    Notes: Abstract Cyclosporin (CsA) is currently the main immunosuppressive agent used in organ transplantation with considerable improvement in graft survival. Oral CsA solution is highly lipophilic, and its bioavailability may be reduced in cystic fibrosis (CF) heart-lung transplant recipients with pancreatic, gastrointestinal, and hepatic insufficiency. The bioavailability of oral CsA solution in 7 CF transplant recipients (5 male and 2 female with a mean age of 27 years and a mean weight of 49 kg) and 3 non-CF heart-lung recipients (1 male and 2 female with a mean age of 41 years and a mean weight of 60 kg) was studied. Following intravenous CsA administration, the kinetic curves were similar with no significant difference in the volume of distribution and clearance of CsA demonstrated between the CF and non-CF groups. The mean daily dose of oral CsA in 7 CF subjects (23.3 mg·kg−1) was significantly higher than the 3 non-CF heart-lung recipients (4.8 mg·kg−1). The mean maximum blood concentration of CsA for the oral dose was 776 ng·ml−1 for the 7 CF subjects, which was comparable with the mean peak values of 789 ng·ml−1 for the 3 non-CF control subjects. Poor enteral absorption of CsA probably accounts for the significantly lower mean bioavailability in the 7 CF subjects (14.9%) compared with the 3 non-CF control subjects (39.4%). The effects on the bioavailability of oral CsA solution by pancreatic enzymes (Creon) and histamine-2 antagonist (ranitidine) were also evaluated in the 7 CF subjects. No significant difference was demonstrated.

    Notes: A method for imaging magnetic domain structures in the transmission electron microscope is described. Coherent Foucault imaging provides a direct means of producing a magnetic interferogram which reveals the quantitative distribution of magnetic induction across the specimen. The technique requires the high coherence of a field-emission gun system and the ability to position an aperture accurately to cut part of the diffraction pattern. A simple analytical theory, together with computer simulations and experimental results, is presented to demonstrate the power of the technique.

    Notes: Objective To describe the management of a family with an inherited predisposition to ovarian and breast cancer. Particular attention is paid to the problems of contraception, screening, prophylactic surgery and hormone replacement therapy.Setting The multidisciplinary Grampian Familial Epithelial Overian Cancer Study Group.Subjects 162 members of a family extending over five generations. In the third generation, five of the 10 women died with epithelial ovarian cancer. Three women in generation IV have developed premenopausal breast cancer. There are now 78 family members in the fifth generation aged between 2 and 22 years.Interventions Counselling of female family members is started at the age of 18 years. The combined oral contraceptive pill is advocated to suppress ovulation. Gynaecological follow-up after the age of 28 includes yearly pelvic examination, transvaginal ultrasonography and serum CA125 estimation. Laparoscopy with peritoneal cytology is indicated if any part of this yearly assessment is abnormal. Prophylactic oophorectomy is advised between the ages of 35 and 40 years after the family is complete. In generation IV, 20 of the 29 women have undergone prophylactic oophorectomy. Oestrogen hormone replacement therapy with a cyclical progestogen is recommended after prophylactic oophorectomy. Breast cancer screening starts at the age of 25 and involves annual clinical breast examination augmented by mammography and breast ultrasound.Conclusions Only by the careful questioning and recording of family history, including at least third degree relatives (cousins), will similar groups with familial ovarian/breast cancer be identified. When predisposing genes are characterized it will be possible to identify carriers within the family and concentrate clinical effort on them while offering appropriate reassurance to those with decreased risk.

    Notes: The Rhizobium leguminosarum biovar viciae nodulation protein NodO is partially homologous to haemolysin of Escherichia coli and, like haemolysin, is secreted into the growth medium. The NodO protein can be secreted by a strain of E. coli carrying the cloned nodO gene plus the haemolysin secretion genes hlyBD, in a process that also requires the outer membrane protein encoded by tolC. The related protease secretion genes, prtDEF, from Erwinia chrysanthemi also enable E. coli to secrete NodO. The Rhizobium genes encoding the proteins required for NodO secretion are unlinked to nodO and are unlike other nod genes, since they do not require flavonoids or NodO for their expression. Although proteins similar to NodO were not found in rhizobia other than R. leguminosarum bv. viciae, several rhizobia and an Agrobacterium strain containing the cloned nodO gene were found to have the ability to secrete NodO. These observations indicate that a wide range of the Rhizobiaceae have a protein secretion mechanism analogous to that which secretes haemotysin and related toxins and proteases in the Enterobacteriaceae.

    Notes: Abstract We report results of anhydrous 1 atm and piston-cylinder experiments on ID16, an Aleutian high-magnesia basalt (HMB), designed to investigate potential petrogenetic links between arc high-alumina basalts (HABs) and less common HMBs. ID16 is multiply saturated with a plagioclase/spinel iherzolite mineral assemblage (olivine, plagioclase, clinopyroxene, orthopyroxene, spinel) immediately beneath the 12 kbar liquidus. Derivative liquids produced at high temperatures in the 10–20 kbar melting interval of ID16 have compositions resembling those published of many moderate-CaO HABs, although lower-temperature liquids are poorer in CaO and richer in alkalies than are typical HABs. Isomolar pseudoternary projections and numerical mass-balance modeling suggest that derivative melts of ID16 enter into a complex reaction relationship with olivine at 10 kbar and 1,200° C–1,150° C. We sought to test such a mechanism to explain the lack of liquidus olivine in anhydrous experiments on mafic high-alumina basalts such as SSS. 1.4 (Johnston 1986). These derivative liquids, however, do not resemble typical arc high-alumina basalts, suggesting that olivine-liquid reaction does not account for Johnston's (1986) observations. Instead, we suggest that olivine can be brought onto the liquidus of such compositions only through the involvement of H2O, which will affect the influence of bulk CaO, MgO, and Al2O3 contents on the identity of HAB liquidus phases (olivine or plagioclase) at pressures less than ∼12 kbar.
