezioni speciali, in modo da approfondire tematiche specialistiche e si ha l'organizzazione della società dei neurologi ospedalieri (SNO).
    Notes: Abstract This paper describes the history of the Italian Society of Neurology (Società Italiana di Neurologia, SIN), originally an offshoot of the Società Freniatrica Italiana, whose origins go back to Aversa in 1861, when Biagio Gioacchino Miraglia founded the Società Frenopatica, the first neuropsychiatric organisation for Italian scientists. The “Società Freniatrica” was founded in Rome in 1873 and was to bring together neurologists and psychiatrists until 1907, when the SIN was founded with its own Articles of Association (subsequently modified in 1971) and the two specialisations of neurology and psychiatry were recognised as being distinct entities. The first SIN Congress was held in Naples in 1908, and an analysis of its themes and those of subsequent Congresses highlights the fact that, right from the start, the interests of neurologists covered the whole range of the neurosciences: neuroanatomy, histology, physiopathology and clinical neurology. Over the years, special Groups have been established to enable the more profound study of specialised aspects, and the Society of Hospital Neurologists founded.

    Description / Table of Contents: Sommario Nel ratto portatore cronico di anastomosi porto-sistemica sono state osservate notevoli alterazioni dei profili aminoacidici plasmatici e cerebrali. Modificazioni analoghe sembrano svolgere un importante ruolo anche nella patogenesi dell'encefalopatia porto-sistemica umana. L'arterializzazione del fegato dopo anastomosi porto-sistemica si è mostrata in grado di migliorare notevolmente le alterazioni biochimiche suddette nel nostro modello sperimentale.
    Notes: Abstract Altered plasma and brain amino acid patterns have been observed in the rat after portacaval shunt. Similar alterations are likely to play an important role in hepatic encephalopathy in humans. In our model, liver arterialization remarkably improves the pathologic amino acid levels following portacaval shunt.

    Description / Table of Contents: Sommario Abbiamo osservato la comparsa di crisi epilettiche in conseguenza alla sospensione di un trattamento con Mianserina alla dose di 60 mg. al giorno per la durata di 2 anni. A nostra conoscenza questa è la prima segnalazione di crisi convulsive insorte dopo sospensione di un farmaco antidepressivo. Abbiamo osservato questo fenomeno in una giovane donna, senza precedenti epilettici o di malattie organiche epilettogene, dieci giorni dopo la brusca interruzione di un protratto trattamento con la Mianserina.
    Notes: Abstract Seizures were observed following the withdrawal of mianserin 60 mg/day for 2 years. To our knowledge, this is the first report of convulsions after suspension of an antidepressant. This phenomenon was observed in a young woman (with no history of epilepsy or other organic diseases that may induce seizures) 10 days after abrupt discontinuation of long-term treatment with mianserin.
