    Notes: Abstract. Computerized techniques (CT and MRI) allow precise delineation of orbital anatomy and abnormalities. Orbital tumors are nicely depicted by these methods; various examples are illustrated in this article, with discussion of the respective impact of CT and MRI. Orbital inflammation and foreign bodies usually represent radiologic emergencies, prompting use of CT (frequently) or MRI (occasionally). Digital subtraction angiography (DSA) is indicated for diagnosis of vascular changes (mainly carotid-cavernous fistula, aneurysms, angiomas, Rendu-Osler disease). Angiography is usually done to ascertain the possibility of an interventional procedure. Orbital vascular interventions include re-canalization of occluded vessels, and embolization of pathologic (tumorous or post-traumatic) vessels.

    Notes: Summary We present the results of a series of 105 patients with cerebral arteriovenous malformations (AVMs) who were treated by a combined protocol including endovascular occlusive measures and open surgical resection. 25 patients were treated only by surgical intervention, 72 patients underwent preoperative endovascular embolization and 8 patients were treated only by embolization, seven of which were only treated for palliation. After superselective angiography the vascular territories suitable for endovascular or microsurgical approach were defined, and in most cases these territories were complementary to each other. In 56 cases, only one embolization was necessary and due to an advantageous co-localization of the departments the whole combined endovascular/neurosurgical procedure was done in one anesthesia. If several endovascular sessions were necessary (16 patients), the resection was mostly carried out immediately after the last neuroradiological session in the same anesthesia with total time of such combined procedure now averaging about 7 hours. According to the proposed grading system by Spetzler we treated 25 grade 1, 24 grade 2, 40 grade 3, 11 grade 4, and 5 grade 5 lesions. The overall success rate defined as complete resection without additional permanent neurological deficit was 89.6% (87 out of 97 surgical cases). The benefits of such combined approach to cerebral AVMs become apparent in shortened and safer surgical procedures as well as in a low complication rate.

    Notes: Summary A group of 59 patients with stroke due to acute vertebrobasilar or carotid territory occlusion have been treated by local intra-arterial fibrinolysis (LIF). A high recanalisation rate was accomplished with either urokinase or recombinant tissue plasminogen activator (r-TPA). However, with either substance, even if a high dose was used, recanalisation was a time-consuming process which usually took 120 min. A reasonable explanation for the lack of effectiveness of these plasminogen-activating substances might be a deficit of substrate, e.g. plasminogen, in aged thrombus. LIF was capable of improving clinical outcome in acute vertebrobasilar artery occlusion, reducing mortality to 50% in patients fulfilling inclusion criteria. In the carotid territory multiple occlusions had a poor prognosis while good clinical results could be achieved in occlusions of the proximal middle cerebral artery or single branches.

    Notes: Abstract Acute necrotising encephalopathy of childhood (ANE) is an uncommon disease which predominantly affects infants and young children living in Japan and Taiwan. A multifocal encephalopathy with symmetrical lesions in the thalamus, tegmentum of the brain stem, cerebral periventricular white matter and cerebellar medulla is characteristic. We present the imaging features in a 4-year-old Japanese boy who had been living in Germany for 21/2 years before presentation.

    Notes: Abstract Our purpose was to assess the haemodynamic changes in the ipsilateral middle cerebral artery (MCA) during and after percutaneous transluminal angioplasty (PTA) of the internal carotid artery (ICA), and to compare them with clinical and angiographic findings. Transcranial Doppler Sonographic monitoring (TCD) of the MCA was performed during PTA in 22 patients with symptomatic severe stenosis of the ICA. Mean blood flow velocity (MBFV) and pulsatility index (PI) were recorded. During PTA, MBFV fell from 41 ± 15 cm/s to 23 ± 11 cm/s (P = 0.0001). Changes in PI were inconsistent. With reduction of MBFV of 50 % or less (in 10 cases) no complication occurred. With a reduction of more than 50 % (in 12), 6 patients developed neurological disturbances (transient ischaemic attacks in 5 and minor stroke in 1). This difference was significant (P = 0.0152). Symptomatic patients also had a higher rate of stroke prior to PTA (4/6) than patients who remained asymptomatic during PTA (0/6). After PTA had been performed MBFV and PI improved significantly (P = 0.0001), MBFV increasing to 48 ± 16 cm/s and PI from 0.64 ± 0.11 to 0.86 ± 0.15. TCD changes proved more sensitive to cerebral haemodynamics than angiography in 8 patients.

    Notes: Abstract Tay's syndrome is a trichothiodystrophy associated with congenital ichthyosis. We report the findings on MRI and spectroscopy in a young girl with sparse, short, ruffled hair, dry skin and delayed milestones. T2-weighted images showed prominent diffuse confluent increase in signal symmetrically in all the supratentorial white matter. These findings are similar to those in a previously described case, and consistent with dysmyelination. Spectroscopy showed increased myoinositol and decreased choline.
