    Notizen: This paper argues against the laws-systems-rules trichotomy which Cushman and Pearce use to organize theoretical options in the communication field. This trichotomy of perspectives does reflect the groupings into which scholars have come to classify themselves, but the analytic implications of this division follow strongly and confound the actual levels of choice inherent in theory building. The organization of theoretical alternatives by attempting to identify a type of necessity unique to each obscures fundamental differences among theorists, unprofitably restricts the range of theoretical alternatives, and will not encompass the variety of viable theoretic perspectives now before us. In place of the laws-rules-systems trichotomy this paper offers a multilevel analytic framework for organizing theoretical alternatives in communication.

    Notizen: Abstract: Epithelial mucins are glycoproteins secreted by epithelial cells and their carcinomas. At least nine mucin genes have been identified, and their products (MUC1–MUC9) are expressed in various epithelia. MUC1 is a mucin expressed in breast epithelial cells, whereas MUC2 and MUC3 are primarily intestinal mucins. Although MUC1 and MUC2 expression has been documented in invasive ductal carcinoma of the breast, mucin expression in pure ductal carcinoma in situ (DCIS) has not been investigated. Sixty-one of 105 cases of DCIS without coexisting infiltrating carcinoma diagnosed during a 30-month period were selected as having sufficient tissue for study. Paraffin-embedded tissue sections were stained using immunohistochemical techniques with mouse monoclonal anti-MUC1, anti-MUC2, and rabbit-specific polyclonal anti-MUC3 antibodies. Immunoreactive epitopes of MUC1, MUC2, and MUC3 were expressed in DCIS in 61, 19, and 16 of 61 cases, respectively. MUC2 and MUC3 staining intensity in DCIS was markedly less than that observed for MUC1. Luminal and/or cytoplasmic patterns of staining were observed for MUC1. MUC2 and MUC3 showed only cytoplasmic staining. Cytoplasmic-only staining of MUC1 was associated with a higher grade of DCIS. Any MUC2 staining was also associated with a higher grade of DCIS. Coexpression of MUC2 and MUC3 was present in only 6 of 61 cases, and MUC3 staining was unrelated to the grade of DCIS. Cytoplasmic expression of MUC1 and MUC2 appears to be associated with a higher grade of DCIS. MUC3 expression appears to be independent of grade and expression of MUC1 and MUC2. The relationship of mucin expression and grade warrants further study.

    Notizen: Summary Sixteen patients undergoing colonoscopy were sedated with propofol using a closed-loop control system guided by the Bispectral Index (BIS). Propofol administration, via a target-controlled infusion, was controlled by a proportional-integral-differential control algorithm. The median (range) propofol target concentration during closed-loop control was 2.3 (1.7–3.6) µg.ml−1. The performance characteristics of the system were excellent, with a median absolute performance error of 7 (1–15)%. Patients were drowsy yet rousable, with a median (range) BIS set-point of 80 (75–85). No patient became apnoeic, required airway support or became haemodynamically unstable whilst sedated. Eight patients moaned or moved during colonoscopy and four had recall. Median (range) time to full consciousness was 4 (2–20) min after the end of closed-loop control. Patient and surgeon satisfaction were high. We conclude that BIS may be a suitable control variable for closed-loop control of sedation with propofol.

    Notizen: Smoking is a risk factor for complications during and after surgery, but most smokers are unable to quit before elective surgery. We tested the efficacy of bupropion in improving smoking cessation rates in this setting by enrolling 47 patients from the elective surgery waiting list in a double-blind randomised controlled trial. Patients receiving bupropion had a lower daily cigarette consumption at the time of hospital admission, median (IQR) cigarettes per day: 6 (2–7) vs. 15 (9–20), p = 0.046. They also had a reduction in end-expired carbon monoxide (p = 0.004), a known contaminant of cigarette smoke, and increased arterial oxygen saturation on pulse oximetry (p = 0.011). They were more likely to have stopped smoking at the 3-week visit (p = 0.036), but not at the 6-week visit (p = 0.25) or at the time of hospital admission for surgery (p 〉 0.99). This study found that smokers waiting for elective surgery are more likely to reduce or stop smoking when treated with bupropion.

    Notizen: Summary Advances in technology have resulted in the development of several depth-of-anaesthesia monitors. Whether any of these monitors can reduce the incidence of awareness is an important issue for anaesthetists and their patients. We therefore surveyed a random selection of anaesthetists, asking for their opinions of awareness and depth-of-anaesthesia monitoring in current clinical practice. Approximately half (52%) of the anaesthetists surveyed had experienced a patient with awareness. Anaesthetists considered that they had a lower incidence of awareness in their own practice when compared with others, 1:5000 vs. 1:10 000 (p 〈 0.001). Anaesthetists rated awareness on an11-point scale as only a moderate problem, median (interquartile range) 5 (2–7). Older anaesthetists were less likely to rate the importance of awareness highly (p = 0.009) and to use awareness monitoring (p = 0.001). Anaesthetists are prepared to use depth-of-anaesthesia monitoring more widely if it can be shown to prevent most cases of awareness in routine practice.

    Notizen: Abstract Energy conservation in Chinese residential buildings merits attention in part due to China's large population, increased standard of living, and high volume of construction. This paper focuses on three major themes: issues exerting an upward pressure on residential energy use, policy for promoting energy conservation, and technical opportunities to decrease energy use and increase comfort. An increase in total floor area, increasing electric appliance ownership, and an increase in demand for heating and cooling characterize the residential sector. Space heating remains the dominant energy end-use. Relatively new buildings in cold regions of China, while a marked improvement over earlier construction, still suffer from inadequate insulation, leaky windows, and lack of attention to construction quality. Energy-conservation standards have addressed these issues in part, but further effort is needed to ensure code compliance. A survey of prospective homeowners shows that energy-efficient housing would best be promoted by marketing the combined benefits of natural lighting and ventilation, better quality, and savings in heating and air-conditioning energy costs, rather than only promoting a dwelling as "environmentally friendly" or offering reduced heating bills. Reviewed technologies concentrate on reductions in heating and cooling energy through improved site layout, building design, building envelope materials, ventilation, and alternative cooling equipment.
