    Description / Table of Contents: Summary Tyrosinase mRNA produced by melanoma cells can be detected by reverse transcriptase-polymerase chain reaction (rt-pcr) with fine-needle tissue punctures. Fine-needle punctures from six suspected skin and lymph node metastases in three patients with malignant melanoma were analysed via rt-pcr for tyrosinase mRNA. All the suspected metastases were surgically removed and histologically examined separately. In each case the rt-pcr results and the histological diagnosis corresponded. This less invasive and highly sensitive method could prove to be a useful alternative in the diagnosis of melanoma metastasis.
    Notes: Zusammenfassung Melanozytäre Zellen unterscheiden sich in ihrem Metabolismus von anderen Zellen durch die Expression des Enzyms Tyrosinase. Bei 3 Patienten mit malignem Melanom konnte an 6 Feinnadelpunktaten von fraglichen Haut- und Lymphknotenmetastasen die Tyrosinase-mRNA mittels Reverser Transkription/Polymerasekettenreaktion (RT/PCR) nachgewiesen werden. Die Veränderungen wurden im Folgenden operativ entfernt und histologisch aufgearbeitet. Die Ergebnisse der RT/PCR korrespondierten jeweils mit der histologischen Diagnose. Diese wenig invasive und sehr sensitive Methode der Feinnadelpunktion mit anschließender molekularbiologischer Untersuchung könnte für die Diagnostik von Melanommetastasen als Alternative zur diagnostischen Exzision bzw. Aspirationszytologie bedeutsam werden.

    Notes: Abstract. The etiology of avascular necrosis (AVN) is multifactorial. Independent of its etiology and localization it shows typical pathologies and radiological images. In the early stages localized subchondral edema is characteristic. In 50 % of all cases accompanying joint effusion may be found. Due to necrosis of the cells of bone marrow and bone fibrovascular, reactions with hyperemia can be delineated. These reactions allow us to visualize necrosis indirectly. The best imaging methods are MRI and, to a lesser extent, bone scintigraphy. In later stages calcification as well as new bone formation and microfractures are typically demonstrated and visualized best with plain X-rays and CT. Why reparations in many cases, particularly in the hip, are incomplete and may stop in any stage is unknown. Over years clinically complete silent AVNs are not an uncommon finding. Prognosis depends on the localization and size of the AVN. The number of repair mechanisms is best outlined with contrast-enhanced MRI and return of fatty marrow.

    Notes: A 51-year-old white male suffering from metastatic malignant melanoma of the skin presented with lymph node metastases and paraneoplastic retinopathy 4 years after resection of the primary tumour. There were no cerebral metastases. Ocular symptoms consisting of night blindness and sensations of ‘shimmering lights’ persisted after total resection of the inguinal lymph node metastases and administration of dacarbazine and prednisone. Perimetry of both eyes was abnormal with concentric restriction. Electroretinography showed significantly reduced amplitudes in both eyes. Only 11 patients with melanoma-associated retinopathy (MAR) have been described. High titres of autoantibodies against whole retina extract were detected by enzyme-linked immunosorbent assay (ELISA) reactions. Indirect immunohistochemistry showed strong autoantibody activity against retinal bipolar cells.

    Notes: A theoretical model is presented which describes time dependent charging phenomena in insulators irradiated by an electron beam. This so called dynamic double layer model (DDM) is based on the simplified assumption of charge development in two axi-symmetric cylindrical volumes at the insulator surface and at a certain depth below the surface, respectively. Important physical interaction mechanisms like the electrostatic interaction between the surface potential and emitted secondary electrons, the transport characteristics of internally generated secondary electrons in a positively charged insulator surface, as well as radiation induced electrical conductivity are included in the DDM model. Experimentally determined stationary and time dependent charging behaviour of Al2O3 single crystals (sapphire) irradiated with an electron beam between 1 and 15 keV energy can be qualitatively explained by the DDM model. © 1995 American Institute of Physics.

    Description / Table of Contents: Zusammenfassung Die Labrumläsion des Hüftgelenks ist eine häufige Begleitläsion bei der Hüftdysplasie des Erwachsen und gilt als Zeichen einer biomechanischen Dekompensation des Gelenks. Die MR-Arthrographie kann diese Labrumläsionen mit einer Treffsicherheit von über 90 % diagnostizieren. Bis heute sind jedoch das klinische Bild, die radiologische Abklärung und die therapeutischen Konsequenzen noch wenig bekannt. In einer prospektiven Studie wurden 56 Patienten mit dem klinischen Verdacht einer Labrumläsion nach einem standardisierten Untersuchungsprotokoll anamnestisch, klinisch, röntgenologisch und mit einer MR-Arthrographie abgeklärt. Die klinischen Zeichen einer Labrumläsion wurden anhand von 6 Kriterien und 2 Provokationstesten erhoben. In 11 % war die klinische Verdachtsdiagnose falsch. Die häufigste Übereinstimmung mit der MR-Arthrographie erreichte der „messerscharfe“ Leistenschmerz (100 %), der Einklemmtest (100 %) und das schmerzhafte Einknicken (83 %). Bei 35 % der Patienten lag eine mäßige Dysplasie (Grad 2) und bei 52 % eine schwere Dysplasie (Grad 3 und 4) vor. In 64 % fand sich unabhängig vom Dysplasiegrad keine oder nur wenig Arthrose (Grad 0 und 1). In 16 % zeigte das Röntgen eine solitäre Pfannendachzyste, die sich in der MR-Arthrographie in allen Fällen als intraossäres Ganglion darstellte. Labrumläsionen vom Typ A (posttraumatisch) fanden sich in 23 % der Patienten ausschließlich jedoch bei den nicht oder mäßig dysplastischen Hüftgelenken (Grad 1 und 2). Labrumläsionen vom Typ B (degenerativ) fanden sich bei 67 % der Patienten hingegen ausschließlich bei schwer dysplastischen Hüftgelenken. Die klinischen Zeichen einer Labrumläsion sind typisch, können jedoch auch bei anderen Hüftgelenkspathologien auftreten. Wir empfehlen daher für Patienten mit dem klinischen Verdacht einer Labrumläsion die röntgenologische Abklärung einer Dysplasie und die Durchführung einer MR-Arthrographie.
    Notes: Summary Labral lesions are common findings in residual hip dysplasia, indicating biomechanical decompensation of the hip joint. MR-Arthrography has shown an excellent accuracy of over 90 % to detect these lesions. Nevertheless, so far clinical sings, radiological diagnosis and therapeutical consequences are not well known. In a prospective study, patients suspicious for labral lesions were evaluated using a standard clinical protocol, including history, clinical signs, radiography and MR-Arthrography. Clinical signs were tested by six criteria and two provocation tests. In 11 % patients clinical suspicious was wrong. Best agreement with MR-Arthrography was found for “knife sharp” groin pain (100 %), impingement test (100 %) and painful giving way (83 %). 35 % of patients showed minor (grade 2) and 52 % severe (grade 3 and 4) dysplasia. Independent from grade of dysplasia, no or only slight arthrosis (grade 0 and 1) was found in 64 % of patients. In 16 % a single acetabular cyst could be detected on radiographs, which all could be identified as intraosseous ganglia on MR-Arthrography. Labral lesions type A (post traumatic) were found in 23 % of the patients with only no or minor dysplasia (grade 1 and 2), whereas labral lesions type B (dysplastic) were found in 67 % of the patients with severe dysplasia (grade 3 and 4). Clinical signs for labral lesions are typical but can also be observed in other pathologies of the hip joint. Based on the findings of this study, we recommend radiographic evaluation for dysplasia and MR-Arthrography in patients with clinical suspicion for labral lesions of the hip joint.
