    Notes: Abstract The Texas Gulf of Mexico coast is experiencing high (5–11 mm/yr) rates of relative sea level (RSL) rise that are the sum of subsidence and eustatic sea level (ESL) rise. Even higher rates are associated with areas of groundwater pumping from confined aquifers. We investigate the possibility of deep petroleum production as a cause for the high regional rates of subsidence. The northeast Texas coast was chosen for the study because it has a high rate of RSL rise, very limited groundwater production, and a long history of petroleum production. We examine in detail the Big Hill and Fannett fields, for which adequate bottom hole pressure (BHP) and well log data are available. The hypothesis of deep petroleum production is tested in three ways. First, industry BHP tests show many of the fields are depressurized to far below hydrostatic pressures. Second, analysis of BHP data over time in the Big Hill and Fannett fields indicates that some Zones in these fields were below hydrostatic when production commenced. This indicates that depressurization from production in neighboring fields or zones within the same field is not limited to the production zone. Third, three models for subsidence (a general 1-D regional model, an intrareservoir model, and a reservoir bounding layer model), using reasonable hydrogeological parameters, predict subsidence within the inferred range of data. The latter two models use data from the Big Hill and Fannett fields. Additional verification of the hypothesis that deep petroleum production is causing or accelerating regional subsidence will require the collection and analysis of data on the subsurface hydrogeological parameters and detailed measurements of the spatial and temporal distribution of subsidence along the Texas Coast.

    Notes: Background The house dust mite has been shown to be an important source of domestic allergens associated with immediate hypersensitivities. The Group I mite allergens Der p I from Dermatophagoides pteronyssinus and Der f I from D. farinae display extensive amino acid sequence homology and have similarities with cysteine protease enzymes.Objective The availability of the complete amino acid sequences for these allergens allowed us to search for the allergic detertninants within these molecules. The aim of the present investigation was to identify any continuous IgE-binding epitopes within these amino acid sequences. We also sought to test the validity of previously reported Der p I peptide epitope sequences.Methods In order to identity any continuous IgE epitopes, the amino acid sequences of Der p I and Der f I were synthesized as decapeptides overlapping in sequence and coupled to plastic pins. The specific IgE-binding capacity of these peptides was assayed using an enzyme-linked biotin-streptavidin procedure and sera from patients known to be sensitive to these allergens. Previously reported Der p I peptide epitopes were synthesized as free peptides and tested for their ability to inhibit specific IgE binding to allergen extract discs.Results None of the pin-coupled Der p I or Der f I peptides was found by the continuous epitope mapping procedure to bind significantly to specific IgE in the sera of hypersensitive patients. The previously reported Der p I peptide epitopes did not inhibit specific IgE binding to mite extract discs.Conclusion The specific IgE binding epitopes of the house dust mite allergens Der p I and Der f I are discontinuous in nature.

    Notes: Following induction of epidural analgesia with 0.375% bupivacaine, 120 healthy parturients were randomly assigned in a double blind fashion to one of eight infusion groups. All received 8ml.h-1 of0.125% bupivacaine either alone (control group), or with alfentanil at 133, 266 or 400μg.h-1 (groups A1-A3) or with diamorphine at 133, 266, 400 or 533μg.h1 (groups D1-D4). Significantly longer top-up intervals were achieved with the two highest doses of both alfentanil and diamorphine when compared with bupivacaine alone (p〈0.01), making the minimum effective doses 266 μg.h-1 of alfentanil and 400 ng.h-1 of diamorphine. Perineal analgesia was better in all the opioid groups compared with the control group (p 〈 0.05). The intensity of motor block was greater in the control group (p 〈 0.05). The incidence of pruritus did not differ between groups. The highest dose of diamorphine caused significantly more nausea. No significant neonatal side-effects were demonstrated.

    Notes: Alkalinisation of local anaesthetic drugs is a controversial technique for improving regional blockade. Forty-two patients scheduled for upper limb surgery received axillary brachial plexus anaesthesia using a cannula technique. Patients were randomly allocated to receive either lignocaine 1.5% with 1 in 200 000 adrenaline (pH = 4.2) or lignocaine 1.5% with I in 200000 adrenaline (pH = 7.2). There was no significant difference in the incidence of satisfactory blockade or distribution of anaesthesia between the two groups. The percentage of patients with complete anaesthesia at 10, 20 and 30 min following injection was significantly increased in the alkalinised group with regard to the ulnar and median nerves, and the median cutaneous nerve of the arm (p 〈 0.05). In the alkalinised group, there was a significant reduction in the time to useful anaesthesia and a reduced requirement for adjuvants (p 〈 0.05). There was no effect on the duration of anaesthesia.

    Notes: We report the changes observed in a number of pulmonary function tests performed on 36 patients undergoing Caesarean section under spinal anaesthesia. The tests comprised peak expiratory flow, forced expiratory volume in one second, forced vital capacity, forced expiratory volume in one second to forced vital capacity ratio and the maximal mid-expiratory flow. Significant changes occurred that are consistent with a restrictive ventilatory defect. These changes persisted for four hours after the induction of spinal anaesthesia. Administration of 35% oxygen by facemask failed to change significantly fetal umbilical vein pH or partial pressure of oxygen.
