    Description / Table of Contents: Zusammenfassung An 42 Kniegelenkspunktaten verschiedenster rheumatischer Erkrankungen wurden enzymcytochemisch die saure Phosphatase, Beta-Glukuronidase, Alpha-Naphthylacetatesterase, Peroxydase und PAS-Reaktion in den Zellen der Synovia bestimmt und nach einem semiquantitativen Verfahren ausgewertet. Bei arthritischen Gelenkergüssen war der Aktivitätsindex der sauren Phosphatase und Beta-Glukuronidase in den Lymphozyten erhöht. Dieser Befund wird im Hinblick auf eine mögliche in vivo Stimulation bzw. Transformation der Lymphozyten diskutiert. In den Monozyten fiel bei Arthritiden eine Erhöhung des Aktivitätsindex der Alpha-Naphthylacetatesterase und Erniedrigung der Beta-Glukuronidase auf.
    Notes: Summary Acid phosphatase, ß-glucuronidase, alpha-naphthylacetatesterase, peroxidase and PAS-reaction were investigated by cytochemical enzyme reactions in synovial fluid cells of 42 samples from knee joints with various rheumatic diseases. These were calculated by a semi-quantitative method. The activity index of acid phosphatase and ß-glucuronidase was increased in lymphocytes of inflammatory synovial fluids. This finding is discussed in view of a possible in vivo stimulation or transformation of the lymphocytes. Concerning the monocytes there was an increase of the activity index of alpha-naphthylacetatesterase and a decrease of ß-glucuronidase in arthritic joint diseases.

    Notes: Summary In a 48-week, double-blind trial, 122 patients were randomly assigned to treatment with auranofin (60) and gold sodium thiomalate (GST) (62) at five centers. Both groups showed significant improvement (P〈0.05) from baseline in parameters of disease activity. Results of the covariance analysis for all patients who completed the trial showed no significant differences (P〈0.05) in efficacy between the two groups. The proportions of patients showing 50% or greater improvement in tender joints, swollen joints, activity index, severity of pain, general health rating, and erythrocyte sedimentation rate (ESR) were similar for both auranofin-treated and GST-treated patients who completed the 48-week trial. When all patients who entered the trial were evaluated, a slightly greater proportion of patients on auranofin had improved. Diarrhea occurred more frequently with auranofin (32%) compared to GST (19%), whereas rash and pruritus were twice as common in those patients treated with GST compared to those treated with auranofin. The withdrawal rate due to adverse reactions was 10% for auranofin vs 26% for GST. It was concluded that the efficacy of auranofin was comparable to that of injectable gold and was better tolerated, as evidenced by the lower withdrawal rate from adverse events for the auranofin patients.

    Description / Table of Contents: Zusammenfassung Zielsetzung: Ziel der Studie war die Übersetzung und die Validierung einer deutschen Version des Health Assessment Questionnaire Disability Index (HAQ) und der Vergleich seiner psychometrischen Eigenschaften mit der Version-P und der Version-PR des Funktionsfragebogens Hannover. Methoden: Nach der Übersetzung wurde die Test-Retest-Reliabilität bei 32 ambulanten Patienten der Abteilung Rheumatologie der Medizinischen Hochschule Hannover mit Hilfe der Pearson-Korrelation überprüft. Eine Woche nach der ersten Beantwortung wurde den Patienten der Fragebogen erneut zugesandt. Zur Validierung wurden 110 stationären Patienten in 3 verschiedenen Kliniken vorgelegt. Alle Patienten erfüllten die Kriterien des American College of Rheumatology von 1987 oder die Rom-Kriterien der inaktiven chronischen Polyarthritis (cP). Zur Beurteilung der Kriteriumsvalidität zogen wir den Keitel-Funktionstest (KFT) und die modifizierte Steinbrocker-Klassifikation (mSK) der funktionellen Behinderung heran. Die interne Konsistenz wurde mit Hilfe des Cronbach Koeffizienten Alpha (CKA) bestimmt. Zur Erfassung der Konstruktvalidität wurden die HAQ- und FFbH-Scores mit verschiedenen klinischen und laborchemischen Variablen verglichen und zur Aufklärung der Varianz multiple Regressionen berechnet. Ergebnisse: Die Test-Retest-Reliabilität des HAQ betrug r=0,94. Es zeigte sich ein CKA von 0,91 (HAQ), 0,90 (FFbH-P) und 0,93 (FFbH-PR). Im Vergleich zum KFT ergab sich ein Korrelationskoeffizient von r=–0,73 (HAQ), r=+0,74 (FFbH-P) und r=+0,71 (FFbH-PR) im Vergleich zur mSK ein r=+0,75 (HAQ), r=–0,72 (FFbH-P) und r= –0,70 (FFbH-PR). Der Korrelationskoeffizient des HAQ/FFbH-P betrug r=–0,87 bzw. r=–0,88 beim HAQ/FFbH-PR. Die Korrelationen zwischen den klinischen und laborchemischen Parametern reichten von r=±0,58 (Schmerz/HAQ) bis r=±0,11 (Anzahl der geschwollenen Gelenke/FFbH-PR). In multiplen, rückwärtsgerichteten Regressionsanalysen fanden sich r2-Werte von 0,64 (HAQ), 0,61 (FFbH-P) und 0,59 (FFbH-PR). Der Schmerz erklärte etwa 1/3 und die Bewegungseinschränkung 16–21% der Varianz. Schlußfolgerung: Die hier vorgestellte deutsche Version des HAQ und die zwei verschiedenen Versionen des FFbH erwiesen sich als reliable und valide Instrumente zur Messung der funktionellen Behinderung in einem deutschsprachigen Patientenkollektiv mit cP. Das mit dem HAQ und den beiden Versionen des FFbH gemessene Konstrukt zeigte eine hohe Übereinstimmung.
    Notes: Summary Objective: To translate the Health Assessment Questionnaire Disability Index (HAQ) into a German version, to validate and to compare its properties with two different versions of the Hannover Functional Ability Questionnaire (HFAQ) in a German speaking population. Methods: The test-retest reliability was tested by Pearson correlation in 32 outpatients of the Department of Rheumatology of the Medizinische Hochschule Hannover. For retesting, the questionnaire was mailed to them 1 week later. To validate the questionnaire it was administered to 110 inpatients in three different hospitals. All patients fulfilled the American College of Rheumatology 1987 revised criteria of rheumatoid arthritis (RA) or the Rome criteria of definitive inactive RA. The internal consistency was measured by Cronbach‘s coefficient alpha (CCA). To assess criterion validity we compared the HAQ and the two versions of the HFAQ with Keitel‘s test (KT) and the modified Steinbrocker classification (mSC). Construct validity was assessed by comparing these instruments with different clinical and laboratory variables. A multivariate analysis was used to identify the most important factors that are influencing the HAQ- and HFAQ-scores. Results: Test-retest reliability of the HAQ was r=0.94. CCA was 0.91 (HAQ), 0.90 (HFAQ-P) and 0.93 (HFAQ-PR). The KT Pearson correlation coefficients reached r=–0.73 (HAQ), r=+0.74 (HFAQ-P) and r=+0.71 (HFAQ-PR). The mSC correlated r=+0.75 (HAQ), r=–0.72 (HFAQ-P) and r=–0.70 (HFAQ-PR). The correlation coefficients of HAQ/HFAQ-P was r=–0.87 and of HAQ/HFAQ-PR r=–0.88. The correlations between other clinical and laboratory variables reached from r=±0,58 (pain/HAQ) to r=±0,11 (number of swollen joints/HFAQ-PR). In backward multiple regression analysis 59–64% of the variance of disability measured by the questionnaires was explained predominantly by pain (32–33%) and by range of motion (16–21%). Conclusion: The German version of the HAQ presented here and the two versions of the HFAQ are reliable and valid instruments for measuring functional disability in a German-speaking population with RA. The construct measured by the HAQ and both versions of the HFAQ showed a high degree of correspondence.

    Description / Table of Contents: Summary In 1998 the German Federal Ministry of Education and Science launched a competition for major comprehensive grants with the ultimate goal of forming networks to link patient care, education and research. There were over 150 applications; 9 of which were successful. Among the winners was our proposal to create a federal medical network in rheumatology („Kompetenznetz Rheuma”) and we are pleased to announce its official start here with the presentation of the abstracts from our first network symposium in Berlin. Our endeavor is aimed at bringing together doctors in clinical practice, patients and scientists to improve patient care in rheumatology. Therefore, this network is made up of various components, namely documentation, patient care and new diagnostic efforts. Particular care is being devoted to take scientific work from the bench to the bedside. The rheumatology medical network is organized and further developed by six regional centers of competence located in Berlin, Düsseldorf, Erlangen, Freiburg, Hannover and Lübeck/Bad Bramstedt. Of course, the network is designed as an open system. We invite everyone, who is interested to participate in our work. Detailed information on our projects and our goals can be gained from the persons involved and on our website (http://www.rheumanet.org/).
    Notes: Zusammenfassung Im Namen aller Beteiligten möchten wir Ihnen das Kompetenznetz Rheuma vorstellen. Dieser Artikel soll Ihnen einen ersten Einblick in die Aufgaben, Strukturen und Ziele unseres Projektes geben. Das Kompetenznetz Rheuma geht auf eine Initiative des Bundesministeriums für Bildung und Forschung zurück. Unter mehr als 150 Antragstellern hat das Ministerium im Rahmen eines Wettbewerbs neun Kompetenznetzwerke in der Medizin für eine spezielle Förderung ausgewählt. Das Ziel war, die in Deutschland vorhandene, aber bundesweit zersplitterte Kompetenz zu gesundheitspolitisch bedeutenden Krankheitsbildern zu bündeln und dadurch effizienter zu gestalten. Das Kompetenznetz Rheuma ist einer der Gewinner und wird über einen Zeitraum von bis zu fünf Jahren mit bis zu 5Mio. Mark jährlich unterstützt.    Im Kompetenznetz Rheuma schließen sich die verschiedenen Ebenen der Rheumatologie zusammen: die Grundlagenforschung, die Versorgungsforschung, die Universitätskliniken, die rheumatologischen Krankenhäuser und Rehabilitationskliniken, die niedergelassenen Rheumatologen und die Hausärzte. Der Wissens- und Informationstransfer untereinander wird durch horizontale und vertikale Vernetzung sichergestellt. Ein wesentliches Ziel besteht darin, die Wege von der Laborbank bis zum Krankenbett zu verkürzen. Denn der enge Verbund von Grundlagenforschung, Krankenbetreuung und Dokumentation bietet einzigartige Möglichkeiten, neue Forschungsergebnisse zügig in die Patientenversorgung zu integrieren und bestehende Defizite zu erkennen und auszuschalten.    Getragen und weiterentwickelt wird das Kompetenznetz Rheuma von den sechs antragstellenden rheumatologischen Kompetenzzentren in Berlin, Düsseldorf, Erlangen, Freiburg, Hannover und Lübeck/ Bad Bramstedt. Das Netzwerk soll aber kein in sich geschlossenes Projekt bleiben. Alle Interessierten sind herzlich eingeladen, sich daran zu beteiligen.     Description / Table of Contents: Summary Objective:To test if continuous LDPT decreases radiographically detectable joint destruction in early RA. Methods: Patients with active RA (〈 2 years from symptom onset) were treated with prednisolone 5 mg daily or placebo for 2 years in a double blind, randomized, multi-center study. At the same time in all patients DMARD treatment with either gold sodium thiomalate (GSTM) or methotrexate (MTX) was started; in the case of side effects or inefficacy the medication could be switched to the other DMARD. Radiographs of hands and forefeet were taken at baseline and after 6, 12 and 24 months. All radiographs were evaluated by one observer (S. W.) knowing the time sequence of the film but unaware of the patient identity or treatment using the Ratingen score and van der Heijde's modification of Sharp's method. Results: 196 patients were included; 76 patients completed the study per protocol. Of these patients 34 were treated with prednisolone, 42 with placebo, 48 initially with GSTM, 28 with MTX. 17 patients switched from GSTM to MTX, 1 from MTX to GSTM. The mean values of the radiographic scores of both groups are given in the table. During the first year, especially during the first 6 months, the radiographic progression within the prednisolone group was significantly lower than in the placebo group: the Sharp erosion score increased by 0.4% of the maximum possible score during the first and the second 6 months in the prednisolone group, while in the placebo group there was an increase of 1.8% during the first 6 month and 0.8% during the second 6 months. During the second year the progression was significantly lower (−0.1% in the prednisolone group, 0.2% in the placebo group). After 24 months the total score had increased by 2.6% of the maximum score in the placebo group and by 1.1% in the prednisolone group. The results of the completers were confirmed by the intention-to-treat analysis (80 patients in the prednisolone group, 86 patients in the placebo group). Conclusion: Continuous low dose prednisolone treatment with 5 mg daily over 2 years administered in addition to conventional DMARD treat with MTX of GSTM decreases radiographic progression in early RA. The results of the study also show that in the placebo group there is a sharp decrease of the progression after 6–12 months as a result of the DMARD treatment. During the second year there is nearly no progression in this group. The data of the “completers” are confirmed by the analysis of the “intention to treat” population.
    Notes: Zusammenfassung Ziel der Studie: Zu untersuchen, ob eine kontinuierliche, niedrig dosierte Prednisolon-Behandlung die im Röntgenbild erkennbare Gelenkdestruktion bei früher chronischer Polyarthritis verzögert. Methoden: In einer multizentrischen randomisierten Doppelblindstudie wurden Patienten mit einer frühen aktiven c. P. (Krankheitsdauer 〈 2 Jahre), die noch nicht mit Gold oder MTX vorbehandelt worden waren, zwei Jahre lang mit 5 mg Prednisolon täglich oder Placebo behandelt. Bei allen Patienten wurde gleichzeitig eine Behandlung mit parenteralem Gold oder Methotrexat eingeleitet. Das Gold konnte bei Auftreten von Nebenwirkungen oder Unwirksamkeit durch MTX ersetzt werden oder umgekehrt. Röntgenbilder der Hände und Vorfüße wurden zu den Zeitpunkten 0, 6, 12 und 24 Monate angefertigt. Alle Bilder wurden von einem Untersucher (S. W.) in Kenntnis der Reihenfolge, aber ohne Kenntnis der Patientenidentität nach dem Ratingen Score und der van der Heijde Modifikation der Sharp-Methode befundet. Ergebnisse: 196 Patienten wurden eingeschlossen. Die hier berichteten Ergebnisse betreffen 76 Kranke, die die Studie protokollgemäß beendeten. 34 Patienten wurden mit Prednisolon, 42 mit Placebo behandelt. Als Basistherapeutikum hatten 48 anfangs Gold, 28 MTX. 17 Patienten wechselten von Gold auf MTX, einer von MTX auf Gold. Die Mittelwerte der Röntgenscores finden sich in der Tabelle. Die Progression in der Prednisolon-Gruppe war während des 1. Jahres, insbesondere während der ersten 6 Monate, signifikant geringer als in der Placebo-Gruppe: so stieg der Erosions-Score in der Prednisolon-Gruppe während der ersten beiden Halbjahre um je 0,4% des maximal möglichen Scores, in der Placebo-Gruppe im 1. Halbjahr um 1,8, im zweiten um 0,8%. Während des 2. Jahres war die Progression signifikant geringer (−0,1% in der Prednisolon-Gruppe, 0,2% in der Placebo-Gruppe). Nach 24 Monaten war der Gesamtscore in der Placebo-Gruppe um 2,6%, in der Prednisolon-Gruppe um 1,1% angestiegen. Diese Ergebnisse der Completer-Analyse wurden durch die Intention-to-treat Analyse (80 Patienten in der Prednisolon-Gruppe, 86 in der Placebo-Gruppe) bestätigt. Schlussfolgerung: Zusätzlich zur konventionellen Basistherapie mit MTX oder i. m. Gold verzögert eine Prednisolon-Behandlung mit täglich 5 mg die röntgenologisch erkennbare Progression der frühen c. P. signifikant. Die Studie zeigt aber auch, dass nach 6–12 Monaten die Progression unter Basistherapie + Placebo ebenfalls auf minimale Werte reduziert wird. Die hier dargestellten Ergebnisse der Completer werden durch die Analyse der Intention-to-Treat Population bestätigt.

    Description / Table of Contents: Summary HCV-infection is an important infectious disease in rheumatology. It is the cause of mixed cryoglobulinemia and other rheumatic manifestations develop frequently during HCV-infection. These comprise: Sicca-syndrome, thromboembolic events associated with anti-cardiolipin antibodies and fibromyalgia. Also associated with HCV-infection is a non-erosive polyarthritis. This synovitis often fulfills the ACR-criteria for rheumatoid arthritis, but the disease course is different with frequent remissions and non-erosive joint involvement. The following autoantibodies are associated with HCV-infection: Cryoglobulins, rheumatoid factor, anti-nuclear antibodies (ANA), anti-smooth muscle antibodies (SMA), anti-phospholipid-antibodies and anti-thyroid-antibodies. In HCV-associated sicca-syndrom, antibodies against Ro (SSA) and La (SSB) are not detected. The course of HCV-infection is often occult, without elevation of liver enzymes. We summarize the clinical and serological signs and symptoms when HCV-infection should be suspected and when HCV-testing should be performed in a rheumatological setting. The identification of HCV-infection in rheumatic patients is important to minimize the risk of aggravating hepatitis by prescription of hepatotoxic drugs and because of the availability of α-interferon as a potential virus eradicating agent.
    Notes: Zusammenfassung Die Hepatitis C-Virus (HCV)-Infektion ist eine für die rheumatologische Praxis relevante Infektionserkrankung. Sie ist Ursache der gemischten Kryoglobulinämie. Zusätzlich entwickeln viele HCV-Patienten rheumatische Begleitmanifestationen. Rheumatologisch relevante Manifestationen sind: Sicca-Syndrom, thromboembolische Ereignisse bei Vorliegen von Anti-Kardiolipin-Antikörpern und Fibromyalgiesyndrom. Ebenfalls HCV-assoziiert, ist eine zumeist seropositive polyartikuläre Arthritis. Diese erfüllt die klinischen Kriterien der chronischen Polyarthritis, nimmt aber einen anderen Verlauf. Remissionen werden häufig beobachtet, erosive Verläufe sind die Ausnahme. Folgende Autoantikörper sind mit der HCV-Infektion assoziiert: Kryoglobuline, Rheumafaktoren, Anti-Nukleäre-Antikörper (ANA), Antikörper gegen glatte Muskulatur (SMA), Anti-Phospholipid-Antikörper, Schilddrüsenautoantikörper. Antikörper gegen Ro und La sind bei der HCV-Infektion nicht nachweisbar. Da die Leberentzündung zumeist okkult verläuft, ist auch bei normalen Lebertransaminasen eine HCV-Infektion nicht ausgeschlossen und die Indikation zur HCV-Testung gegeben, wenn bestimmte serologische und klinische Hinweise vorliegen. Bei positivem HCV-Nachweis sollte auf hepatotoxische Medikamente verzichtet und die Möglichkeit einer viruseradizierenden Therapie mit α-Inferferon erwogen werden.

    Description / Table of Contents: Summary Borrelia burgdorferi specific DNA has been detected by polymerase chain reaction (PCR) in different specimens of patients with Lyme disease (LD). The aim of the present study is to evaluate PCR-diagnostic of urine specimens regarding rheumatologic diagnosis of Lyme disease. Urine specimens of 77 patients (LD, n=34; undifferentiated arthritis (UA), n=25; arthralgia/myalgia (AM), n=18) and 15 controls were investigated. Flagellin gene (60 specimens) or OspA-plasmid (32 specimens) were used as targets. Sensitivity of the flagellin-nested-PCR was 27%, by OspA-nested-PCR only one positive PCR result was found. Despite of low sensitivity PCR enabled the correct diagnosis of LD in two patients classified as UA. Therefore, PCR can give valuable hints in single cases if LD is clinically suspected.
    Notes: Zusammenfassung Borrelia burgdorferi– spezifische DNA läßt sich mittels Polymerase-Kettenreaktion (PCR) in verschiedenen Untersuchungsmaterialien von Patienten mit einer Lyme-Borreliose (LB) nachweisen. Ziel der vorliegenden Untersuchung ist es, den Wert der PCR-Untersuchungen von Urin im Hinblick auf die rheumatologische Diagnostik der Lyme-Borreliose zu überprüfen. Von 77 Patienten (LB n=34, undifferenzierte Arthritis (UA) n=25, Arthralgien/Myalgien (AM) n=18) und 15 Kontrollpersonen wurden Urinproben untersucht. Als Zielsequenzen dienten Flagellin-Gen (60 Proben) oder OspA-Plasmid (32 Proben). Es fand sich eine Sensitivität der Flagellin-nested-PCR von 27%, mittels OspA-nested-PCR wurde dagegen nur ein positiver Befund erhoben. Trotz formal niedriger Sensitivität konnte bei 2 Patienten mit einer UA erst durch die PCR die Diagnose LB gesichert werden. In Einzelfällen bietet die PCR somit diagnostisch wertvolle Hinweise bei klinischem Verdacht auf eine LB.

    Description / Table of Contents: Summary Cat scratch disease (CSD) is a rarely recognized infectious disease in Germany. Only a few years ago the causative agent, Bartonella henselae, could be isolated. The typical clinical manifestations of CSD consist of skin changes at the inoculation site and a benign lymphadenopathy; other manifestations are rare. We report the case of a 47 year old woman, who developed a reactive spondylarthropathy with synovitis of finger joints, polyarthralgias of large- and medium-sized joints, and inflammatory spinal pain after a cat bite. The rheumatic manifestations resolved after 10 months by treatment with non-steroidal antirheumatic drugs. Only a few cases of rheumatic manifestations associated with CSD have been described in the literature. Because the prevalence of Bartonella henselae infection of cats is high in Europe, rheumatic manifestations might be more frequent. Diagnosis of CSD is now improved by the development of serological tests. We provide an overview of the clinical manifestations and the diagnostic criteria.
    Notes: Zusammenfassung Bei der Katzenkratzkrankheit (KKK) handelt es sich um eine in Deutschland wenig bekannte Infektionskrankheit. Erst vor wenigen Jahren konnte der Erreger, Bartonella henselae, isoliert werden. In der Mehrzahl der Fälle findet sich ein typisches Krankheitsbild mit Hautveränderungen im Bereich von Katzenbiß- oder Kratzwunden und einer benignen Lymphadenopathie. Weniger häufig kommt es zu weiteren Organmanifestationen. Wir berichten über eine 47-jährige Patientin, die nach einem Katzenbiß eine reaktive Spondarthritis mit Synovialitiden der Fingergelenke, Polyarthralgien der mittleren und großen Gelenke und entzündliche Lumbalgien entwickelte. Die rheumatische Symptomatik war zeitlich limitiert und sprach gut auf NSAR an. Bislang sind nur wenige Fälle von Gelenkbeteiligung im Rahmen einer KKK beschrieben worden. Da die Durchseuchung von Katzen mit Bartonella henselae in Europa jedoch hoch ist, könnte es sich um eine häufigere Ursache von Gelenkbeschwerden handeln, als bislang angenommen. Die Diagnostik der KKK wurde in den letzten Jahren durch die Entwicklung serologischer Teste erleichtert. Wir geben einen Überblick über die klinischen Manifestationen und fassen die diagnostischen Kriterien dieser Erkrankung zusammen.
