    Notes: The pneuPAC Model 2-R pneumatic logic ventilator with a new air entrainment valve that offers the option of an Fio2 of l.0 or 0.45 is described. Its robustness, portability, variable Fio2, choice of positive end expiratory pressure and pressure relief valves make it versatile and suitable for transporting critically ill patients, as well as for resuscitation use when adverse conditions may be encountered. Two new medical air compressors, one mains driven and the other battery powered, designed for use with the ventilator, are also described. The function of the ventilator with the new valve was assessed using a piped gas supply and then reassessed when powered by the compressors.

    Notes: We studied sensitization to dinitrochlorobenzene (DNCB) in psoriatic patients before, during and 2,4, and 6 weeks after a course of photochemotherapy (PUVA). The inhibition of sensitivity observed during PUVA was completely reversed after 6 weeks. Thus the PUVA-mediated inhibition of cutaneous immune responsiveness is of short duration. However, whether there are long-term sequelae remains to be established.

    Notes: A capacitive-discharge spot welder is described which is primarily intended for making electrical connections between small (〈25–150 μm diameter) wires and samples of metallic materials, although it can be used in many applications where miniature welds must be made. It has been particularly useful in allowing low resistance (several mΩ) contacts to be created on samples of rare earth and actinide intermetallic compounds which are highly resistant to soldering. The device has a minimum time constant of about 25 μs and can deliver a maximum of between 700 μJ and 400 mJ of energy to the weld region, depending on the resistance of the latter. A suitable electrode holder is also described, and considerations for choosing the electrode materials, selecting the contact wires, preparing the sample, and creating a weld are discussed. © 1998 American Institute of Physics.

    Notes: Summary Human piebaldism is a rare autosomaal dominant disorder characterized by congenital depigmented patches of skin and hair. Piebaldism results from mutations of the kit proto-oncogene, which encodes a cell-surface receptor, tyrosine kinase, whose ligand is the stem/mast cell growth factor. We report four unrelated patients with piebaldism and consider the variations in phenotype in relation to the site of the kit gene mutation.

    Notes: A patient is described with the vascular abnormality typical of Klippel-Trenaunay-Weber Syndrome, associated with hypotrophy of the affected leg, rather than hypertrophy, and with reduced blood How and a vascular necrosis of the femoral head (Perthe's Disease) on the same side. This case supports the idea that the abnormality of limb size results from the abnormality of blood flow. It is likely that blood flow is grossly uneven, due to AV anastamoses, and therefore measurement of total flow-is unhelpful.

    Notes: Forty-two patients with chronic resistant varicose ulcers were observed for 6 weeks on standard out-patient therapy, and then randomly allocated to daily treatment with either cadexomer iodine (CI), a new topical agent, or dextranomer for a further 6 weeks, at which time ulcers judged clinically not to be responding could be changed to the other treatment for the remaining 20 weeks of the trial. There was no significant reduction in ulcer area during the first 6 weeks on CI or dextranomer. A significant preference for CI was shown by the optional crossover. Retrospective analysis of ulcer size shows this was not related to severity and preference was probably due to bias towards the new treatment, the trial not being blind. Neither therapy significantly reduced colonization by β-haemolytic Streptococcus, Staphytocoaus aureus, Pseudomonas or Proteus. The discrepancy between these and previous findings with CI is largely due to different methods of analysis rather than different results. We conclude that, in chronic resistant venous ulcers, topical therapy is of little importance compared with measures which improve tissue oxygenation and repair.

    Notes: Summary A simple procedure is described for the determination of the 1,2-propanediol (propylene glycol) content of cream formulations containing the antiviral drug Acyclovir. Analysis of a solvent extract by gas chromatography coupled to an infra-red spectrometer enables the target analyte to be positively identified and an estimate of its concentration to be made. This application utilises the newer Tracer cryogenic deposition interface for gas chromatography and infra-red spectroscopy. This offers an alternative and complementary analytical strategy to gas chromatography coupled to mass spectrometry.

    Notes: Summary The ability of methanogenic bacteria to adapt to high concentrations of calcium was investigated using a 9 l Upflow Anaerobic Sludge Blanket (UASB) reactor fed semi-continuously with a synthetic waste. It was found that a calcium concentration of up to 7000 mg/l had no inhibitory effect on anaerobic digestion.
