    Suprasellar germinomas; Relationship between tumour size and diabetes insipidus (1992)

    Notes: Summary The clinical and neuro-endocrinological aspects of 17 suprasellar germinoma patients treated between 1972–1991 are reported. Surgical extirpation was not initially attempted, but all patients received irradiation with or without a biopsy. Sixteen of those have led useful lives with appropriate hormonal replacement therapy during a mean follow up of 8 years. Seven tumours at diagnosis were less than 2 cm in diameter (type 1), nine tumours more than 2 cm (type 2), and one double midline tumour was of unknown size. All 7 type 1 patients required 1-deamino-8-D-arginine-vasopressin (DDAVP) to control diabetes insipidus (DI), but only 2 of the 9 type 2 patients have needed DDAVP since completion of the treatment. Patients with smaller tumours required more DDAVP following tumour disappearance, than those with larger tumours. The prognostic indicators for the post-treatment course of DI and retarded growth appeared to be the tumour size and the age at diagnosis. We also emphasize the absence of metastasis in unoperated cases and the use of tumour markers as a diagnostic criterion which obviates surgical acquisition of tissue to make the diagnosis. Possible reasons are discussed and the literature reviewed.

    Notes: Abstract Carbon dioxide (CO2) intraarterial digital subtraction angiography (IADSA) provides retrograde visualization of the portal vein via a peripheral segmental hepatic artery. IADSA was performed in 12 patients with known hepatic diseases by injecting a peripheral hepatic artery with both CO2 gas and an iodinated contrast medium. The portal vein was constantly visualized only with CO2 IADSA in all patients. The injected CO2 may flow back into the portal vein through an anastomotic system known as the peribiliary or periportal plexus. This new method is safe and useful to image the portal venous system in patients with hepatic malignancy.

    Notes: To investigate the relationships between allergen inhalation challenge and other diagnostic procedures, inhalation challenge with house dust (HD) allergen, intradermal skin tests with HD allergen, inhalation challenge with methacholine and circulating HD allergen-specific IgE levels were examined in 104 patients with bronchial asthma. Using the single exposure method, allergen inhalation challenge was performed. Forty-three patients had positive bronchial responses to allergen and 61 patients had negative bronchial responses. With serially diluted HD allergen (10-3 to 10-6, w/v), skin-test sensitivity was expressed as the highest dilution required to produce a weal of more than 9.9 mm. With the continuous exposure method, bronchial responsiveness to methacholine was evaluated as the number of units of inhaled methacholine (PD35-Grs) from the start to the point at which Grs had decreased by 35% from its baseline value. The level of circulating HD allergen-specific IgE was measured with the Phadebas. RAST system and the results were assessed as a RAST score. Using discriminate analysis, in which the independent variables were skin-test sensitivity. PD35-Grs and the RAST score, only in 30% of all patients was bronchial responsiveness to inhaled HD allergen predictable. Therefore, we suggest that inhalation challenge with allergen is an essential test for determining the role of a specific allergen in airways at present.

    Notes: Erasing magneto-optical disks were studied using constant frequency pulses. The magnetic layer was Gd12Tb12Fe76. The linear velocity was 7.2 m/s and writing and rewriting were done at 1.8 MHz. The magnetic field was the same during writing and erasing. If the erasing power is too low, bits are incompletely erased, and if the erasing power is too high, erase pulses nucleate domains. Erasing performance can be improved by high frequencies and low duty cycles. Erasing performance decreases as the magnetic field in the write direction becomes stronger. However, almost the same rewriting C/N as the original C/N can be obtained in a weak magnetic field. We obtained a rewriting C/N of 29 dB, while an original C/N was 31 dB at a magnetic field of +50 Oe.

    Notes: Abstract The aim of this investigation was to gain further insight into the prevalence of different serotypes of mutans streptococci in the Italian population by using specific monoclonal antibodies in an enzyme immunoassay. Isolates from dental plaque samples, collected from an adult population living in Pisa (Italy), were identified as mutans streptococci on the basis of their morphological and biochemical properties, and were then serotyped. The results show that 77.5% of the strains isolated belonged to serotype c or f (i.e., S. mutans), 15.9% were serotype e (i.e., S. mutans) and only two strains (1.4%) belonged to serotype g (i.e., S. sobrinus). These data are partially in agreement with other studies in Europe and in the U.S.A. The distribution pattern of the various serotypes turned out to be substantially similar among the different groups of patients, subdivided on the basis of their caries status, indicating that none of the serotypes was particularly associated with dental caries.

    Notes: Summary The effect of (aminooxy)acetate, an inhibitor of aminotransferases, on the sulfate formation froml-cysteine andl-cysteinesulfinate in rat liver mitochondria was studied. Incubation of 10 mMl-cysteine with rat liver mitochondria at 37°C in the presence of 10 mM 2-oxoglutarate and 10 mM glutathione resulted in the formation of 4.60 and 1.52µmol of sulfate and thiosulfate, respectively, per 60 min per mitochondria obtained from 1 g of liver. Under the same conditions sulfate formation froml-cysteinesulfinate was 24.96µmol, but thiosulfate was not formed. The addition of (aminooxy)acetate at 2 mM or more completely inhibited the sulfate and thiosulfate formation froml-cysteine and the sulfate formation froml-cysteinesulfinate. These findings support our previous conclusion that cysteine transamination and 3-mercaptopyruvate pathway (MP pathway) are involved in the sulfate formation froml-cysteine in rat liver mitochondria (Ubuka et al., 1992).

    Management problems in continuous care of male patients with mild hypertension with special reference to alcohol-intake (1993)

    Notes: Abstract Forty two male patients with borderline and mild hypertension were followed up for 3.9 years without medication. During this period, all subjects had nonpharmacological interventions including salt restriction, weight reduction, anti-smoking or anti-alcohol instruction, and physical exercise after the established protocol. Comparisons of biomedical data between pre- and post-observation revealed no significant improvements. Although the smoking habit decreased during this period, the drinking habit rarely changed and even significant increase of the total amount of alcohol ingestion was observed. These data indicate great limitation of nonpharmacological intervention in the care of these asymptomatic hypertensive patients for a lifelong base.
