    Notes: Summary The aim of this double-blind, placebo-controlled, cross-over study was to investigate the effect of trospium chloride on gall bladder contraction, gastric emptying of a liquid meal, gastrooesophageal reflux, and orocaecal transit time in healthy subjects. Gall bladder contraction was examined by ultrasonography before and after stimulation with two raw eggs. Gastric emptying was evaluated by an intubation technique and by sonography. To determine gastrooesophageal reflux and orocaecal transit time, 24-hour pH metry and a hydrogen breath test were used. The gall bladder ejection fractions were significantly lower after oral treatment with both 4×10 mg and 4×20 mg trospium compared to placebo, but no difference was seen between the two doses of drug. Gastric emptying of a liquid meal was significantly delayed after intake of 4×15 mg trospium, whilst the time course of the intragastric volume determined by ultrasound did not differ from that after placebo, suggesting an antisecretory effect of trospium on gastric secretion. The fractional time of oesophageal pH〈4 as a percentage of the entire 24-hour investigation period was significantly increased by treatment with trospium 3×15 mg per day. The orocaecal transit time of 10 g lactulose was significantly prolonged. Provided that the observed effects on gall bladder contraction, gastric emptying, and orocaecal transit time are reproduced in disease states, trospium should be regarded as a potentially useful antispasmodic agent.

    Notes: Background: Coffee and tea are believed to cause gastrooesophageal reflux : however, the effects of these beverages and of their major component, caffeine, have not been quantified. The aim of this study was to evaluate gastro-oesophageal reflux induced by coffee and tea before and after a decaffeination process, and to compare it with water and water-containing caffeine. Methods: Three-hour ambulatory pH-metry was performed on 16 healthy volunteers, who received 300 ml of (i) regular coffee, decaffeinated coffee or tap water (n = 16), (ii) normal tea, decaffeinated tea, tap water, or coffee adapted to normal tea in caffeine concentration (n= 6), and (iii) caffeine-free and caffeine-containing water (n= 8) together with a standardized breakfast. Results: Regular coffee induced a significant (P 〈 0.05) gastro-oesophageal reflux compared with tap water and normal tea, which were not different from each other. Decaffeination of coffee significantly (P 〈 0.05) diminished gastro-oesophageal reflux, whereas decaffeination of tea or addition of caffeine to water had no effect. Coffee adapted to normal tea in caffeine concentration significantly (P 〈 0.05) increased gastro-oesophageal reflux. Conclusions: Coffee, in contrast to tea, increases gastrooesophageal reflux, an effect that is less pronounced after decaffeination. Caffeine does not seem to be responsible for gastro-oesophageal reflux which must be attributed to other components of coffee.

    Notes: Abstract A measurement technique to determine the surface heat transfer distribution in complex turbulent flows is described. For this purpose, a constant wall heat flux test surface has been designed. To measure the surface temperature of the test plate, an infrared camera was used. The instrumentation allows the determination of the heat transfer with high accuracy and detailed spatial resolution. In examining combustor-type separated flow, the capabilities of the technique are demonstrated and its accuracy is verified by appropriate conventional techniques.

    Notes: Abstract Eighteen patients were examined at rest by technetium 99m methoxyisobutylisonitrile (99mTc-MIBI) myocardial scintigraphy 1 day before and 1 week after aorto-coronary bypass operation with planar and single photon emission tomography (SPET) imaging. One day postoperatively, a planar scintigraph in the intensive care unit (ICU) was done. Inter-observer variability was 3.8% for all examinations and for SPET alone, 3.9%. The quality of the planar images taken under emergency conditions in the ICU was quite comparable with those taken under routine conditions. The postoperative myocardial infarction in a patient who died 6 days later could clearly be demonstrated. In 16.2% of all segments which were hypoperfused at rest on preoperative scintigraphy, an amelioration of perfusion could be shown in the 1 st week after the bypass operation. 99Tc-MIBI proved to be a useful agent to assess perioperative perfusion, in the ICU as well as under standard conditions.

    Notes: Summary Acute ingestion of pure ethanol has been reported to delay gastric emptying and to enhance the propulsive movements of the intestine. The aim of the present study was to investigate the comparative effect of beer (7.0% v/v), white wine (7.5% v/v), ethanol (7.5% v/v), and water on the gastric emptying of a liquid test meal and on the gastrocaecal transit time of lactulose added to the test meal. Gastric liquid emptying was assessed by means of a nasogastric intubation technique using polyethylene glycol 4000 as the non-absorbable marker. The gastrocaecal transit time was evaluated by a hydrogen breath test. Beer (P〈0.001) and white wine (P〈0.05) significantly accelerated gastric emptying in comparison with ethanol of the same concentration. The gastrocaecal transit time was significantly shorter when the liquid meal was administered with beer compared with ethanol (P〈0.005) and water (P〈0.01). The constituents in beer and white wine responsible for our observations remain to be found.

    Notes: Abstract We have measured the inclusivep T spectra of soft photons produced at central and backward rapidities in 450 GeV/cp-Be collisions down to 1 MeV/c in transverse momentum. In the region 1〈p T 〈20 MeV/c an excess of photons over those expected from hadronic decays is observed. This excess is comparable, within systematic errors, with estimates of direct photons produced via hadronic bremsstrahlung. An upper limit is derived on the presence of additional sources of direct photons at small transverse momentum.

    Notes: Abstract Five subtypes of muscarinic receptors have been distinguished by pharmacological and molecular biological methods. This report characterizes the muscarinic subtype present in human gastric mucosa by radioligand binding studies. The receptor density was 27 ±6 fmollmg protein and the tritiated ligand N-methylscopolamine had an affinity of (KD)0.39±0.08 nM (n=11). The M1 receptor selective antagonist pirenzepine and the M2 receptor selective ligand AF-DX 116 had low affinities of 148±32 nM (n=13) and 4043±1011 nM (n=3) KD,respectively. The glandular M3 antagonists hexahydrosiladifenidol and silahexocyclium had high affinities of KD 78±23 nM (n=5) and 5.6±1.8 nM (n=3). The agonist carbachol interacted with a single low-affinity site and binding was insensitive to modulation by guanine nucleotides. Antagonist and agonist binding studies thus showed an affinity profile typical of M3 receptors of the glandular type.
