    Notes: Summary We investigated 270 patients (321 hips) treated by Lord-type cementless total hip arthroplasty (THA) and reported results in 234 hips that could be directly observed among 251 that were 5-year or more after operation. The mean Merle d'Aubigné and Postel hip score improved from 8.9 before operation to 15.7 at the final follow-up. However, our 20-year experience showed revision due to aseptic loosening in 27 patients. In particular, biological fixation could not be achieved in some cases treated using a screw-in ring for acetabular dysplasia or using the Fluted type stem with a smooth surface. This may be the cause of aseptic loosening, and further follow up is necessary.

    Notes: Abstract We report an unusual case of open vertical dislocation of the talus without malleolar fracture following motor vehicle accident. Most previously reported cases are medial, lateral, posteromedial and posterior dislocations. The anterolateral, posterolateral, and purely superior dislocations of the talus make up smaller a percentage. The mechanism of this injury appears to be forced eversion of the foot when it is maximally plantar flexed and axially loaded. Our case has treated by open reduction of the talus and anatomical repair of the disrupted deltoid ligament at the time of initial incision and open reduction with internal fixation of the calcaneus also achieved good long-term functional and radiographic results.

    Notes: Abstract Pancreaticobiliary maljunction (PBM), a congenital anomaly, causes regurgitation of pancreatic juice into the biliary tract, where it exerts a hazardous influence. However, changes in the common bile duct (CBD) remain obscure due to a lack of suitable experimental models. Using cats, we have developed an experimental model of PBM without bile-duct dilatation that allows the pure effects of PBM to be studied. Histologic and cellular kinetic changes in the CBD were analyzed in 6 controls and 9 experimental animals that survived for more than 6 months. CBD sections were stained with hematoxylin and eosin and a monoclonal antibody to the proliferating cell nuclear antigen (PCNA). Invaginations of the bile-duct epithelium or parietal sacculi increased, and peribiliary glands were well-developed. PCNA-positive cells significantly increased in the CBD, especially in the parietal sacculi and glands. It is concluded that PBM increases the cell cycle in CBD epithelium and subsequently developed peribiliary glands. These developed glands may be associated with the formation of protein plugs, often seen in patients with PBM.

    Notes: Abstract We previously reported a histomorphometric study of iliac bone obtained at the time of cementless total knee arthroplasty (TKA) in patients with rheumatoid arthritis (RA). In this paper, we further extended our analysis to investigate the postoperative results and radiographs after TKA in association with bone metabolism. All the patients were female, ranging in age from 38 to 68 years (mean, 56.8 years). Histomorphometric results demonstrated that bone volume and trabecular thickness were decreased, whereas bone absorption and bone formation rate (BFR) were increased. The bone in patients with mutilating disease (MUD) showed remarkably increased bone turnover and osteoporosis compared with those of the more erosive subset (MES) patients. Sixteen out of the 20 patients were alive at the time of follow-up (follow-up period between 5 years 10 months and 8 years 11 months). Among them, 10 patients could be further examined. Radiolucent lines (RLL) were assessed by the modified Knee Society evaluation (scoring) system. The number of RLL (N-RLL) were correlated with BFR in morphometric examination. In addition, the weight-bearing zones in the N-RLL were more significantly correlated with BFR. The Japanese Orthopedic Association (JOA) score and pain score negatively correlated with eroded bone surface. The present study indicated that bone dynamics, rather than the bone volume at the time of TKA, were involved in the presence of RLL and in pain after surgery for TKA.

    Notes: Abstract  Renal biopsy was performed in a 12-year-old girl with hematuria and proteinuria which was first detected at the age of 7, and the findings were the mesangial proliferative glomerulonephritis with IgG and C3 deposits. The routine blood examination for the biopsy disclosed the presence of the prolonged activated partial thromboplastin time and the biological false positive reaction in the syphilis test. These results led us to the further investigation, which revealed the presence of high titers of anticardiolipin antibodies. Since this girl presented no extra-renal symptoms of systemic lupus erythematosus (SLE) and had negative serologic tests for SLE, we hypothesize that her nephritis is closely related to antiphospholipid antibodies.

    Notes: Abstract We document a case of bilateral ulnar nerve palsy that developed in an 27-year-old Japanese man who had bilateral hypoplasia of the humeral trochlea. Surgical management produced good results regarding the ulnar nerve palsy. The pathogenesis of the nerve paresis in this particular condition is discussed. There have been no reports outside Japan. Whether this deformity occurs only in persons of Japanese extraction or is simply overlooked by foreign surgeons is an interesting question.

    Notes: Abstract The initial solid fixation of an uncemented acetabular component affects the amount of bone ingrowth. We had several problems with broken screws in cases of acetabular revision. In recent years, the development of uncemented components without screws has attempted to improve these problems. We started to use “press-fit”-type acetabular shells in November 1996. Our thirty cases undergoing 2 mm under-reaming show good initial stability. The aim of our biomechanical study was to assess the most suitable degree of under-reaming of the bony acetabulum for the implantation of an uncemented hemispherical porous coated component.

    Description / Table of Contents: Résumé  Les auteurs rapportent 4 cas de fracture ancienne du bassin pour lesquels les situations d’urgence mettant en jeu le pronostic vital n’avaient pas permis initialement un traitement correct. Il s’agit de quatre patients dont l’âge s’échelonne de 24 à 51 ans. La gêne fonctionnelle était représentée par une boiterie, des douleurs dans les fesses et les membres inférieurs. Le délais entre l’accident et l’intervention était de 2,5 à 3 ans. Les douleurs et la capacité de marche ont été améliorées chez tous les patients mais une paralysie n’a pas pu être améliorée.
    Notes: Abstract  We report on 4 patients, aged 24–51 years, with untreated pelvic ring fractures. The chief complaints were gait disturbances and pain in the buttocks and lower limb. The period between the initial injury and the surgical treatment ranged from 2.5 months to 3 years. Palsy was not improved, but pain and gait ability was improved in all patients.
