    Notes: Objectives: The purpose of this clinical study was to evaluate the efficacy of the Ultra Sonex Ultima® in comparison with a conventional manual toothbrush.Material and methods: In all, 64 healthy volunteers (32 females, 32 males) took part in the parallel-design treatment-blind study. After a screening examination and stratification by age, sex and papillary bleeding index (PBI), the participants were randomly assigned to two groups with 32 subjects each. Four weeks later, the Turesky modification of the Quigley–Hein plaque index (PI), the approximal plaque index (API), and the PBI were recorded at baseline. Four and 8 weeks after baseline, the indices were recorded again.Results: During the study period of 8 weeks, the API showed no difference between the manual and the electric toothbrush. For the median PI, a statistically significant difference (p 〈 0.001) was found after 4 (manual: 2.16 vs. electric: 1.34) and 8 weeks (1.96 vs. 0.92). After 4 weeks, the median PBI was 0.75 for the manual and 0.43 for the electric brush (p 〈 0.01). The values after 8 weeks were 0.63 (manual) and 0.29 (electric, p 〈 0.001).Conclusion: The Ultra Sonex Ultima may be more efficacious than manual toothbrushes in removing plaque and preventing gingivitis in patients without severe periodontal disease.

    Notes: Abstract. The aim of this study was to determine the plaque removing ability of a new toothbrush with a u-shaped head (Superbrush) compared to a conventional manual toothbrush (Elmex) and an electric toothbrush with a rotating head (Braun Plak control). 36 healthy volunteers, aged 6–60 years, participated in this single blind cross-over study. They were randomly assigned to 3 groups (A, B, C) with 12 participants each. To obtain a plaque-free condition at the baseline, professional toothcleaning was performed in each participant. After instructions on how to use the toothbrushes, each group started the experiment with a different type of toothbrush. After 1 week of application, the Quigley-Hein plaque index (QHI) and the proximal plaque index (API) were used to assess the oral hygiene status of each participant. This was followed by 1 week of recess before each group switched to the next type of toothbrush. The duration of the study was 5 weeks. All examinations were performed by 1 examiner. Compared to the 2 other brushes, the Superbrush was more effective in removing plaque (medians of QHI: 0.84 versus 1.56 (Elmex) and 1.56 (Braun); p〈0.001; medians of API: 0.69 versus 0.94 (Elmex) and 0.87 (Braun); p〈0.001). The study indicated that the new toothbrush may be an effective alternative to commonly used toothbrushes.

    Notes: Objectives: To evaluate the efficacy of two power toothbrushes (Cybersonic and Braun 3D Excel) and one manual brush (Elmex super 29).Material and Methods: After professional toothcleaning 120 subjects were randomly assigned to three groups. Four weeks later, at baseline, the Quigley–Hein plaque index (QHI), the modified approximal plaque index (API), and the papillary bleeding index (PBI) were recorded. Thereafter the subjects used the assigned toothbrushes for 8 weeks. Improvements of the indices after 4 and 8 weeks were calculated as medians. Kruskal–Wallis- and Mann–Whitney U-test served for statistical analysis.Results: All indices showed statistically significant reductions for both power toothbrushes which were superior to the manual brush (4 weeks: Cybersonic: API, 0.21; PBI, 0.25; QHI, 0.23; Braun: API, 0.20; PBI, 0.39; QHI, 0.22; Elmex: API, 0.04; PBI, 0.02; QHI, 0.07; 8 weeks: Cybersonic: API, 0.28; PBI, 0.36; QHI, 0.41; Braun: API, 0.36; PBI, 0.61; QHI, 0.35; Elmex: API, 0.03; PBI, 0.10; QHI, 0.08; p〈0.001). The Braun was superior to the Cybersonic with respect to API after 8 weeks (p〈0.05) and PBI after 4 and 8 weeks (p〈0.01).Conclusion: Cybersonic and Braun 3D Excel may be more efficacious than a manual toothbrush in removing plaque and reducing gingivitis.

    Notes: It has been suggested that specific preventive programs be developed for children with high caries risk. One possibility is to consider whole classes in socially deprived schools as caries-risk groups and perform an intensified preventive program for the entire class. Objective: The purpose of this study was to evaluate the effectiveness of such a program which includes the application of a fluoride varnish (Duraphat®). Methods: The application of the varnish was offered three to four times a year in a community with low socio-economic status and a generally high caries level. Two hundred and sixty-nine children in six primary schools of Linden/Limmer, a district in Hannover, Germany, were allocated to a test or a control group. The test group received the fluoride varnish for 4 years whereas the control group received no professional fluoride application. DMFT was recorded at the beginning of the study and after 4 years. Results: At the end of the study, children who had received a minimum of two fluoride applications per year showed a significantly lower caries increment in comparison with the control group (0.88 DMFT vs 1.39 DMFT, P 〈 0.05). Conclusions: It can be concluded that a minimum of two applications of the fluoride varnish Duraphat® per year may be an effective measure in preventing caries in socially deprived children with high caries activity.

    Notes: Objective: In 1996 the caries prevalence of schoolchildren living in Neukölln, a district of Berlin, Germany, was higher than the mean of total Berlin. Hence, a special preventive program including the application of fluoride varnish was initiated. All 49 primary schools of the district joined the project. The aim of the present study was to evaluate in a cross-sectional study whether the preventive program resulted in a reduced caries prevalence. Methods: The complete program included oral health education as well as the application of a highly concentrated fluoride varnish (Duraphat®). All children whose parents' consent was given could receive fluoride varnish twice a year. All children were examined once a year by four calibrated dentists. The baseline examination was conducted in 1995–96, the final examination in 1999–2000 (1995–96: n= 7,748; 1996–97: n= 15,673; 1997–98: n= 19,362; 1998–99: n= 19,822; 1999–2000: n= 17,984). Results: A total of 80,589 dental records were used for data analysis. A decline of DMFT-values was observed in all age groups and school years. The major improvement was found in school year 1999–2000, when the program had been established for four years. Conclusion: The described program may be an effective public health measure for 6- to 12-year-old children with high caries prevalence.
