    Notes: Summary Blood viscosity (shear rates 100s-1 and 0.94 s-1) and several of its major determinants (haematocrit, plasma fibrinogen and plasma viscosity) have been measured in 38 male insulin-treated diabetics, aged 18–50 years, and in 38 non-diabetic control subjects matched for age and smoking habit. Diabetics without fundoscopic retinopathy (n=20) had higher mean blood viscosity than controls at the high shear rate (7.07 cP vs 6.75 cP, p〈0.05) and the low shear rate (21.2 cP vs 18.7 cP, p〈0.025). These differences persisted after correction of blood viscosity to a standard haematocrit, and were associated with increased plasma viscosity (1.41 cP vs 1.34 cP, p〈0.025) and plasma fibrinogen (2.9 g/L vs. 2.5 g/L, p〈0.025). Diabetics with retinopathy (n=18) had higher mean blood viscosity than diabetics without retinopathy at the high shear rate (7.53 cP vs 7.07 cP, p〈0.05) and the low shear rate (24.3 cP vs. 21.2 cP, p〈0.05), associated with a higher haematocrit (p〈0.05). Blood viscosity and haematocrit correlated with the duration of diabetes (r〉0.32, p〈0.05).

    Notes: Conclusions The adolescent diabetic has a totally different set of problems from either the child or adult with diabetes. Failure to recognise this and deal with it will result in non-compliance and non-attendance at the clinic. Anyone taking on the care of a teenage diabetic must be prepared to try to resolve all their problems in the one clinic. There is nothing worse for the youngster than to be rendered artificially schizophrenic when she has to attend the dietician for advice on weight reduction, a family planning clinic for advice on contraception, a psychiatrist to find out how to resolve family problems etc. It has been said that when dealing with the interplay between diabetes and psychological factors in the diabetic adolescent, “the well rounded clinician who can treat the disease and the emotional decompensation that accompanies it, is indeed the flower of our profession” [38].

    Notes: Abstract Young oysters (Crassostrea virginica) were continuously exposed to Aroclor® 1254, a polychlorinated biphenyl (PCB), in flowing, unfiltered seawater. Growth rate (height and in-water weight) was significantly reduced (α=0.05) in oysters exposed to 5 μg/l (ppb) for 24 weeks. Growth rate was not affected in oysters exposed to 1 ppb for 30 weeks. Mortality was not significant in exposed and control groups. In oysters exposed to 5 ppb, greatest PCB residue (whole body) was 425 mg/kg (ppm), 85,000x the concentration in the water, and less than 0.3 ppm was retained after 28 weeks depuration in PCB-free water. In oysters exposed to 1 ppb, greatest residue was 101 ppm, 101,000x the concentration in the water, and less than 0.2 ppm was retained after 12 weeks depuration. Examination of oysters exposed to 5 ppb of this PCB for pathogenesis revealed atrophy of digestive diverticular epithelium and degeneration of vesicular connective tissues concomitant with leukocytic infiltration, but tissue recovery seemed excellent after 12 weeks depuration.

    Notes: Summary Different doses of phenylbutazone have been compared in a double blind study on 32 patients with rheumatoid arthritis in order to determine the minimum effective dose. Of 8 different dose levels studied (90 mg, 150 mg, 180 mg, 240 mg, 270 mg, 300 mg, 360 mg and 450 mg/day) the most efficacious was found to be 300 mg/day. Doses below this did not produce full benefit; no further improvement occurs with higher doses. Although 7/32 patients developed adverse reactions there was no relationship between these and the plasma levels of either phenylbutazone or oxyphenbutazone. An attempt was made to distinguish ‘responders’ from ‘non-responders’. We found no relationship between response and plasma levels of phenylbutazone or oxyphenbutazone.

Two oral corticosteroids, prednisolone (8 mg/day) and betamethasone (1 mg/day) have been compared in terms of efficacy and adrenal suppressive activity when used in chronic oral dosage in rheumatoid arthritis. 2. 20 patients were entered to a single blind crossover study receiving each drug for a two-week period. Clinical and laboratory assessments were performed. 3. At this dosage there was no significant difference between any of the clinical parameters assessed for either drug though patient preference was 13 for prednisolone and 7 for betamethasone. 4. At this dosage adrenal suppression was not equivalent, being significantly more marked with betamethasone. 5. The results suggest that prednisolone is the drug of choice for chronic dosage.

    Notes: Summary The pharmacokinetics of prednisolone elimination have been studied in both arthritic patients and normal volunteers using tritiated prednisolone alone, and in conjunction with unlabelled prednisolone in doses of 0.15 mg·kg−1 and 0.3 mg·kg−1 body weight. With increasing dose there is prolongation of the plasma half-life and increase in the volume of distribution and plasma clearance of prednisolone. It is proposed that these changes in pharmacokinetic parameters may be associated with non-linear binding of the steroid to plasma proteins.

    Notes: Conclusions Our results suggest that chronic exposure to Aroclor 1254 increased susceptibility of test pinfish and spot to disease, and also appeared to be toxic to these fish. This PCB is rapidly stored by pinfish and spot, and persists in tissues for approximately three months. Our findings emphasize the need for further study on the effect of chronic exposure of aquatic organisms to polychlorinated biphenyls.
