    Notizen: We estimate the effects of hospital competition on the level of and the variation in quality of care and hospital expenditures for elderly Medicare beneficiaries with heart attack. We compare competition's effects on more-severely ill patients, whom we assume value quality more highly, to the effects on less-severely ill, low-valuation patients. We find that low-valuation patients in competitive markets receive less intensive treatment than in uncompetitive markets, but have statistically similar health outcomes. In contrast, high-valuation patients in competitive markets receive more intensive treatment than in uncompetitive markets, and have significantly better health outcomes. Because this competition-induced increase in variation in expenditures is, on net, expenditure-decreasing and outcome-beneficial, we conclude that it is welfare-enhancing. These findings are inconsistent with conventional models of vertical differentiation, although they can be accommodated by more recent models.

    Notizen: If humans monitor streams of rapidly presented (≈ 100-ms intervals) visual stimuli, which are typically specific single letters of the alphabet, for two targets (T1 and T2), they often miss T2 if it follows T1 within an interval of 200–500 ms. If T2 follows T1 directly (within 100 ms; described as occurring at ‘Lag 1’), however, performance is often excellent: the so-called ‘Lag-1 sparing’ phenomenon. Lag-1 sparing might result from the integration of the two targets into the same ‘event representation’, which fits with the observation that sparing is often accompanied by a loss of T1–T2 order information. Alternatively, this might point to competition between the two targets (implying a trade-off between performance on T1 and T2) and Lag-1 sparing might solely emerge from conditional data analysis (i.e. T2 performance given T1 correct). We investigated the neural correlates of Lag-1 sparing by carrying out magnetoencephalography (MEG) recordings during an attentional blink (AB) task, by presenting two targets with a temporal lag of either 1 or 2 and, in the case of Lag 2, with a nontarget or a blank intervening between T1 and T2. In contrast to Lag 2, where two distinct neural responses were observed, at Lag 1 the two targets produced one common neural response in the left temporo-parieto-frontal (TPF) area but not in the right TPF or prefrontal areas. We discuss the implications of this result with respect to competition and integration hypotheses, and with respect to the different functional roles of the cortical areas considered. We suggest that more than one target can be identified in parallel in left TPF, at least in the absence of intervening nontarget information (i.e. masks), yet identified targets are processed and consolidated as two separate events by other cortical areas (right TPF and PFC, respectively).

    Notizen: Summary.  Recombinant activated factor VII (rFVIIa), licensed in 1999 for treatment of haemophilia patients with inhibitors (HI), represents an important advance in the therapeutic armamentarium. Standard bolus dosing ranges from 90 to 120 mcg kg−1 every 2–3 h until arrest of bleeding. As licensure, clinical use of rFVIIa has increased and broadened. Clinicians now use a wide dose range, 90–300 mcg kg−1. High-dose regimens may optimize thrombin generation or burst, and may allow for prolonged dose interval. The Hemophilia and Thrombosis Research Society (HTRS) maintains a registry database to study haemophilia treatment and related disorders, particularly treatment of acute bleeding in HI, acquired haemophilia, FVII deficiency and von Willebrand's disease (VWD). To assess the effect of rFVIIa dose on efficacy and safety in the treatment of acute bleeding in HI, data from the HTRS database from January 2000 through June 2002 were analysed. Bleeding episodes were grouped by bolus rFVIIa dose range: 〈100, 100–150, 150–200 and 〉200 mcg kg−1. Investigator-reported efficacy for the first 72 h of treatment was evaluated. Thirty-eight congenital HI patients were treated for 555 bleeding episodes. Patient age range was 1–55 years (median: 14). Bleeding episodes were spontaneous (45%), caused by trauma (38%), or because of surgery, dental, diagnostic, or medical procedures (17%); bleeding occurred in joint, muscle, and intra/extracranial sites. Treatment location included: 80% at home, 12% at other facilities (treatment centres, ER, inpatient and OR), and 8% at both home/other facilities. Median total dose given over 72 h was 360 mcg kg−1 (range: 40–4281, mean: 537). Bleeding stopped in 87% of the episodes. Bleeding cessation rate was 84% for the three lower dose groups, and 97% for the highest dose group (P 〈 0.001). Five patients experienced nine adverse events (AEs). AE rates were 〈1% for 〈100, 5% for 100–150, 0% for 150–200, 〈1% for 〉200 mcg kg−1 dose group. Decreased therapeutic response accounted for eight of the nine AEs. These data, which represent the most comprehensive report of rFVIIa use since the USA licensure, demonstrate that bleeding episodes in HI patients can be treated safely and effectively at home and that doses up to 346 mcg kg−1 appear to be well-tolerated. Additionally, rFVIIa doses 〉200 mcg kg−1 appear to significantly increase efficacy (97% in the high-dose group, compared with 84% in the lower dose groups). Optimal dosing remains to be determined; specifically, what the lowest effective dose is and whether a single high-dose bolus eliminates the need for repeated dosing. Recombinant FVIIa appears to have a wide safety margin that may allow dose escalation to address these questions.

    Notizen: Summary.  Background:  Deletion of the B-domain of recombinant blood coagulation factor VIII (BDDrFVIII) increases the manufacturing yield of the product but does not impair in vitro or in vivo functionality. BDDrFVIII (ReFacto®) has been developed with the additional benefit of being formulated without human albumin.Objective:  The primary objective of this three-way crossover-design study was to compare the pharmacokinetic (PK) parameters of two BDDrFVIII formulations (one reconstituted with 5 mL of sterile water, the other reconstituted with 4 mL sodium chloride 0.9% USP) with those of a plasma-derived, full-length FVIII preparation (Hemofil® M) in patients with haemophilia A to determine bioequivalence.Methods:  A series of blood samples were collected over a period of 48 h after i.v. administration of each of the FVIII preparations. Plasma FVIII activity was determined using a validated chromogenic substrate assay. Plasma FVIII activity vs. time curves was characterized for a standard set of PK parameter estimates. Two parameter estimates, the maximum plasma concentration (Cmax) and the area under plasma concentration vs. time curves (AUCs), were used to evaluate bioequivalence. The two preparations were considered bioequivalent if the 90% confidence intervals for the ratio of geometric means for Cmax and AUCs fell within the bioequivalence window of 80% to 125%.Results/Conclusion:  Results show that each BDDrFVIII formulation is bioequivalent to Hemofil M and the two formulations of BDDrFVIII are bioequivalent to each other.

    Notizen: Due to its high quantum efficiency (QE) for luminescence, conventional coarse-grainedYAG:Ce (Y3Al5O12:Ce) finds widespread use in light conversion and scintillator applications.Nanocrystalline YAG:Ce may possess modified optical properties which are advantageous fortechnological applications, but this will depend on highly efficient energy conversion. In this work,the effect of the particle size and Ce concentration on the quantum efficiency and the opticallifetime of the YAG:Ce emission will be characterized and discussed. Nanocrystalline YAG:Cewith an average particle size of 20 to 50 nm was synthesized by the chemical vapour reaction(CVR) method and subsequently analyzed using various techniques. When comparing thenanocrystalline samples to a coarse-grained reference sample, the particle size and dopingconcentration was found to have a significant influence on quantum efficiency. It was establishedthat the nanocrystalline samples investigated exhibit a lower QE at ambient temperature than thecoarse-grained reference. The results of the optical lifetime measurements are discussed in relationto this reduction in QE

In relation to the conventional quenching procedures inaqueous media, gas quenching exhibits a number of technological, ecological, and economicaladvantages. The quenching intensity can be adjusted by the variable parameters gas pressure, gasvelocity as well as the kind of gas and thus can be adapted to the requirements of the component.By the higher uniformity and the better reproducibility, gas quenching offers a high potential toreduce distortion. Cost savings would be possible, because of reduced distortion and therefore lessreworking. High-pressure gas quenching with nitrogen or helium, as well as air quenching atambient pressure in a gas nozzle field was applied to the spray formed aluminum alloyAl-17Si-4Fe-3Cu-0.5Mg-0.4Zr (DISPAL S232). Hardness and tensile tests have been carried out todetermine the mechanical properties after gas quenching and aging compared to water quenching.The distortion behavior of a forged aluminum component of the spray formed alloy was examinedafter gas quenching and after water quenching.
