    Description / Table of Contents: Summary A survey was undertaken of the methods available for injecting contrast media into the vertebral artery. A new, safe and certain method of puncture is described for injecting contrast media into the vertebral artery lateral to the atlanto-occipital joint. The topographical relations between the tip of the mastoid process and the transverse process of the atlas together with the angle of the puncturing cannula were determined by skull measurements and angular computation using a body specific coordination system on the basis of diagrams. Experimental puncture on cadavers is discussed. The clinical procedures are described, beginning with entry in front of the mastoid and above the transverse process of the atlas up to the puncture (from antero-lateral) of the vertebral artery as it runs horizontally on the posterior arch of the atlas. The method is illustrated by figures. No significant complications were observed in any patients.
    Abstract: Résumé Ilest d'abord fait un rappel des méthodes utilisées pour l'injection de produits de contraste dans l'artère vertébrale. Une méthode nouvelle et sûre de ponction par injection de l'artère vertébrale dans son angle latéral atlo-occipital est décrite. Les relations topographiques qui existent entre l'extrémité de la mastoïde et l'apophyse transverse de l'atlas et l'angle de ponction ont été fixées grâce à des mensurations du crâne en utilisant un système de coordination spécifique à base de diagrammes. La ponction expérimentale sur le cadavre a été réalisée. Les procédés sont décrits à partir de la piqûre faite devant la mastoïde et au dessus de l'apophyse transverse de l'atlas jusqu'à la ponction de l'artère vertébrale qui parcourt horizontalement l'arc postérieur de l'atlas. Des figures illustrent la méthode. Aucune complication n'a été observée.
    Notes: Zusammenfassung Es wird ein Überblick über die Methoden zur Kontrastmittelfüllung der A. vertebralis gegeben. Eine neue, ungefährliche und sichere Punktionsmethode zur Kontrastmittelfüllung der A. vertebralis lateral vom Atlanto-Occipitalgelenk wird beschrieben. Die topographischen Beziehungen zwischen Mastoidspitze und Querfortsatz nebst Einstichwinkel der Punktionskanüle werden durch Schädelmessungen und Winkelberechnungen zu einem körpereigenen Koordinatensystem an Hand von Zeichnungen ermittelt. Es werden die Versuchspunktionen an Leichen diskutiert. Die klinische Durchführung der Methode vom Einstich vor dem Mastoid über den Knochenkontakt mit dem Atlasquerfortsatz bis zur Punktion der horizontal auf dem Atlasbogen verlaufenden A. vertebralis von vorn seitlich wird beschrieben. Durch Bilder wird die Methode veranschaulicht. Komplikationen irgendwelcher Art wurden bei keinem Patienten bemerkt.

    Notes: Abstract Halm-Zielke instrumentation (HZI) was developed to eliminate the disadvantages of Zielke instrumentation (VDS) in terms of lack of primary stability and a kyphogenic effect. HZI is an anterior double-rod system. The system is composed of a lid-plate, which is fixed at the lateral aspect of the vertebral body with two screws, a sunk screw anteriorly and a VDS screw posteriorly. The lid-plate design provides the lowest possible implant profile. The longitudinal components consist of a threaded VDS rod and a solid, fluted rod. Correction is performed with the threaded rod and the solid rod. The solid rod allows internal derotation and relordosation, eliminates the Zielke three-point lever system and augments the system. The fluted design of the rod provides rotatory stability. This is a report of the first ten consecutive adolescent idiopathic scoliosis patients in a prospective clinical trial using HZI with a minimum follow-up of 2 years. Curves ranged from 36° to 77°. Correction of the frontal plane averaged 77.5% and 72.2% postoperatively and at follow-up, respectively. Thoracolumbar kyphosis was present in three patients and corrected in all from an average of + 18° to +1.7° at follow-up. Implant-related complications were not observed. All patients were treated without any additional external immobilization. In our opinion, HZI is a major improvement on the original Zielke VDS. It eliminates the kyphogenic effect and provides primary stability.

    Description / Table of Contents: Résumé Analyse rétrospective de 91 courbures dorsales idiopathiques traitées par l'opération V.D.S. de Zielke. L'amélioration obtenue a été de 68.6% en moyenne dans le plan frontal et la correction de la rotation de 50%. Les lordo-scolioses ont été amenées à 15° de cyphose et les hypercyphoses considérablement réduites. Pendant la période d'observation (de 3.6 ans en moyenne) la capacité vitale a été augmentée de 4.1%.
    Notes: Summary This paper reports a retrospective review of 91 patients with idiopathic thoracic scoliosis corrected by Zielke VDS instrumentation. The results were analysed to demonstrate the correction in all three planes. Satisfactory correction was obtained, and was particularly good in the sagittal and horizontal planes. There were no disadvantages from the thoracotomy.

    Description / Table of Contents: Zusammenfassung Bericht über die 26 Fälle, die mit der neuentwickelten Derotationsspondylodese operiert wurden. Die ersten Ergebnisse mit diesem neuen Verfahren übertreffen die Korrekturergebnisse der Harrington-Methode, und da das Verfahren auch die operative Derotation und Lordosierung erlaubt, auch die der Methode nach Dwyer. Das Risiko der Operation erscheint gegenüber dem distrahierenden Verfahren nach Harrington geringer. Die Größe des operativen Eingriffes steht in keinem Verhältnis zu der relativen Einfachheit der postoperativen Behandlung. Die Auswertung der ersten 19 Fälle ergibt, daß mit dem Verfahren der Ventralen Derotationsspondylodese offenbar eine Methode geschaffen wurde, deren Vorteile sie in vielen Fällen den bisherigen operativen Korrekturmethoden in der Skoliosenbehandlung vorziehen lassen.
    Notes: Summary Results about 26 cases operated with the new developped “Ventrale Derotations-Spondylodesis”. The first results with this new method are better than the results of the Harrington and the Dwyer method as it allows the operative derotation and lordosation. The risk of the operation put in relation to the distraction method by Harrington seems less. The importance of the operation one can't put into relation to the rather simple postoperative treatment. The results of the first 19 cases give the impression that the advantages of the Ventral Derotations-Spondylodesis make this method prefer to other instruments in the operative treatment of scoliosis.
