    Notes: We compared image computation in the rabbit retina by two different cell types: the so-called ‘local edge detecting’ ganglion cells and the well-known brisk–sustained ganglion cells. From both anatomical and physiological evidence, these cells are present in nearly equal numbers and thus overlap to sample the same regions of visual space. We recorded simultaneously from overlapping cells on a dense microelectrode array. The results were analysed using an anatomically realistic simulation of the retina's processing levels. The ‘local edge detecting’ cell was found to be tuned to higher spatial frequencies and to have a narrower spatial frequency bandpass than the brisk–sustained cells. Simulation revealed that this is due primarily to the ‘zero-crossing’ detector implied by the definition of the local edge detector. The outputs of the simulations in response to complex images were analysed quantitatively. The results showed the population of local edge detectors to transmit a sparser code than the brisk–sustained cells.

Intramusculäre Lipome sind seltene benigne Tumore, die wegen ihres nicht abgrenzbaren Wachstums auch als infiltrierende Lipome bezeichnet werden und tief lokalisiert die Skelettmuskulatur befallen. Klinisch imponiert als einziges Symptom eine palpable Raumforderung. Mikroskopisch findet sich zwischen den Muskelfasern monovacuoläres Fettgewebe, welches die Myocyten verdrängt. Im Extremfall kann die präexistente Muskulatur völlig durch Fettgewebe ersetzt werden. Eine sorgfältige pathohistologische Untersuchung aus einer repräsentativen Biopsie ist notwendig, um die Verwechslung mit einem gut differenzierten Liposarkom zu vermeiden. Die verspätete Diagnosestellung, die Ausdehnung und die Lokalisation des Tumors lassen R0-Resektionen oft nicht zu und beeinflussen die Rezidivraten, die zwischen 3 und 62 % angegeben werden [4]. R0-Resektionen sind in Fällen, bei denen postoperativ bedenkliche Funktionsverluste zu erwarten sind, nicht anzustreben. Funktionsstörungen werden langsam eintreten und können weitgehend kompensiert werden, im Gegensatz zu einem sofortigen Funktionsverlust nach radikaler Chirurgie.
    Notes: Summary. Intramuscular lipomas are rare, benign unencapsulated tumors. They are also called infiltrating lipomas because of their infiltrative growth pattern which, deeply localized, seizes the skeleton muscle. The only obvious symptom is a palpable mass. Microscopically, monovacular fat tissue shows up between the muscle fiber. In extreme cases the preexisting muscle can be totally replaced by fat tissue. A precise pathohistological examination is needed to avoid the mistaken diagnosis of a well-differentiated liposarcoma. Late diagnosis, the extent and the localization of the tumor often do not allow for an R0 resection and influence the postoperative recurrence rates, which lie between 3 and 62 %. R0 resections are not desirable when postoperative severe loss of function can be expected. Malfunctions will slowly take place and can greatly be compensated, in contrast to a radical procedure with immediate loss of function.

    Notes: Abstract In the operative treatment of appendicitis the so called negative appendectomy is an important issue because of its increased morbidity. From the hypothesis that the intestinal diamine oxidase activity is a suitable marker of mucosal integrity, the distribution pattern of the enzyme in appendices histologically classified as inflammed or not inflammed was studied. Histologically apparent inflammation of the appendix was connected with a significant reduction of diamine oxidase activity. The determination of this enzymic activity may be a simple and sensitive test for mucosal inflammation of the appendix even at a very early state. This could reduce the rate of negative appendectomies and influence thereby risk-cost-benefit calculations.

    Notes: Abstract Abstract Non-ketotic hyperglycinaemia (NKH) is a severe seizure disorder associated with high glycine levels. Glycine is a major inhibitory neurotransmitter in the CNS, but has also modulating effects at one of the glutamate receptors, the N-methyl-d-aspartate-(NMDA) receptor. Based on this knowledge we treated a female newborn suffering from severe NKH with the NMDA receptor blocker ketamine in association with strychnine and magnesium supplementation. This treatment led to cessation of seizures, reappearance of swallowing and sucking and improved the neurological status. Some pharmacokinetic data of strychnine and ketamine in the infant are given. Conclusion Ketamine in combination with strychnine may be beneficial in non-ketotic hyperglycinaemia.

    Notes: Zum Thema Die Problematik ist bekannt: Patienten suchen den Hausarzt vor einer Fernreise auf – oft leider relativ kurz vorher – und wünschen entsprechende Schutzimpfungen. Was in solchen Fällen noch möglich ist oder hätte früher geschehen sollen, ist an dieser Stelle nachzulesen. Zumindest sollten die Leser dieser Arbeit eine Konsequenz ziehen: Impfungen, die auch in Deutschland erforderlich und empfohlen sind, sollten in Bezug auf eine Reise eigentlich kein Problem darstellen, nämlich Tetanus, Diphtherie, Poliomyelitis (Typen 1–3), Hepatitis A und eventuell B. Der Impfstatus bezüglich dieser Krankheiten sollte bei den üblichen Check-up-Untersuchungen regelmäßig überprüft werden. Wie die Erfahrung lehrt, wissen Patienten mit ständigem Hausarzt dies sehr zu schätzen und würdigen es zu Recht als Ausdruck echter Vorsorge, wenn sich dieser auch prophylaktisch um Impfangelegenheiten kümmert. Wenn die genannten Schutzimpfungen vorhanden sind, verengt sich das Spektrum weiterer Schutzimpfungen, von denen nur die Gelbfieberimpfung durch eine behördlich genehmigte Gelbfieber-Impfstelle durchgeführt werden muß. Weitere Impfungen sind besonderen Regionen und Reiseumständen vorbehalten, die im allgemeinen voraussehbar sind und in der ärztlichen Beratung erörtert werden sollten. Dazu werden hier viele Informationen gegeben.
