    Notes: Objective To compare the use of outpatient and inpatient procedures in the investigation of abnormal uterine bleeding.Design A randomised controlled trial.Setting Two university teaching hospitals.Participants Four hundred women with abnormal uterine bleeding (postmenopausal bleeding, menorrhagia, intermenstrual bleeding, postcoital bleeding, or irregular periods) above the age of 35 years, between June 1993 and January 1995.Main outcome measures 1. Incidence of detection of abnormal pathology by vaginal ultrasound, outpatient hysteroscopy and endometrial biopsy compared with inpatient hysteroscopy and curettage; 2. Number of ‘lesions’ (e.g. fibroids, polyps, endometrial hyperplasia or malignancy) found by hysteroscopy that would have been missed by the combination of endometrial sampling and ultrasound; 3. Comparison of the quality of tissue obtained for histology by outpatient endometrial sampling and inpatient curettage; and 4. An evaluation of patient acceptability of outpatient and inpatient procedures.Results 1. A combination of transvaginal sonography, Pipelle endometrial biopsy and outpatient hysteroscopy has similar efficacy to inpatient hysteroscopy and curettage for the investigation of abnormal uterine bleeding; 2. Hysteroscopy will detect some fibroids and polyps missed by a combination of transvaginal ultrasound and Pipelle endometrial sampling; 3. The quality of histological samples obtained by outpatient Pipelle were comparable to those obtained by formal inpatient curettage; and 4. Outpatient procedures were well tolerated, with good patient acceptability.Conclusion Transvaginal sonography and endometrial biopsy can safely be used as the initial investigations in the management of abnormal uterine bleeding. Hysteroscopy can be used as a second line investigation. Outpatient hysteroscopy with local anaesthesia is well tolerated although general anesthesia may occasionally be necessary.

    Notes: The spin–orbit energy contributions to the ground state potential energy curves for the main group hydrides, TIH through AtH are estimated by differencing multireference, single promotion, configuration interaction (MRS-CI) energies with and without the spin–orbit operator. The spin–orbit contributions are then summed into the energies determined at the λ−s MRSD-CI level (both single and double promotions). The agreement between the resultant curves and those obtained using intermediate coupling MRSD-CI is within 1.2 kcal/mol over a range of internuclear separations. This suggests that, contrary to previous arguments, spin–orbit coupling and correlation energies are very nearly separable for the main group hydride ground states. Furthermore, the computational effort expended by this separate evaluation is up to 12 times less than that for a comparable intermediate coupling CI. The analysis of some properties of these hydrides indicates that bond length shifts due to spin–orbit coupling are small (0.03 Å) while harmonic vibrational frequencies decrease by up to 9%. Dissociation energies are predicted to change considerably in the presence of the operator in agreement with previous findings. © 1998 American Institute of Physics.

    Notes: Cardiomotor and vasomotor responses were assessed during isoproterenol tilt-induced vasovagal reaction in patients with a history of syncope. In a case controlled study, all patients and controls were subjected to a standard protocol: baseline supine (10 min), baseline tilt (70±, 45 min), isoproterenol supine (0.05 μg/kg per min, 10 min), and isoproterenol tilt (70±, 10 min). The participants were 11 consecutive patients referred for syncope evaluation (5 men, 6 women; mean age, 34.1 ± 10.4 years; range, 18–56 years) and 11 age and sex matched controls (5 men, 6 women; mean age, 35.5 ± 12.2 years; range, 19–63 years). On-line, beat-to-beat measurements of cardiomotor functions (heart rate, stroke volume, and cardiac output) and vasomotor functions (systolic, mean, and diastolic blood pressures and total peripheral resistance [TPR]) were detected noninvasively by volume clamp photoplethysmography and impedance cardiography. Patients and controls had similar cardiomotor and vasomotor responses during passive tilt and during isoproterenol infusion in the supine position. Immediately after tilt during isoproterenol infusion and before the onset of symptoms, decreases in vasomotor functions were significant in study patients when compared with those in controls; whereas responses in cardiomotor functions were similar between the two groups. When compared with baseline supine findings, TPR decreased by 56.5%± 10.9% and 29.5%± 23.3% in the patient population and controls, respectively (P = 0.005). When compared with isoproterenol supine findings, TPR decreased by 27.5%± 22.8% in the study patients and increased by 22.6%± 48.1% in the controls (P = 0.005). The inability to overcome isoproterenol-induced vasodilatation during orthostatic stress played an important role in the initiation of a vasovagal response. These observations hold the key to early detection of hemodynamic changes and potential therapeutic interventions before patients become symptomatic.

    Notes: Freshwater planarians possess remarkable regenerative abilities that make them one of the classic model organisms for the study of regeneration. These free-living members of the phylum Platyhelminthes are representatives of the simplest triploblastic organisms possessing bilateral symmetry and cephalization. Furthermore, planarians occupy an important position in the evolution of Metazoa, which allows for the possibility of vertically integrating molecular studies of regeneration in this organism to other, more widely studied animal model systems. Because of their relative simplicity, developmental plasticity, and evolutionary position, planarians are an attractive system to dissect the molecular processes underlying regeneration. The objective of this article is to present a molecular strategy to identify and functionally manipulate genes involved in the process of blastema-derived regeneration. Ultimately, the genes identified in planarians and their interactions during regeneration will define a series of useful molecular templates that may help unravel the more complex epigenetic processes of vertebrate regeneration and may perhaps uncover the factors that make regeneration permissive in some, but not all, metazoans.

    Notes: Abstract The lack (or limited amount) of audio feedback for many persons with hearing disabilities greatly reduces the quality of speech development and use. Current methodologies for the teaching speech techniques are relatively primitive, and generally require the active participation of a therapist. This makes essential practice difficult, time consuming, and expensive for the student. However, using multimedia technology, it is possible to amplify the efforts of the teachers and students for improving the quality of speech. This paper presents a multimedia visualization system that augments the limited hearing capabilities of hearing impaired persons using visual and tactile feedback approaches and provides alternate representations of sound for increasing speech capabilities and reduced learning time. The proposed system has the capability of not only decreasing the learning time for many persons, but can also be adopted for teaching young children correct speech patterns through the use of speech learning “games” based on the system.

Beetles captured on sticky traps baited with single and multicomponent lures were no different from the control traps for two species ofAulacophora. Yellow colored traps and squash blossoms are attractive toAulacophora beetles which detect sub-microgram quantities of cucurbitacins on silica gel. Leaf feeding behavior and flight activity data are correlated with varietal preference of threeAulacophora species. The common response byDiabrotica andAulacophora to cucurbitacins reinforces the two groups' coevolutionary association with the Cucurbitaceae. The apparent lack of a common response toCucurbita blossom volatiles suggests recent evolutionary pathways are substantially different for these two groups of beetles.
