    Notizen: Abstract The severity and predicted outcome of postoperative Pseudomonas aeruginosa (P. aeruginosa) infections (PPAI) was evaluated using a severity scoring system based on a simplification and modification of the APACHE II system. A total of 86 patients in whom P. aeruginosa was isolated from various sources were examined. PPAI developed in 50 patients, resulting in an overall mortality rate of 24%. An increased severity score (SS) correlated with an increased risk of developing PPAI. Thus, PPAI developed in 33% of the patients with an SS of 0–1, in 66.7% of those with an SS of 2–3, and in 100% of those with an SS of 6 or higher. Moreover, the mortality rate of the patients with an initial score of 6 or higher was 50%. The mean (±SD) initial severity score was 5.4±2.9 for survivors and 2.9±2.6 for nonsurvivors (P〈0.01). In the patients who subsequently died, the SS remained high throughout the clinical course despite therapy, whereas in the survivors the SS decreased progressively, reflecting a favorable clinical course. These results suggest that our severity scoring system was useful for predicting outcome and monitoring the response of PPAI to therapy.

    Notizen: Abstract We examined the clinical features of 14 men (mean age 72 years) with postoperative enteritis caused by methicillin-resistantStaphylococcus aureus (MRSA). The patients had all undergone surgery for the treatment of digestive diseases and had received antibiotic prophylaxis consisting of an extended-spectrum cephem. Diarrhea appeared a mean of 3.3 days postoperatively and lasted for 5 days on average. In severe cases organ insufficiency was involved. Coagulase-positive staphylococci were the predominant organisms isolated from watery diarrhea. In 13 of 14 patients, coagulase type II isolates producing enterotoxins A, C and toxic shock syndrome toxin-1 (TSST-1) withenterotoxin A, C, andtst genes were isolated. These strains were sensitive to vancomycin and arbekacin; however, they were highly resistant to many other antibiotics. We also investigated the effects of a glucocorticoid hormone and gamma globulin on production of tumor necrosis factor-α (TNF-α) and interleukin-2 (IL-2) obtained from healthy volunteers. TNF-α and IL-2 production was enhanced by TSST-1 and the supernatant of Iscove-modified dulbecco medium, in which coagulase type II isolates producing enterotoxins A, C and TSST-1 with enterotoxin A, C were cultured for 24h. Both glucocorticoid hormone and gama globulin suppressed TNF-α and IL-2 production, thus suggesting that these drugs may be effective in treating postoperative MRSA enteritis.

    Notizen: Abstract This study was designed to determine the influence of ileal pouch capacity and anal sphincteric function on the clinical outcome after ileal pouch-anal anastomosis. A total of 24 patients who had undergone ileal pouch-anal anastomosis (J pouch) for ulcerative colitis were studied. The 24-hour stool frequency was found to be inversely correlated with the sensitivity threshold volume (STV), maximal tolerance volume (MTV), and distensibility, but was independent of the maximal resting pressure and maximal squeeze pressure. Patients experiencing nocturnal fecal incontinence had maximal resting pressures that were significantly lower than those of nocturnally continent patients. Among the patients with fecal incontinence, those with frequent soiling had lower resting pressures, STV, and distensibility than the patients with intermittent spotting. In addition, the STV in patients needing nocturnal evacuation were lower than those of patients who did not evacuate after falling asleep. The conclusions are as follows. Both stool frequency and the need for nocturnal pouch evacuation correlated directly with pouch volume. Anal incontinence was more common in patients with low internal sphincteric function. In addition, frequent and gross nocturnal incontinent patients demonstrate a worse function in both the anal sphincter and reservoir than those with intermittent spotting.

    Notizen: Abstract A 29-year-old woman was hospitalized with a 1-month history of postprandial epigastric pain, nausea, and vomiting. An upper gastrointestinal tract X-ray series showed a marked narrowing of the pyloric region. A histological examination of duodenal mucosal biopsy samples showed granulomatous inflammation, and thus a diagnosis of intrinsic duodenal Crohn's disease was made. A second upper gastrointestinal tract X-ray revealed a persistent gastric outlet obstruction. At laparotomy, the duodenal wall was found to be thickened over a distance measuring 3.5cm in length from the pyloric ring. A longitudinal incision was made over the entire length, up to 5.5cm beyond the pyloric ring on either side, while Finney-type anastomosis was also performed. A postoperative upper gastrointestinal tract X-ray showed an improvement in the gastroduodenal passage. Enteral nutrition therapy was provided postoperatively. Omeprazole was administered at a dose of 20mg/day for 2 months. The patient currently remains on maintenance therapy with famotidine at 20mg/day and is clinically doing well.

    Notizen: Abstract Wood charcoal carbonized at various temperatures was analyzed by X-ray photoelectron spectroscopy (XPS), Fourier-transform infrared spectroscopy (FTIR), and X-ray diffractometry to investigate the changes of chemical structures during the carbonization process. From the infrared spectra, the carbon double bonds and aromatic rings were seen to form at a carbonization temperature of about 600°C. From the XPS spectra, the ratio of aromatic carbons increased in the temperature range 800–1000°C and over 1800°C. The condensation of aromatic rings proceeded as carbonization progressed. The drastic reduction of electrical resistivity of charcoals was observed in almost the same temperature range. It was found that the condensation of aromatic rings had some relation to the decline in electrical resistivity. Wood charcoal carbonized at 1800°C was partly graphitized, a finding supported by the results of X-ray diffraction and XPS. The functional groups containing oxygen diminished with the increase in carbonization temperature.

    Notizen: Abstract A practical approach to enhancing the fire retardancy of wood-based materials by adding fire-retardant chemicals to the glue was developed. Plywoods were manufactured using urea melamine formaldehyde resin mixed with ammonium pentaborate or dihydrogen phosphate. Treated plywoods had better incombustibility than untreated ones. X-ray photoelectron spectroscopy (XPS) analysis clearly demonstrated the distribution of boron and phosphorus, which had migrated from the glue to the wood, contributing to better fire retardant properties. The cross-sectional micrographs from scanning electron microscopy showed that untreated specimens exhibited a foamy structure near the interface in the glue layer and the deformed structure of wood cells. The cell structure and cell wall thickness retained intact in the specimens treated with urea melamine formaldehyde resin mixed with ammonium pentaborate or dihydrogen phosphate. When observing the effect of the thickness of overlay veneers on incombustibility, a shorter glowing time was obtained from the specimens with a thicker surface layer when the fire retardant chemical was added at 2%, but the differences were smaller at the higher chemical retention of 4%. A similar tendency was observed for the char length.

    Notizen: Abstract Pit fractures of refractory coniferous heartwoods caused by precompression in the radial direction were investigated and are discussed in terms of improved liquid penetration. Small cracks appeared at the boundary between the torus and margo, along the outer margin of the margo, and on the torus when specimens were compressed and deformation was fixed by drying. The remarkable cracks were generally observed for Cryptomeria japonica D. Don. Pseudotsuga menziesii Franco showed peculiar detachment of the torus from the pit border, and Larix leptolepis Gordon exhibited only small cracks on the torus. These fractures patterns were clearer when the precompressed specimens were recovered by water impregnation and then redried.

    Notizen: Abstract Sugi (Cryptomeria japonica D. Don) wood powder was carbonized at varying temperatures and used as a material to remove heavy metals from their aqueous solutions. Single solutions of mercuric chloride and mixed aqueous solutions containing lead nitrate, arsenic chloride, and cadmium chloride as well as mercuric chloride (1, 5, and 10 ppm) were prepared to determine the efficiency of removing heavy metals by these materials. Wood powder and carbonized wood at 200°, 600°, and 1000°C removed mercury within the concentration range 1–10ppm; mercury was preferentially removed even when mixed with other heavy metals. Wood powder carbonized at 1000°C achieved the best removal of heavy metals among the wood-based materials and even commercial activated carbon in both single and mixed solutions.

    Notizen: Abstract Seven species of bacteria were isolated and identified from ponded sugi (Cryptomeria japonica D. Don) logs, and six species showed potent wood-degrading activities. To evaluate the effects of these isolated bacteria on the drying and anatomical characteristics of wood, small fresh blocks of sugi were immersed in water suspensions containing bacteria for 1–7 months. The permeability and drying properties were evidently improved. Most of the encrusting substances adhering to the cell lumens and the pit chambers were removed, and the pit membranes were destroyed. These anatomical changes due to bacterial activity were assumed to improve the permeability of sugi wood.
