    Notes: We placed patients with invasive epithelial ovarian cancer into four distinct prognostic groups: ‘low’, ‘moderate’, ‘high’ and ‘extreme’ risk. The ‘moderate-risk’ group contained all residual negative, stage I and II patients with two exceptions: stage Ia or b, grade 1 cancers and grade 3 cancers. They were treated with primary surgery, usually including bilateral salpingo-oophorectomy, hysterectomy and omentectomy. Chemotherapy was then given (cisplatin at 100 mg m−2 every 2 weeks for three cycles) followed by pelvi-abdominal irradiation (2250 cGy in 10 fractions to the pelvis and 2250 cGy in 22 fractions to the whole abdomen including pelvis). An early cohort with ascites or positive washings instead received six cycles of cisplatin and cyclophosphamide at 75 mg m−2 and 600 mg m−2 every 4 weeks with the same pelvi-abdominal irradiation sandwiched between cycles 3 and 4. One-hundred and nine patients were treated between November 1983 and December 1989. Median follow-up was 4.7 years (range 0.7–9 years). The 5-year actuarial overall and failure-free survivals were 81% and 76%, respectively. Chronic toxicity, although usually minor, included 15% with peripheral neuropathy or ototoxicity and 23% with chronic abdominal complaints. Our combined-modality results are similar to those obtained by other centers utilizing either pelvi-abdominal irradiation alone or cisplatin-based chemotherapy alone.

    Notes: This investigation has employed the NIH 3T3 cell transfection assay in an effort to detect transforming genes in DNA from squamous carcinomas of the head and neck. Of 11 tumor DNAs tested, 1 DNA sample from a gingival squamous carcinoma was able to produce primary and secondary transformants containing the human K-ras oncogene. This is the first report of an activated ras oncogene derived from a carcinoma of the head and neck. Head and neck cancers may possess activated ras oncogenes more often than is indicated by this study because of the relative inefficiency of transfection assays in detecting large transforming genes such as K-ras.

    Notes: Abstract Enamel does not remodel, and disturbances occurring during development may remain in the tooth as a permanent record of the upset. Mineralization in prenatal and postnatal deciduous enamel was studied in the shed deciduous incisors of low-birth-weight (LBW: 〈 2kg) children. The specific objective was to gain further insight into the mechanism of formation of developmental defects of enamel. Sections at a resolution of 22–40 μm were reconstructed using X-ray microtomography (μCT) giving absolute measurements of linear absorption coefficient for AgKα radiation. Detail to ca. 1 μm resolution was obtained using automated, digital backscattered electron (BSE) imaging of PMMA-embedded material. Matching the histograms of BSE and μCT images made possible the calibration of the mean atomic number-dependent signal in the BSE images. The comparison of abnormal, affected enamel regions and post-recovery, normal, unaffected regions could be made in the same teeth, since these zones were easily recognized from the distribution of hypoplasia and hypomineralization. The μCT values, converted to calculated mineral densities, ranged from 2.3 g cm-3 to 2.6 g cm-3 in LBW hypoplastic, and between 2.65 and 2.78 g cm-3 in control primary enamel and post-defect, post-natal LBW enamel. Hypoplasia with or without minimal hypomineralization indicated recovery of the ameloblasts in the maturation phase. Disturbance during late matrix formation and early maturation resulted in hypoplasia and hypomineralization.

    Notes: Abstract Type I diabetes is strongly associated with the major histocompatibility complex (MHC) class II region (DR and DQ loci), and to a lesser extent the class III region (complement C4 loci). Restriction fragment length polymorphism analysis was employed to investigate the C4 and heat shock protein 70 (HSP70) loci of 176 patients with type I diabetes and 92 healthy controls. In the patient population there was an excess of deletions of the C4A locus (48.5% vs 22.1%, P 〈 0.0005). The HSP70 probe in conjunction with the restriction endonuclease Pst I detects two alleles of 9 or 8.5 kilobases (kb). The 8.5 kb allele was significantly increased in the patient group compared to healthy controls (0.569 vs 0.353, respectively, P 〈 0.0005). Furthermore, a C4A deletion nearly always occurred with the 8.5 kb HSP70 allele, suggesting that it may be a marker of the HLA-A1, B8, C4A deletion, DR3 extended haplotype.

    Notes: Abstract Charpy impact tests have been carried out on specimens removed from the joint region in butt fusion-welded polyethylene pipe. In order to ensure that crack initiation occurs precisely at the weld line, a novel method of notching has been used. G c data have been derived from these tests, which were conducted at temperatures of – 150 and 23 °C. Results obtained from pendulum impact and instrumented falling weight tests show consistent differences, probably attributable to the speed of impact. Data have also been obtained for the parent unwelded material.

    Notes: Abstract This paper describes, from a general system-design perspective, an artificial neural network (ANN) approach to a stock selection strategy. The paper suggests a concept of neural gates which are similar to the processing elements in ANN, but generalized into handling various types of information such as fuzzy logic, probabilistic and Boolean information together. Forecasting of stock market returns, assessing of country risk and rating of stocks based on fuzzy rules, probabilistic and Boolean data can be done using the proposed neural gates. Fuzzy logic is known to be useful for decision-making where there is a great deal of uncertainty as well as vague phenomena, but lacks the learning capability; on the other hand, neural networks are useful in constructing an adaptive system which can learn from historical data, but are not able to process ambiguous rules and probabilistic data sets. This paper describes how these problems can be solved using the proposed neural gates.

    Notes: Abstract Osteoporosis is a major health problem in Hong Kong, affecting about 200 000 patients among a population of 6 million. To evaluate the rate of bone loss and determinants of low bone mass among the Southern Chinese, we measured the total body and regional bone mass with the XR-26 dual-energy X-ray absorptiometry in 90 normal females. The results show that the total and regional bone densities were stable before menopause, with a change of around 0.5% every year. After menopause, a rapid loss is evident for total body and regional bone mass. The rate of bone loss varies from 1.2% for the pelvis to 3.1% for the Ward's triangle per year. Thus, by the age of 70, most Southern Chinese females would have lost at least 25% of their bone. Age, body weight, and skeletal area are important determinants for the total body bone mass. Bone densities in weight-bearing regions, including hip, pelvis, spine and legs correlate strongly with each other (r=0.68, P〈0.001). Correlation is poor among unrelated regions, such as between head and arms (r=0.39, P〉0.5). To conclude, normal Chinese women in Hong Kong lose bone rapidly after menopause at a rate of about 2% per year. This may account for the major incidence of osteoporosis-related complications in Hong Kong.

    Notes: Abstract To determine whether testosterone replacement therapy reverses the detrimental effects of hypogonadism on bone density, we measured the total body, lumbar spine and proximal femur bone mineral density (BMD) by dual-energy X-ray absorptiometry in 14 patients with Klinefelter's syndrome on long-term testosterone replacement therapy and compared the results with 14 age- and sex-matched normal controls. Seven of the patients were receiving oral testosterone undecanoate thrice daily (240 mg/day) and the others were having intramuscular testosterone enanthate injections once every 3 weeks (250 mg/injection). Their serum testosterone levels were maintained within the normal limits (10–40 nmol/l). We showed that patients on testosterone replacement had decreased amount of bone density in the left femoral neck when compared with the controls (p〈0.01). Similar decreases were also observed in the left Ward's triangle (p〈0.01) and in the left trochanter (p〈0.05). There were no significant differences in the total body and the lumbar spine measurements in these two groups of subjects. No correlation was found between the BMD values of femur and the duration of testosterone treatment in the patients with Klinefelter's syndrome. The type of testosterone treatment was also not associated with significant differences in BMD. In conclusion, sufficient testosterone replacement with currently available methods does not reverse the decrease in bone mass associated with hypogonadism in patients with Klinefelter's syndrome.

    Notes: Summary Free vascularized fibula grafts have been used to reconstruct long tibial bone defects since Taylor's report in 1975. A pedicled vascularized fibula graft is also useful for small bone defects or pseudoarthrosis because of its excellent vascularity. The pedicled vascularized fibula transfer does not need the microvascular anastomosis and can be performed in much less time. It can be distally or proximally pedicled, depending on the site of the bone defect. A proximally or distally pedicled vascularized fibula with or without a monitoring flap was transferred in five cases of small bone defects and pseudoarthrosis of the tibia. In all 5 cases, the postoperative course was uneventful and good bony union was obtained.
