    Notes: Summary Seven cases of senile dementia of Alzheimer type (SDAT) with unusual clinico-pathological findings are reported. The patients showed neuronal loss in laminar pattern, with gliosis exclusively confined to the CA1 of the hippocampus, the area of the hippocampal gyrus (entorhinal cortex) and medial occipitotemporal cortex. This change was more pronounced in the oral region. The subcortical white matter showed more pronounced fibrillary gliosis than loss of myelin. Both Alzheimer's neurofibrillary tangles and senile plaques were less marked than those usually seen in SDAT. The mental disturbance started after the age of 65 in all patients. The main clinical feature was marked character change in addition to disturbance of cognitive function. Cranial computed tomography showed marked dilatation of the oral portion of the inferior horn of the lateral ventricle in the early stage. It was apparent that although the cases in this group could be incorporated within in the spectrum of SDAT, they could also be considered to represent a variant of SDAT. This group could contribute to an understanding of the clinico-pathological spectrum of SDAT as well as indicating ways of managing such patients.

    Notes: Summary The ultrastructure of the curly fibers was examined by the transmission and immunoelectron microscopy as well as by the rapid-freeze, deep-etch and replica method. The curly fibers consisted mainly of paired helical filaments (PHF) in the neuropils, both pre- and post-synaptic. On the deep-etch replicas, PHF in the neurites had similar dimensions to those of Alzheimer's neurofibrillary tangles in the nerve cell somata, having the width of 28 to 36 nm. The two component filaments, 14 to 18 nm in width, were twisted left-handedly with about 70- to 90-nm intervals. There were also cross-bridging fibrils of about 6 nm in diameter connecting the neighboring PHF.

    Notes: Abstract The neuropathological findings, including immunohistochemistry and electron microscopy, of two patients with clinical findings consistent with corticobasal degeneration (CBD) are reported. Both patients showed degeneration of the precentral cortex, the substantia nigra, the pallidum, and the thalamus. Many ballooned neurons were seen in the cerebral cortex, and argentophilic, skein-like inclusions suggesting neurofibrillary tangles (NFTs) were found in the brain stem and precentral cortex in patient 1. In contrast, patient 2 clearly showed NFTs in the brain stem and dentate nucleus which were indistinguishable from those seen in progressive supranuclear palsy (PSP), while only a few ballooned neurons were found in the cerebral cortex. Gallyas silver stain showed many argentophilic inclusions suggesting NFTs in the brain stem, subcortical nuclei, and cerebral cortex in both patients. Immunohistochemistry for tau showed tau-positive neurons in the cerebral cortex, brain stem, subcortical nuclei and spinal cord, and tau-positive glial cells were seen in the cerebral cortex, white matter and subcortical nuclei, and thread-like structures were seen in the cerebral cortex and white matter. Electron microscopy of the brain stem showed NFTs consisting of paired helical filaments in patient 1, and paired helical filaments and straight tubules in patient 2. Immunoelectron microscopy revealed parallel tau-positive filaments in the cerebral cortex in patent 1. From the two patients, the widespread appearance of abnormal tau and NFTs is one of the essential pathological features in CBD, and it also appears that CBD and PSP have some common underlying pathological processes. Patient 2 is closer to PSP than patient 1 and suggests CBD would link to PSP.

    Notes: Abstract An autopsy case of an 83-year-old Japanese woman with pancreatic cancer with significantly elevated serum α-fetoprotein levels is reported. The pancreatic tumor was a mucinous cystadeno-carcinoma with multiple metastasis to the liver. The immunohistochemistry for α-fetoprotein revealed positive reactivity in the cytoplasm of cancer cells in the primary and liver metastatic lesions. The current case is the first reported in which mucinous cystadenocarcinoma of the pancreas produced α-fetoprotein.

    Notes: Summary Sixteen cases of malignant brain tumours comprising 6 anaplastic astrocytomas, 3 glioblastoma multiforme, 1 medulloblastoma and 6 metastatic brain tumours were investigated independently by a silver colloid method for nucleolar organizer regions (NORs) and an immunohistochemistry using a monoclonal antibody against a nuclear antigen, Ki-67, in proliferating cells. The correlation between the mean number of NORs and the percentage of Ki-67 labelled cells (Ki-67 labelling index) was examined. In addition, four normal brain tissue samples without neoplastic cells were stained for NOR. The mean number of NORs in these malignant brain tumours was significantly greater than that in normal astrocytes (p〈0.001). Moreover, both the mean number of NORs and the Ki-67 labelling index in metastatic brain tumours were significantly greater than those in high-grade gliomas (p〈0.001). The Ki-67 labelling index and the mean number of NORs in malignant brain tumours including metastatic brain tumours were found to be linearly related (r=0.86). These results suggest that the proliferative potential of malignant brain tumours could be evaluated by NOR score as well as Ki-67 labelling index and that such indices provide clear discrimination between high-grade gliomas and metastatic brain tumours.

    Notes: Summary We describe five patients with chronic inflammation of the hypophysis including three pituitary granulomas of unknown aetiology. In contrast to the previously reported cases, the involvement of neurohypophysis or hypothalamus was a distinct clinical feature in these patients. Impairment of anterior pituitary function was less prominent, while polyuria and polydipsia occurred in all cases. Enlargement of the sella turcica was absent in three and slight in two cases. CT scan and MR images demonstrated a contrast-enhanced sellar mass in all patients; abnormally thickened pituitary stalk and infundibulum with contrast-enhancement was observed in four. The fibrous tissues were removed by the transsphenoidal approach in four patients, and by the subfrontal approach in one case. In all patients, the endocrinological dysfunction was prolonged. No increase in the size of the remaining pituitary mass was demonstrated on repeated MR images in any of the patients. On histological examination, granulomatous formation was present in three samples, and multinucleated Langhans' giant cells were seen in one. The epithelioid cells and multinucleated giant cells constituting the granulomas were positive for anti-macrophage antibody. No firm laboratory or histological evidence was obtained supporting the presence of systemic disease leading to granulomas. In the other two cases, the pituitary lesions were composed of chronic inflammation tissue, and serum antipituitary antibodies were present in a patient with concurrent Hashimoto's thyroiditis. Our experiences with chronic inflammation of the hypophysis indicate that these patients are best managed by histological confirmation of the lesion followed by adequate hormonal replacement. Based on our findings and those reported in the literature, we propose that patients with granulomatous hypophysitis or chronic inflammation of the hypophysis be managed as follows: When an underlying disease is detected, the patient should begin to receive conservative treatment for the causative disease plus hormonal replacement therapy, as necessary. However, when visual disturbance is progressive and uncontrollable by conservative means, surgical decompression of the chiasma is required. If the pathogenesis of the pituitary lesion cannot be identified, surgical exploration is essential for a precise pathological diagnosis. When a granulomatous or chronic inflammatory process is evident intra-operatively, partial removal or biopsy are recommended. Radical resection of fibrous and adhesive tissue with infundibular impairment will lead not only to exacerbation of the pre-existing anterior- and/or posterior pituitary dysfunction, but also to grave hypothalamic injury.

    Notes: To study performance and reproducibility of photodiodes (PDs) on Si in the long-wavelength region, InAlAs/InGaAs metal-semiconductor-metal PDs were fabricated on high-quality heteroepitaxial InP layers on Si substrates. A dark current of 0.5–2×10−8 A and a responsivity of 0.05–0.15 A/W were reproducibly obtained, at least at one of the ±5 V bias voltages. These dark currents and responsivity are similar to those of PDs with the same structure fabricated on InP substrates. The PDs on Si have pulse responses with full widths at half-maximum of 150–600 ps.

    Notes: The anti-proliferative effects of interferon gamma (IFN-γ) and CPT-11, a new derivative of camptothecin, both singly and in combination, on two human endometrial carcinoma cell lines, HHUA and ISHIKAWA, were examined in vitro. The HHUA cells were responsive dose-dependently to IFN-γ, while the ISHIKAWA cells were unresponsive to IFN-γ. Both cell lines were responsive dose-dependently to CPT-11. In both cell lines, IFN-γ at clinically achievable concentrations (10 and 100 U ml−1 enhanced the anti-proliferative activity of CPT-11 in the range of concentrations where CPT-11 showed more than 10% cell growth inhibition. Sequential treatment with CPT-11 followed by incubation with IFN-γ resulted in significant cell growth inhibition, but not vice versa. Flow cytometric studies indicated that the combined anti-proliferative effect did not correlate with cytokinetic alterations. Treatment with IFN-γ did not change the extractable topoisomerase I activity of the HHUA and ISHIKAWA cells. The combination therapy of IFN-γ and CPT-11 could provide a new approach against endometrial cancer.
