    Description / Table of Contents: Résumé Le traitement local de tumeurs bénignes et de cancers du rectum soignement sélectionnés a été largement publié et reconnu. Trente-trois patients porteurs de lésions bénignes ou malignes de la partie distale du rectum ont été soumis à une évaluation prospective après résection locale selon la technique d'électro-transition transanale décrite par Faivre. Les patients ont été divisés en trois groupes: 1) excision locale pour une lésion bénigne (12 patients), 2) résection curative pour une tumeur maligne localisée (7 patients) et 3) excision locale palliative pour une tumeur maligne (14 patients). Les complications comportent une incontinence passagère et un saignement rectal. Les patients ont été suivis pendant une période moyenne de 18 mois (1 à 54 mois); quatre récidives (28%) se sont développées chez les patients du groupe 3). Les autres patients avec des lésions malignes n'ont pas développé de récidive tumorale durant la période de suivi. Ce traitement est idéal pour des lésons bénignes et d'autres études avec un suivi plus long sont nécessaires pour déterminer la place de ce traitement en cas de lésion maligne.
    Notes: Abstract Local therapy has been widely reported and accepted for benign tumours and carefully selected cancers of the rectum. Thirty-three patients with benign or malignant low rectal tumors have undergone a prospective evaluation after local resection with transanal electroresection as described by Faivre. Patients were divided in three groups: 1) local excision of benign tumours (12 patients), 2) curative resection for localised malignant tumours (7 patients) and 3) palliative local resection for malignant tumours (14 patients). The complications included transient incontinence and rectal bleeding. Patients have been followed for a mean period of 18 months (1–54). In group 3 there were four (28%) recurrences. The remainder of the patients with malignant tumours have been free of disease on follow-up. Although it is ideal for benign neoplasms, further follow-up is needed to determine its true indication in malignant lesions.

    Notes: Abstract  Behavioral and morphogenetic responses to conjugative interactions of Aspidisca sedigita and A. steini were studied using transmission and scanning electron microscopy. The following changes related to the phenomenon of conjugation have been evidenced. Most of the potential mates show a lifting of the peristomial lip before sticking together in pairs. At the beginning of pairing, mates face each other fusing their lifted peristomial lips. The fusion involves only the mates’ plasma membranes, whereas cortical alveoli are disrupted and unconnected. The left member of the pair (B mate) soon turns through about 180°. Eventually the right ventral side of the rotating B mate overlaps the left dorsal side of the non-rotating one (A mate). Because of this rotation the fusion zone stretches and becomes like a ”tube” rather than being a simple ”door” connecting the mates. In addition to the behavioral differences, the contribution to the ”tube” formation is also different between the two partners of the mating. The B mate supplies the majority of both pellicle and cytoplasm. The dorsal surface of the A mate is partially involved. The morphogenetic pattern during the mating reaction in Aspidisca species has been analyzed and compared with that of members of related genera. It can be related to that of Euplotes species, although differences in the relative timing of the events have been observed.

    Description / Table of Contents: Sommario I pazienti affetti da sclerosi multipla (SM) frequentemente presentano turbe della minzione. Al fin di valutare la sensibilità dei potenziali evocati somatosensoriali del nervo pudendo (pPES) nell'identificazione di una compromissione delle vie centrali di controllo della minzione, abbiamo registrato i pPES in 16 pazienti affetti da SM probabile. In tutti i pazienti è stato eseguito inoltre uno studio urodinamico e sono stati registrati i potenziali evocati visivo, uditivo e somatosensoriale dei nervi mediano e tibiale. Anomalie del PES del nervo pudendo sono state riscontrate nei 6 pazienti con disturbi della minzione e in 8 dei 10 pazienti asintomatici (87,5%). In 12 di essi (75% dei casi) sono state riscontrate anomalie del profilo urodinamico suscettibili di trattamento. Le nostre osservazioni sembrano deporre per un frequente e precoce coinvolgimento delle vie di controllo della minzione nei pazienti affetti da SM e per un'elevata frequenza di anomalie subcliniche.
    Notes: Abstract Bladder dysfunctions are often observed in patients with multiple sclerosis (MS). In order to evaluate their sensitivity in detecting abnormalities in bladder central control pathways, pudendal nerve somatosensory evoked potentials (pSEPs) were recorded in 16 patients with clinically probable MS: six were affected by retention or urge incontinence, and ten were asymptomatic. Conventional visual, auditory and somatosensory evoked potentials were also recorded, and all of the patients underwent a urodynamic examination. Prolonged latency or the absence of pSEP cortical responses was found in eight of the ten asymptomatic patients, and in all of the symptomatic cases (87.5%). The urodynamic evaluation revealed abnormalities in 12 patients (75%). Our findings seem to indicate an early and frequent involvement of bladder control pathways in MS patients, as well as a high rate of subclinical disorders.

    Notes: Summary Epixenosomes live on the dorsal surface of their ciliate host,Euplotidium itoi. They lack a nuclear envelope and divide like prokaryotes. On the other hand they have a morphological and functional cell compartmentalization and possess tubules that are sensible to tubulin inhibitors and positively react with different antitubulin antibodies. In the present paper, as a first step to investigate their real nature, the in situ hybridization technique was applied at the ultrastructural level. Different prokaryotic and eukaryotic probes suitable for detecting rRNA genes were used. An additional test was performed with the gene encoding for β tubulin in the ciliateEuplotes crassus. Positive results, evidenced by a precise localization of gold particles, were obtained with all the eukaryotic probes used. These probes were obtained from organisms belonging to three different kingdoms (Protista, Animalia, Plantae). On the contrary, no hybridization was obtained with prokaryotic probes, not even when the probe used was an oligonucleotide complementary to all bacterial 16S rRNA so far sequenced. On the basis of these results and of the other observations so far accumulated, the possible eukaryotic nature of epixenosomes is discussed.

    Notes: Summary The extrusive apparatus is the most prominent and complex structure of epixenosomes. In the present paper the mechanisms activating its ejecting process were investigated by means of in vivo treatments and cytochemical procedures at the ultrastructural level. The results obtained clearly demonstrated that the ejecting process in epixenosomes is triggered by the detection of external signals through membrane receptors and the consequent activation of the adenylate cyclase-cyclic AMP system as a transduction mechanism. The membrane receptors coming into play have an affinity for soybean agglutinin and have a precise localization at the top of the organism, just where a membrane interruption appears as a first step in the whole process. The factors that trigger ejection in nature are still unknown. In the laboratory, ejection was obtained in the presence of adrenalin, which has been proved to bind to the same receptors shown to have affinity for soybean agglutinin. So epixenosomes appear to possess specific binding molecules for a mammalian hormone in the appropriate location, i.e., in the plasma membrane, and this hormone induces a precise biological response. These results are particularly interesting if we consider that epixenosomes are enigmatic organisms in which prokaryotic and eukaryotic characteristics appear to coexist.

    Description / Table of Contents: Sommario I tentativi di rinnovamento dei sistemi sanitari vigenti nei vari paesi sviluppati hanno lo scopo di contenere l'enorme crescita della spesa sanitaria. Le politiche rivolte al mero contenimento della spesa rischiano tuttavia di influenzare negativamente la qualità dell'assistenza. Per rinnovare i sistemi sanitari non basta porsi l'obiettivo di una maggiore efficienza dei servizi, ma occorre stabilire se le prestazioni erogate sono valide in termini di rapporto costi-efficacia e se le prestazioni stesse sono erogate in modo tempestivo e qualitativamente adeguato. Per realizzare l'obiettivo di migliorare la qualità dell'assistenza riducendo gli sprechi, è necessario investire sulla ricerca biomedica promuovendo studi di outcome in grado di fornire linee guida affidabili e valutare in modo adeguato i benefici connessi all'innovazione tecnologica, eliminando i fattori che favoriscono un uso distorto dell'innovazione stessa. Occorre altresì attivare forme organizzative che prevedano un maggior input dei medici e degli altri addetti all'assistenza nel processo decisionale ai vari livelli.
    Notes: Abstract The Author discusses some aspects of the Health Care systems in developed countries, and analyzes the reasons for the failure of recent reform programs to provide good quality service at a reasonable price. While not condemning business-style management completely, the Author points out how, in the neurological field too, under the control of physicians, biomedical research and innovation with inter-departmental collaboration can be managed according to specific aims, so as to prevent much costly and wasteful diagnostic and therapeutic practice, thus improving service quality while reducing hospital running costs.
