    Description / Table of Contents: Abstract Background: Droperidol even in low doses such as 0,5 mg to 1,25 mg can increase postoperative anxiety and state of tension. The aim of this study was to determine whether these side effects occur frequently following low-dose droperidol and to see whether these are dose related. Methods: 184 female in- and outpatients ASA grade 1 and 2 undergoing gynaecological laparoscopy were recruited to this prospective, double-blind study. General anaesthesia was standardized (induction with thiopentone, fentanyl 2 µg/kg and vecuronium 0,1 mg/kg, tracheal intubation, maintainance with enflurane in N2O/O2). Patients were randomly allocated to receive saline (n=45), 0,625 mg (n=46), 1,25 mg (n=47) or 2,5 mg (n=46) droperidol i.v. 10 minutes before the end of surgery. 1, 3, 6, and 24 hours postoperatively, the patients’ anxiety, state of tension and overall mood was evaluated using two psychological questionnaires which had been tested for the perioperative period (Erlanger anxiety and tension-scale / BSKE-EWL-test). Sedation was evaluated by the staff of the recovery room. In addition, postoperative nausea and vomiting (PONV) was assessed using a 100 mm visual analogue scale and by counting the episodes of retching or vomiting. PONV was then rated over the whole observation period as none, mild, moderate or severe using a fixed scoring algorithm. Statistical analysis was performed using the ANOVA and the chi2-test. Results: The patients did not differ with regard to biometric data, duration of surgery and anaesthesia. The postoperative scores for anxiety, state of tension and overall mood were not different between the groups at any observation time (Fig. 1: anxiety and tension: P=0,5687; figure 2: overall mood: P=0,0647). Quality of sleep in the first night after surgery was the same in all groups (Table 2 and 3). Sedation was not significantly different (Table 4; P=0,0704). Furthermore, duration of stay in the recovery room did not differ (P=0,4353). On the other hand, three patients from the 2,5 mg droperidol group had to stay unexpectedly on the ward overnight, because they had been too much sedated to be discharged at home. This was not the case with any patient from the other groups. Compared to placebo, PONV over the whole 24 h observation period was significantly reduced by droperidol (Fig. 3; P=0,0338): completely free from PONV: placebo: 41,3%, 0,625 mg droperidol: 67,4%, 1,25 mg droperidol: 53,2%, 2,5 mg droperidol: 71,7%. Also the severity of PONV was reduced. Conclusion: In gynaecological laparoscopy under general anaesthesia with tracheal intubation, we recommend droperidol 0,625 mg in the prevention of PONV, as it reduces PONV as well as 2,5 mg with no severe sedation in this dosage. Psychological side effects did not occur more frequently after droperidol compared to placebo in any of the investigated dosages.
    Notes: Zusammenfassung In vorliegender Studie wurden das Ausmaß postoperativer Angst, innerer Anspannung und allgemeiner Befindlichkeitsstörungen nach intraoperativ verabreichtem Droperidol und eine mögliche Dosisabhängigkeit dieser Nebenwirkungen untersucht. Zu diesem Zweck wurden erstmals zwei evaluierte Selbstbeurteilungsfragebögen eingesetzt. 184 ASA I und II Patientinnen, die sich einer gynäkologischen Laparoskopie in standardisierter Intubationsnarkose unterzogen, wurden in diese randomisierte, doppelblinde und plazebokontrollierte Untersuchung aufgenommen. Zehn Minuten vor Narkoseende erhielten die Patientinnen i.v. Kochsalz, 0,625 mg, 1,25 mg oder 2,5 mg Droperidol. Angst und allgemeines Befinden sowie Übelkeit und Erbrechen in der postoperativen Phase (PONV) wurden 1, 3, 6 und 24 h postoperativ registriert. Die Werte für Angst und allgemeines Befinden zeigten zu keinem Untersuchungszeitpunkt Gruppenunterschiede. Die Schlafqualität in der ersten postoperativen Nacht war in allen Gruppen gleich. Verglichen mit Plazebo wurde PONV durch Droperidol in jeder Dosierung gesenkt (p=0,0338): komplett frei von PONV: Plazebo: 41,3%, Droperidol 0,625 mg: 67,4%, 1,25 mg: 53,2%, 2,5 mg: 71,7%. Bei gynäkologischen Laparoskopien in Intubationsnarkose empfehlen wir zur Prophylaxe von PONV 0,625 mg Droperidol, da es ebensogut antiemetisch wirksam ist wie 2,5 mg. Psychische Nebenwirkungen wie Angst oder andere Befindlichkeitsstörungen treten in keiner der untersuchten Droperidoldosierungen vermehrt auf.

    Notes: Abstract  As part of a broader study, the aim of which is to identify soil factors that might be associated with yield decline of sugar cane, microbial biomass and protease activities were examined in soil samples collected from seven paired old and new land sites in three cane-growing districts of north Queensland. No consistent changes in soil protease activities were observed, although some sites exhibited specific effects, as a result of extensive periods of sugar cane monoculture. Soil microbial biomass, however, was significantly lower in those soils where sugar cane had been grown for extended periods. The implications of a lowering of soil microbial biomass on sugar cane yields and sustainability are discussed.

    Notes: This paper reports on a 4 × 4 factorial design experiment conducted to examine the combined effects of temperature and salinity on embryonic development and growth and survival of black-lip pearl oyster, Pinctada margaritifera (L.) larvae. The temperatures used were 20 °C, 25 °C, 30 °C and 35 °C, and the salinities were 25°/oo, 30°/oo, 35°/oo and 40°/oo. Response surface contour diagrams were generated from the survival and growth data to estimate optimal conditions. Normal development of embryos occurred only from 25 °C to 30 °C. The optimal conditions for maximum survival and growth were 26–29 °C and 28–32°/oo. Temperatures of 35 °C or greater were lethal for larvae and, at all temperatures tested, larval growth and survival were lowest at a salinity of 40°/oo.

    Notes: Abstract A retrospective study of 50 patients with Guillain-Barré syndrome (GBS) correlated analysis of serial motor nerve conduction studies with the presence of antibodies toCampylobacter jejuni, GM 1 and GDlb, determined by ELISA. GBS patients with antibodies toC. jejuni (n = 8), GM1 (n = 4), or GD1b (n = 4) showed electrophysiological features suggestive of demyelination with prolonged distal motor latencies and temporal dispersion/conduction block similar to GBS patients without these specific antibodies. Three of 50 GBS patients had poor recovery with inability to walk at 1 year after onset of symptoms. All three patients had antibodies to C. jejuni, but not to GM 1 or GD 1 b. Although later on in the clinical course distal motor responses were absent in two of these patients, reflecting extensive axonal degeneration, early nerve conduction studies showed findings suggestive of demyelination. We suggest that demyelination of peripheral nerve may be the initial disease mechanism in GBS independent of the presence of antibodies toC. jejuni, GM1 or GDlb.

    Description / Table of Contents: Summary Recently, Zenner et al. implanted the first totally implantable electronic hearing devices in patients with SNHL (HNO 46 [1998] 844–852). In the present report, technical and audiological features of the implant TICA are published. The development of the piezoelectric transducer and the microphone for implantation in the posterior wall of the auditory canal as components for the present fully implantable hearing system has already been described (HNO 45, 1997, 792–880). Here we report about our experience with the electronic main module that completes the TICA LZ 3001 system. This module is suited for implantation in the mastoid bone and contains the signal-processing electronics and an integrated battery that can be recharged transcutaneously with a portable charger. The recharging time is around 2 h for an implant operating time of 50h. The microphone and transducer connectors allow for easy replacement of the main module when the battery lifetime is reached. This lifetime is around 3–5 years. A small wireless remote control allows volume adjustment, contains an on/off switch, and permits selection of four different individual hearing programs. The basic audiological features are provided by a flexible, digitally programmable 3-channel-AGC-system with a peak clipping function. The total bandwidth is around 10 kHz. To our knowledge this is the first fully implantable hearing system that has been in implanted in humans.
    Notes: Zusammenfassung Kürzlich wurde über die Entwicklung eines elektromechanischen, piezoelektrischen Wandlers und eines Mikrofons zum subkutanen Einbau in die hintere Gehörgangswand als Komponenten eines zukünftigen vollständig implantierbaren Hörsystems für Innenohrschwerhörige berichtet (HNO 45, 1997, 792–880). Zwischenzeitlich konnte die Entwicklung eines elektronischen Hauptmoduls zur Implantation auf dem Planum mastoideum abgeschlossen werden, das diese Mikrofone und Wandler zu dem kompletten Hörimplantat TICA® LZ 3001 ergänzt. Dieses Hauptmodul enthält neben der signalverarbeitenden Elektronik eine spezielle Batterie, die transkutan mit einem portablen Ladegerät nachgeladen wird. Nach einer Volladung, die ca. 2 h benötigt, ist das Implantat für rund 50 h kontinuierlich betriebsbereit. Das Ladegerät wird ähnlich wie bei Mobiltelefonen in einer netzbetriebenen Station nachgeladen. Lösbare Steckverbindungen zu Mikrofon und Wandler ermöglichen den einfachen operativen Austausch des Hauptmoduls, wenn die Batterielebensdauer erreicht ist. Dies wird nach ca. 3–5 Jahren erwartet. Dem Patienten steht eine kleine, drahtlose Fernbedienung zur Verfügung, mit der Lautstärke, Ein/Aus sowie 4 Hörprogramme für unterschiedliche Hörsituationen eingestellt werden können. Die grundlegenden audiologischen Eigenschaften sind durch ein flexibel digital programmierbares 3-Kanal-AGC-System mit Peak-clipping-Funktion gegeben. Die gesamte Übertragungsbandbreite beträgt ca. 10 kHz. Das vollimplantierbare Hörsystem wurde im Rahmen der klinischen Erprobung Anfang Juni 1998 erstmals am Menschen angewendet.

This is caused by the sterical hindrance of the keto group of the anthracene in position 9; thus, the cyclocondensation proceedsvia reaction of the ester group of the β-oxo-propionate. In the same way, 9-acetylanthracene reacts with phenylisothiocyanate and α-CH-acidic compounds to keten-S,N-acetals10 and, in an additional step, to 2-anilino-3-(9-anthracenoyl)-thiophenes11 and 2-(9-anthracenoyl)-methylene-3,4-diphenyl-2,3-dihydro-(1,3)-thiazole12, respectively. The structure of all new compounds was determinated by 2D NOESY NMR spectroscopy.
