    Notizen: We describe a nurse-led pre-assessment system at an elective surgical centre. A targeted referral system was used by trained nurses to direct referrals to a supervising consultant anaesthetist or to the surgical team. Of 2726 patients pre-assessed in the first 2 years, 105 patients (3.9%) were cancelled or postponed for medical optimisation after pre-assessment. There were 137 cancellations (5.0%) on the day of surgery, despite pre-assessment, but only 36 were for anaesthetic or medical reasons. Only eight of these 36 were considered a ‘failure’ of the pre-assessment system. These results are much better than the cancellation rate of about 11% in the Trust as a whole. There were 18 transfers of patients postoperatively from the elective centre to another hospital. A review suggested that four of these transfers could have been reasonably predictable from the patients' medical history. We conclude that a pre-assessment clinic has an important role to play in minimizing cancellations on the day of surgery and also in reducing the number of patients transferred to other hospitals. This last conclusion has an important implication for the planning of systems in hospitals that perform only elective surgery.

    Notizen: Angioedema of the face and neck is an uncommon but potentially life-threatening complication of angiotensin-converting enzyme (ACE) inhibitor therapy. This condition is of particular concern to the anaesthetist as it can rapidly progress to upper airway obstruction. We describe the presentation and management of five cases of ACE inhibitor related angioedema, all of which were associated with significant upper airway obstruction.

    Notizen: Abstract Field surveys at five sites within desert locust breeding habitats around Port Sudan during three successive seasons indicated that early in the rainy season the incoming solitary females oviposited predominantly in the vicinity of Heliotropium spp. (∼66%) and millet (∼32%) seedlings. Solitary nymphs also preferred to feed on these plants. Follow-up cage experiments were conducted in the field in which solitary and gregarious female locusts were presented with choices of selected desert plants and egg pods. When presented with bulrush millet, Heliotropium spp., Zygophyllum simplex, and untreated moistened sand, solitary females oviposited adjacent to the first two plants (40% and 60%, respectively). However, when offered a choice of either or both of these plants together with egg pods derived from gregarious and/or solitary insects, solitary females showed a significantly higher preference for ovipositing near gregaria egg pods than near the plants, with solitaria egg pods eliciting the least response. In contrast with solitary females and in the absence of gregaria egg pods, gregarious females preferred to oviposit in untreated moist (control) sand (74–77%) away from the plants (6–14%) or solitaria egg pods (∼4%). However, when present, gregaria egg pods elicited significantly more oviposition. These and previous results indicate a hierarchy of phase-dependent oviposition preferences in the desert locust and are interpreted in terms of a strong propensity of the species to exploit opportunities under appropriate conditions to facilitate congregation and the gregarization of the progeny.

    Notizen: Abstract Nosema locustae, a microsporidian parasite of locusts and grasshoppers, was transovarially transmitted to the progeny of infectedLocusta migratoria reared for up to F14 generations. The mortality of infected progeny in each generation was higher than that of uninfected controls and ranged from 67.6% to 95.5%. Infected female survivors transmitted the microsporidium to the progeny via eggs. The developing eggs harboured vegetative stages ofN. locustae, and development of the microsporidium occurred during embryonation. Spores accumulated in the yolk and, after blastokinesis, both the yolk and the spores were enclosed in the midgut of the embryo. germinated spores infected the functional midgut epithelium and invaded internal tissues. The mortality of newly hatched instars was high when embryonic tissue had been infected during development.
