Zusammenfassung Wir untersuchten die Wirkung von Amitryptilin, Theophyllin und Furosemid auf c-AMP-Konzentration und PRA im Nierenvenenblut. Zusätzlich wurde die Stimulierbarkeit der PRA durch Furosemid nach vorheriger Gabe der beta-adrenergen Rezeptor-Antagonisten Propranolol und Practolol und die Konzentration von zyklischem AMP im Plasma bestimmt. Die Substanzen wurden intravenös verabfolgt. Nach Amitryptilin steigt die c-AMP-Konzentration um das 1,5fache des Ausgangswertes an, die PRA bleibt unbeeinflußt. Nach Theophyllin wird die c-AMP-Konzentration um das 1,2fache des Ausgangswertes erhöht, die PRA um das 2fache. Nach Furosemid fällt die c-AMP-Konzentration im Verlauf von 15 min kontinuierlich bis auf 70% des Ausgangswertes ab, die PRA ist bereits nach 7 min um das 2fache erhöht und wird bis zur 15. min nicht verändert. Durch Practolol werden c-AMP-Konzentration und PRA innerhalb von 10 min gleichzeitig um zirka 20% des Ausgangswertes vermindert. Die nachfolgende Gabe von Furosemid läßt die Stimulierbarkeit der PRA unbeeinflußt (Anstieg der PRA auf das 2fache), die c-AMP-Konzentration wird nicht weiter verändert. Nach Propranolol fallen c-AMP-Konzentration und PRA um etwa 20% des Ausgangswertes ab. Die c-AMP-Konzentration bleibt durch die nachfolgende Furosemid-Gabe unbeeinflußt, die PRA kann durch Furosemid nur bei 36% der mit Propranolol behandelten Patienten stimuliert werden. Unsere Ergebnisse zeigen, daß Veränderungen der Plasmakonzentration von c-AMP und der PRA im Nierenvenenblut zwei voneinander unabhängige Vorgänge widerspiegeln. Die Erhöhung des intrazellulären c-AMP-Spiegels mittels Phosphodiesterase-Hemmung durch Amitryptilin führt allein zu keiner Steigerung der PRA. Es ergeben sich keine Hinweise, daß das Adenylzyklase-System direkt oder zwangsläufig am Mechanismus der Reninsekretion beteiligt ist. Die Wirkung von Propranolol und Practolol auf die Ausgangswerte von PRA und c-AMP-Konzentration im Plasma sind gleich stark ausgeprägt. Der unterschiedliche Einfluß von 10 mg Propranolol i.v. und 20 mg Practolol i.v. auf die Stimulierbarkeit der PRA durch 40 mg Furosemid ist wahrscheinlich ein Dosisproblem.

Summary The influence of amitryptiline, theophylline and furosemide on the concentration of cyclic-AMP and plasma renin activity (PRA) was investigated in renal vein plasma. Additionally, the stimulating effect of furosemide on the PRA after application of the beta-adrenergic receptor antagonists propranolol and practolol and the cyclic AMP concentration in the plasma were measured. All drugs were given intravenously. After amitryptiline cyclic-AMP concentration increased about 1.5-fold compared with the basal value, PRA was not altered. After theophylline cyclic-AMP concentration increased about 1.2-fold, PRA 2.0-fold compared with the basal value. After furosemide cyclic-AMP concentration decreased continuously within 15 min about 30% compared with the basal value, PRA increased within 7 min and no further increase was observed till the 15th min. After practolol cyclic-AMP concentration and PRA decreased about 20% compared with the basal value within 10 min. The stimulating effect of subsequently applied furosemide on PRA was not altered, but the cyclic-AMP concentration was not changed in this time by furosemide. After propranolol cyclic-AMP concentration and PRA decreased about 20% compared with the basal value. The cyclic-AMP concentration was not influenced by the following furosemide application, in agreement with the findings after practolol, however PRA could be stimulated only in 36% of these patients under beta-receptor blockade. Our results show that changes of the concentrations of cyclic-AMP and of PRA are independent of each other. An elevated intracellular cyclic-AMP level due to the inhibition of phosphodiesterase is as a single factor unable to stimulate renin release. Our results give no evidence of a direct involvement of the adenylcyclase-system in the mechanism of renin release. The effect of propranolol and practolol on the basal value of PRA and cyclic-AMP is equal. The different influence of 10 mg propranolol and 20 mg practolol on the stimulating effect of 40 mg furosemide on the PRA can be interpreted as a dosage problem.
