    Notizen: In a double-blind, placebo-controlled study we investigated the effect of a single pre-induction intravenous dose of dexmedetomidine 2 μg.kg−1 on anaesthetic requirements and peri-operative haemodynamic stability in 50 patients undergoing minor orthopaedic and general surgery. Patients were anaesthetised with nitrous oxide/oxygen/fentanyl, supplemented if necessary with isoflurane. The mean (SD) intra-operative isoflurane concentration was lower in the dexmedetomidine-treated patients than controls (0.01 (0.03)% compared to 0.1 (0.1)%; p = 0.001) although six of the 25 treated patients required isoflurane at some stage. The haemodynamic response to tracheal intubation and extubation was reduced in the dexmedetomidine group as was intra-operative heart rate variability; postoperative analgesic and anti-emetic requirements and peri-operative serum catecholamine concentrations were lower in the dexmedetomidine group. Hypotension and bradycardia occurred more frequently after dexmedetomidine.

    Notizen: Abstract This paper studies the impact of order independence to the learnability of indexed families $$\mathcal{L}$$ of uniformly recursive languages from positive data. In particular, we considerset-driven andrearrangement-independent learners, i.e., learning devices whose output exclusively depends on the range and on the range and length of their input, respectively. The impact of set-drivenness and rearrangement-independence on the behavior of learners to their learning power is studied in dependence on thehypothesis space the learners may use. We distinguish betweenexact learnability ( $$\mathcal{L}$$ has to be inferred with respect to $$\mathcal{L}$$ ),class-preserving learning ( $$\mathcal{L}$$ has to be inferred with respect to some suitably chosen enumeration of all the languages from $$\mathcal{L}$$ ), andclass-comprising inference ( $$\mathcal{L}$$ has to be learned with respect to some suitably chosen enumeration of uniformly recursive languages containing at least all the languages from $$\mathcal{L}$$ ). Furthermore, we consider the influence of set-drivenness and rearrangement-independence for learning devices that realize thesubset principle to different extents. Thereby we distinguish betweenstrong-monotonic, monotonic, andWeakmonotonic orconservative learning. The results obtained are threefold. First, rearrangement-independent learning does not constitute a restriction except in the case of monotonic learning. Next, we prove that for all but two of the learning models considered set-drivenness is a severe restriction. However, class-comprising set-drivenconservative learning is exactly as powerful as unrestricted class-comprisingconservative learning. Finally, the power of class-comprising set-driven learning in the limit is characterized by equating the collection of learnable indexed families with the collection of class-comprisingly conservatively inferable indexed families. These results considerably extend previous work done in the field (see, e.g., [20] and [5]).

    Notizen: Abstract Thirty-three patients with fractures of the thoracolumbar spine were treated by fusion and Harrington instrumentation after early reduction and stabilisation by postural reduction or halo-bifemoral traction. The rod-long-fuse-short technique was used. Postoperatively, all patients were mobilised with an external support. The mean follow-up was 6 years (range 1–13 years). Twenty-eight patients were pain-free, three patients needed sporadic pain medication. One patient was not working due to pain in his leg. One patient had constant pain. Twenty-three patients returned to work. The mean kyphosis increased from 8° postoperatively to 13° at follow-up.

    Notizen: Abstract Objective: While there is a lot of experience with the design, conduct and interpretation of bioequivalence studies, the methodology of trials concerning therapeutic equivalence is still at an early stage of development. Two-armed equivalence studies involve special problems of interpretation, which can be partly solved by the introduction of a third (placebo) arm. We describe a trial in which the therapeutic equivalence of horse chestnut seed extract (HCSE) and compression treatment was to be demonstrated in patients with chronic venous insufficiency (CVI). Compression is regarded as the standard therapy in this field. However, the efficacy of compression in terms of the variable of primary interest, namely oedema reduction, has never been demonstrated according to current methodological rules. Thus, the `standard' had to be established in the trial itself. This can be achieved by demonstration of relevant superiority in comparison with placebo. Methods: Two hypotheses had to be tested: (1) the relevant superiority of compression compared with placebo as a precondition for (2) the at most irrelevant inferiority of HCSE in comparison with compression (`equivalence'). For both corresponding statistical tests, the irrelevance criterion – formulated as standardized difference – was set to 0.5. Results: Therapeutic equivalence could not be demonstrated following this design, because compression failed to be relevantly superior compared with placebo, even though HCSE was shown to be at most irrelevantly inferior compared with compression. Explorative analyses show that it is not possible to reject simultaneously both null hypotheses with the obtained data when using equal irrelevance limits for both tests. Conclusion: Although the primary objective of the trial could not be achieved, the results were encouraging. Thus, a new study was planned and started based on the observed data. The concept of a shifted null hypothesis may be applied to `routine' clinical trials too; using `no difference' as the null hypothesis in a trial does not seem to be meaningful when in fact an at least relevant difference is required.

    Notizen: Abstract  The antisecretory factor, AF, is a 41-kDa protein, cloned and sequenced from a human pituitary library. AF is a potent inhibitor of experimental intestinal hypersecretion in rats and pigs. An antiserum against the C-terminal of the truncated, recombinantly produced AF protein was raised in rabbits. The affinity-purified antiserum was used to study the expression of AF in mucosal membranes and in the pituitary gland of the pig; distinctly stained cells were found in lymphoid cells in the connective tissue of all parts of the gastrointestinal, respiratory and urinary tracts. Cytoplasmic AF was demonstrated in endocrine and epithelial cells in the pituitary gland. In situ hybridisation with a digoxigenin-labelled mRNA probe also demonstrated specific cytoplasmic staining in epithelial and lymphoid cells in all of these tissues. The cells stained by either method were similarly distributed topographically within the tissues. The results suggest that a specific defined cell population in these various tissues possesses the capability of both synthesising and storing the AF protein within the cellular cytoplasmic compartment.
