    Notes: Abstract Human herpesvirus 6 (HHV6) has been reported as a rare cause of meningoencephalitis and leukoencephalitis. We present an HIV-infected patient with ¶lesions of progressive multifocal leukoencephalopathy (PML), but also meningoencephalitis apparently due to HHV6. Immunohistochemistry for HHV6 antigens and in situ polymerase chain reaction for HHV6 genome showed many positive lymphocytes and microglia in the meningeal and cortical lesions. More importantly, dead and dying neurons were conspicuous; some were undergoing neuronophagia and some displayed evidence of HHV6 infection. A pathogenic role for this almost universal, and usually commensal, virus in inflammatory brain lesions and PML is briefly discussed.

    Notes: Abstract A sensitive analytical method was established for the determination of Th and U in activated concrete samples. The method combines an anion-exchange separation step with an ICP-MS determination technique. In the ICP-MS measurement, a few μg mL–1 of Al and Ca, a few ng mL–1 of Mn, La, Ce, Nd and Pb and pg mL–1 amounts of Li, Zr, Nb and Ba coexisting in the anion-exchange fraction of Th and U did not interfere. No adverse interference effects were observed in real sample analyses. The obtained detection limits (3σ, n = 10) of Th and U were 2.3 and 1.8 pg mL–1, respectively. The analytical precisions for ca. 5 μg g–1 Th and ca. 1 μg g–1 U in real activated concrete samples were equally less than 7% RSD. The accuracies obtained by the analysis of GSJ rock standard samples were –18.1 to 0.4% for the Th determination and –14.0 to –5.7% for the U determination. The method uses the conventional absolute calibration curve. The internal standard calibration is unnecessary.

    Notes: We report the case of a 22-year-old woman with a nevoid plaque that we termed localized follicular hamartoma. The plaque was noticed at puberty on a unilateral site of the face and scalp. Clinically, it revealed numerous, skin-colored to light brown papules alone and in groups, occasionally bearing a single hair. Histologically, branched epithelial nests of squamoid and/or basaloid cells were revealed in connection with the interfollicular epidermis and the upper portions of hair follicles, of which the lower portions showed normal structures. Immunohistochemically, the epithelial nests showed the keratin expression consisted with that of the infundibular epithelium. S-100-positive cells were found in the epithelial nests and the stroma. Factor XIIIa-positive dendritic cells were numerous in adjacent to the epithelial nests. Ultrastructurally, immature melanocytes with a small number of premelanosomes and Merkel cells were found in the nests. Stromal dendritic cells showed the adherent features of the cytoplasmic processes to anchoring fibrils or basal lamina of the epithelial nests. From these findings, our case is a hamartoma, which seems to be an abortive growth of secondary hair germs with a limited differentiation to the upper follicular portion.

    Notes: Lesions of trachea cuticles are a pathological histological characteristic of bronchial asthma. Furthermore, collected tracheal cuticles desquamated from the respiratory tract are found in patients’ sputum when asthma attacks occur or after the induction of allergen inhalation. From these facts, it is assumed that desquamation of trachea cuticle cells is a pathological symptom of bronchial asthma. However, there has not been any chronological report of desquamation of trachea cuticles through the process of bronchial asthma attacks.For this report, we made an experimental bronchial asthma model using guinea pigs, and conducted chronological examinations of trachea cuticle lesions related to pathological symptoms of bronchial asthma using a transmission electron microscope and a scanning electron microscope.The experimental asthma models were made by injection of ovalbumin into the abdominal cavity of guinea pigs. Then the airway responses to inhaled aerosolized ovalbumin were induced. The trachea were enucleated and examined under an optical microscope, a transmission electron microscope (hereafter abbreviated as TEM), and a scanning electron microscope (hereafter abbreviated as SEM) after 1, 2, 4, 8, 12, 24 h and 7 days after the immediate airway responses.Intercellular oedema of ciliated epithelium was observed in the sensitization groups immediately after the immediate airway response. SEM observation revealed increased mucus secretion and shortening of cilium. A slight case of desquamation or deciduation of ciliated epithelium was also beginning to appear. TEM observation revealed a dilation of ciliated epithelium intervals. Infiltration of eosinophilic leucocytes was already detectable beneath the ciliated epithelium. The degree of ciliated epithelium desquamation and infiltration of eosinophilic leucocytes progressed with time. When the late airway response occurred 4 hours later, eosinophilic leucocytes had increased drastically, and ciliate epithelium had desquamated to the extent that basal cells were exposed. Seven days after the immediate airway response, epithelium intercellular oedema had improved, and cilium had been reproduced.These results suggest that desquamation of epithelium caused by trachea cuticle lesions appears at an early stage of an asthma attack, owing to the contraction of the trachea, and that the damage is intensified by the infiltration of eosinophilic leucocytes.

    Notes: Aim : To characterize the histological features of intestinal Behçet's disease and simple ulcer syndrome and to clarify the possible mechanisms involved in their development by analysing the type of inflammatory infiltrates in the diseased intestine and the expression of adhesion molecules on endothelial cells.Methods and results : Tissue samples from 10 patients diagnosed as having intestinal Behçet's disease or simple ulcer syndrome were studied. Immunohistochemical studies were performed on paraffin-embedded tissue using a labelled streptavidin–biotin method. In all cases, phlebitis was remarkably seen in submucosal inflammatory lesions, but the adjacent arteries were not affected. Inflammatory infiltrates around the affected vessels consisted of neutrophils and mononuclear cells, and neutrophils predominated over CD68+ macrophages and lymphocytes. The majority of mononuclear cells were CD3+ T cells, and CD4+ cells were more frequent than CD8+ T cells. As for adhesion molecule expression, intercellular adhesion molecule-1, but not vascular cell adhesion molecule-1, was expressed in most endothelial cells of the vessels with phlebitis, some of which were also positive for HLA-DR.Conclusion : Neutrophilic phlebitis may be involved in the pathogenesis of intestinal Behçet's disease and simple ulcer syndrome.

    Notes: We have prepared a sintered CoSb3–FeSb2 composite where the FeSb2 particles are dispersed in the CoSb3 matrix by mechanical grinding (MG) and hot pressing, and investigated the Seebeck coefficient, electrical resistivity, and thermal conductivity in order to estimate the corresponding figure of merit. The thermal conductivity of the composite is lower than that of CoSb3. The electrical resistivity of the composite increases with increasing MG time, but it is lower than that of CoSb3 at high temperature. The decrease in the thermal conductivity and the lesser increase in the electrical resistivity are ascribed to the enhancement of phonon scattering caused by the dispersion of FeSb2 particles in the CoSb3 matrix and the low electrical resistivity of FeSb2, respectively. As a result, the composite whose molar ratio of CoSb3 to FeSb2 is 0.7:0.3 and the MG time is 25 h has a maximum figure of merit value of 6.1×10−4 K−1 at 756 K. This value is much larger than the maximum value of CoSb3 at 482 K of 3.2×10−4 K−1. © 2000 American Institute of Physics.

    Notes: (Zn1−yMgy)1−xAlxO powders were synthesized by the polymerized complex method and then consolidated by spark plasma sintering apparatus. The microscopic structure and thermoelectric properties were examined comparing with the experimental results of the samples prepared by the conventional solid-state reaction method. A small amount of ZnAl2O4 spinel phase as the second phase was observed in the sintered samples with x≥0.02 by x-ray diffraction and a scanning electron microscope. The grain size of the samples prepared by the polymerized complex method is much smaller than that of the samples prepared by the conventional solid-state reaction method. The absolute values of the Seebeck coefficient and electrical resistivity decrease with increasing x up to about x=0.01, but above x=0.01 they are almost independent of x. This result indicates that the solubility limit of Al in Zn1−xAlxO is about x=0.01, which is also confirmed by 27Al nuclear magnetic resonance spectroscopy. At a fixed composition of x, the absolute values of the Seebeck coefficient and electrical resistivity for the samples prepared by the polymerized complex method are smaller than those for the samples prepared by the solid-state reaction method, which indicates that the doping of the carrier into the material can be more easily realized in the samples prepared by the polymerized complex method. The thermal conductivity decreases with increasing x, but the further suppression of the thermal conductivity was attained by the additional substitution on the Zn site by Mg. The Seebeck coefficient of (Zn1−yMgy)1−xAlxO is almost independent of Mg content y, but the electrical resistivity increases with increasing y. As a result, (Zn0.90Mg0.10)0.9975Al0.0025O shows a maximum dimensionless figure of merit of 0.10 at 1073 K. © 2002 American Institute of Physics.

    Notes: Aim : To investigate the effect of the eradication of Helicobacter pylori on histological gastritis.Methods : Twenty-six patients with moderate to severe atrophy received successful eradication therapy of H.pylori. Four patients dropped out and 22 were followed up prospectively for 5 years. The grades of gastritis were estimated from gastric biopsy specimens. The grade of intestinal metaplasia was also evaluated by dye-endoscopy using methylene blue (methylthioninium chloride). The serum levels of pepsinogen, gastrin and anti-parietal cell antibody were also determined.Results : The grades of atrophy decreased in patients with successful eradication therapy in the gastric corpus (before vs. 5 years after eradication, 2.09 ± 0.15 vs. 0.91 ± 0.17; P 〈 0.01) and in the antrum (2.14 ± 0.17 vs. 1.36 ± 0.17; P 〈 0.01). The levels of intestinal metaplasia were also decreased in the corpus (0.91 ± 0.24 vs. 0.50 ± 0.16; P 〈 0.05) and in the antrum (1.41 ± 0.20 vs. 1.00 ± 0.16; P 〈 0.05), which was also demonstrated by the methylene blue (methylthioninium chloride) staining method (33.4 ± 8.2% vs. 23.0 ± 6.5%; P 〈 0.05). The improvement of corpus atrophy correlated well with the high serum level of pepsinogen I (P = 0.005), but showed no correlation with the levels of anti-parietal cell antibody.Conclusions : These results suggest that gastric atrophy and intestinal metaplasia are reversible events in some patients.

    Notes: Summary We report a case of febrile ulceronecrotic Mucha–Habermann disease (FUMHD) in a 21-year-old man. This disease is a severe form of pityriasis lichenoides et varioliformis acuta (PLEVA) and is characterized by the sudden onset of diffuse ulcerations associated with high fever and systemic symptoms. It is sometimes lethal especially in elderly patients. In the present case, intense generalized maculopapular erythematous plaques with central necrosis developed progressively in association with a high fever. Initial treatment with systemic betamethasone had been unsuccessful and the skin lesions, which covered about 50% of the body surface, became severely ulcerated. Although the development of new lesions had ceased spontaneously, widespread ulceration of the skin remained. Debridement of the necrotic skin and skin grafting using cultured epidermal autografts and meshed allografts of cadaver skin led to prompt reepithelization.
