    Description / Table of Contents: Zusammenfassung 16 Fälle von teilweise dramatischen Unverträglichkeitsreationen nach Humanalbumininfusionen wurden untersucht. Die Reaktionen manifestierten sich als Fieberanstiege, Schüttelfrost, urticarielle Exantheme und Blutdruckschwankungen bis zum Schock. Sie waren durch Prednisolon günstig zu beeinflussen und klangen nach Absetzen der Humanalbumininfusionen restlos ab. Im zeitlichen Auftreten ließ sich eine Früh- von einer Spätreaktion unterscheiden, wobei die Konzentration des verwendeten Humanalbumins eine Rolle zu spielen schien. Frühreaktionen wurden bevorzugt durch 20%iges, Spätreaktionen immer durch 5%iges Humanalbumin hervorgerufen. Als auslösender Faktor konnten Verunreinigungen der verwendeten Infusionslösungen ausgeschlossen werden. Die Ergebnisse von Intrakutantestungen, Antigeneliminationstechnik, sowie der Nachweis von präzipitierenden Antikörpern gegen Makroaggregate des Albumin-Ultrazentrifugats sprechen dafür, daß eine immunologische Reaktion pathogenetisch von Bedeutung sein könnte. Möglicherweise läßt sich durch Applikation eines desaggregierten Humanalbumins das Risiko von Unverträglichkeitsreaktionen auch bei Gabe großer Humanalbuminmengen an allergisch disponierte Patienten verringern.
    Notes: Summary 16 cases of clinical incompatibility reactions due to human serum albumin (HSA) were recorded. The reactions consisted in increases in body temperature, sometimes together with shivering, urticarial exanthema and blood pressure changes as far as shock. They responded well to prednisolone application and disappeared completely after discontinuing the albumin-therapy. Regarding the onset in time, “early” (1 hour) and “late” (3 days) reactions could be distinguished; early reactions were mostly caused by 20%, late reactions by 5% albumin solutions. Pollutions of the administered HSA-batches could be excluded as an etiologic factor. The results of skin tests, antigen-elimination technique as well as the demonstration of precipitating antibodies directed against the concentrated macroaggregates of the albumin ultracentrifugation sediment suggest a pathogenetic influence of immunological reactions. Ultracentrifugation of the albumin prior to the administration might possibly reduce the risk of incompatibility reactions after high dose application in hyperreactive patients.

    Description / Table of Contents: Zusammenfassung Bei 127 mit ALG vom Pferd behandelten Patienten wurden die möglichen hämatologischen und klinischen Nebenwirkungen erfaßt, ihre prophylaktische Verhinderung und therapeutische Beherrschung geschildert. Durch Anwendung eines exakten Therapieschemas in Kombination mit Azathioprine und Corticosteroiden nach sorgfältiger Patientenauswahl und versuchter Erzeugung einer immunologischen Toleranz gegen Pferde-IgG, ließen sich Häufigkeit und Intensität von Nebenreaktionen signifikant verringern. Dies wird im Vergleich zweier Patientengruppen gezeigt. Die für die Auslösung der Nebenreaktionen verantwortlichen pathogenen Faktoren werden erörtert. Da prinzipiell unvermeidbare, lebensbedrohende Nebenwirkungen bisher nicht aufgetreten sind, wird die Frage nach der ärztlichen Verantwortbarkeit einer ALG-Therapie bejaht, jedoch sorgfältige klinische, klinisch-chemische und immunologische Überwachung gefordert.
    Notes: Summary In 127 patients treated with horse anti-human lymphocytic globulin (ALG) the possible haematological and clinical side effects, their prevention and treatment were recorded. Frequency and intensity of side reactions can be reduced significantly by the application of a strict therapy schedule in combination with azathioprine and corticosteroids, careful selection of patients and induction of immunological tolerance against horse-IgG. This is shown when comparing the two groups of patients. The pathogenic factors of the side-reactions are discussed. Because principally unavoidable, life-threatening side effects have not been observed, the answer to the question of the ethical justification of ALG therapy is positive. Careful clinical, biochemical and immunological controls are postulated.

    Description / Table of Contents: Zusammenfassung Die derzeit gebräuchlichen i.v. Gammaglobuline (GG) zerfallen im wesentlichen in zwei Gruppen: Gespaltene und ungespaltene Produkte. Gespaltene GG erhält man durch Pepsin- oder Plasmin-Behandlung, ungespaltene durchβ-Propiolacton-Stabilisierung, Säure-pH-4-Behandlung oder Polyäthylenglykol-Hydroxyäthylstärke-(PEG/HÄS)-Fällung. Die einzelnen Präparate unterscheiden sich in ihrer biologischen Aktivität (bestimmte Funktionen der Komplementaktivierung und Opsonisierung sind an die Fc-Struktur gebunden) sowie im Eliminationsverhalten (ungespaltene GG haben längere intra- und extravasale Halbwertszeiten). Während die Wirkung von GG in tierexperimentellen Studien gesichert erscheint, stehen für die meisten klinischen Indikationen kontrollierte Studien noch aus. Eine prospektive Studie an chirurgischen Risikopatienten ergab, daß die prophylaktische Gabe von hohen Dosen eines 7 S-GG die Häufigkeit septischer Komplikationen im post-operative Verlauf zu senken vermochte. Die Zukunft der GG-Therapie scheint in der Entwicklung eines größeren Spektrums von Hyperimmunglobulinen zu liegen.
    Notes: Summary The presently available i.v. gammaglobulines (GG) can be classified into two groups: Degraded and intact GG. Degraded GG are produced by pepsin or plasmin digestion. Intact GG are obtained byβ-propiolactone treatment, acidification at pH 4 or precipitation with polyethylenglycol-hydroxyethylstarch (PEG/HES). The various products have different characteristics with regard to their biological activity (certain functions of complement activation and opsonization are connected with the Fc structure) as well as their elimination (intact GG have longer intra-and extravasal half-life times). While their is no doubt about an effect of GG in animal experiments, little controlled studies have been done for most of the clinical indications. One controlled prospective study showed that in surgical high risk patients the frequency of septic complications can be reduced by prophylactic application of high doses of 7 S-GG. For the future, the development of a broad spectrum of hyperimmunoglobulines seems desirable.

    Notes: Summary Adverse reactions to foods are not infrequent. They may be mediated by immunological mechanisms (food allergy) or non-immunologically (idiosyncrasy, pseudo-allergy, intolerance). Furthermore toxic effects of foods have to be clearly distinguished from food allergy as well as poorly defined conditions such as hyperkinesis or “tension-fatigue syndrome”, the causal relation of which to foods is not well established. The diagnosis of food allergy includes convincing history, positive provocation and demonstration of immunological sensitization (mostly IgE, however other types of immune reactions may also be of importance). In the treatment of food allergy specific elimination diets as well as pharmacotherapy with the use of mast cell blocking agents are recommended. In single cases oral hyposensitization may be tried.

    Description / Table of Contents: Summary A 59-year-old Caucasian man suffered from persistent disseminated necrolytic skin lesions for 8 months. They failed to respond to a variety of therapeutic regimens but then cleared spontaneously within a few days. After one week an invasive squamous cell carcinoma of the hypopharynx became clinically apparent. At autopsy, the expected glucagonoma often associated with necrolytic migratory erythema was not found.
    Notes: Zusammenfassung Nach achtmonatiger therapierefraktärer Persistenz disseminierter nekrolytischer Hautveränderungen kam es bei einem 59jährigen Patienten zu einer spontanen Abheilung der Läsionen innerhalb weniger Tage. Eine Woche später manifestierte sich klinisch ein invasiv wachsendes Plattenepithelkarzinom des Hypopharynx. Ein im Rahmen der kutanen Paraneoplasie zu erwartendes Glukagonom konnte autoptisch nicht nachgewiesen werden.

    Notes: Abstract Toxic epidermal necrolysis (Lyell's syndrome) with erythematous skin lesions and bulla formation developed in a 4-year-old girl. An accurate diagnosis using the cryostal technique on the top of a bulla was available within 1 h of hospital admission. The course was unusually mild, probably because of early treatment with corticosteroids. Skin prick tests revealed salicylamide as the agent responsible for inducing the disease. The patient was advised to avoid this substance for the rest of her life.

    Notes: A case of erythema annulare centrifugum-type psoriasis in a 58-year-old white woman with a history of chronic plaque psoriasis is described. Initial failure of antipsoriatic treatments and an untypical histology complicated the diagnosis. After several trials acitretin at a low maintenance dose combined with oral fish oil and topical calcitriol led to sustained long-term remission. The spectrum of clinical differentiation of this rare disease, the histological characteristics and its nosological classification are discussed. It is suggested that this dermatosis represents a variant of acute psoriasis, rather than a variant of pustular psoriasis.

    Notes:   Topical Calcineurin Inhibitors (TCIs) used for the treatment of atopic eczema modify the immune regulatory function of the skin and may have the potential to enhance immunosuppressive ultraviolet (UV) effects. Current recommendations on UV protection in eczema patients treated with PCIs are inconsistent and have given rise to uncertainty and anxiety in patients. Therefore, the European Dermatology Forum (EDF) developed a position statement which reviews critically the available data with regard to the problem, especially analysing and commenting the limitations of rodent models for the human situation. There is no conclusive evidence from rodent trials to indicate that long-term application of TCIs is photococarcinogenic. There is a need for further studies to investigate the validity of mouse models as well as long-term cohort studies in patients using TCIs. Available data suggest that long-term application of TCIs is safe, that there is no evidence of increased skin cancer risk and that it is ethical to treat patients with TCIs when indicated.
