    Notes: Abstract One hundred and fifty-one children with acute lymphocytic leukaemia (ALL) received multiple agent induction chemotherapy followed by intensive phase treatment. One hundred and thirty-seven patients were randomised for the first year of maintenance treatment to receive reinforcement therapy (pulses) with either intermediate-dose methotrexate (ID-MTX) and prednisone (PRED) or vincristine (VCR) and PRED. The probablity of continuous complete remission (CCR) at 5.5 years is 0.80 for the ID-MTX group and 0.84 for the VCR group. Extramedullary relapses were not prevented either in the ID-MTX group nor in the VCR group. Since in previous studies VCR/PRED pulses did not increase CCR rates when given after intensive combination chemotherapy, it can be concluded from this study that neither did ID-MTX reinforcement therapy further improve treatment results in our patients with ALL when given after aggressive chemotherapy.

    Notes: Abstract Forty-seven children and adolescents with brain tumors were examined by magnetic resonance imaging (MRI) after tumor resection. The typical changes and complications after surgery and chemotherapy, as well as the corresponding MRI findings, are discussed.     Description / Table of Contents: Zusammenfassung Seit 1981 wird im Mainzer Universitätsklinikum vor allem bei peripher lokalisierten Ewing-Sarkomen eine Kombinationsbehandlung durchgeführt, die ohne erhöhtes Überlebensrisiko eine bleibende Funktionstüchtigkeit der betroffenen Extremität ermöglicht. Es handelt sich hierbei um eine zeitlich genau aufeinander abgestimmte Anwendung 4 verschiedener Chemotherapeutica, einer lokalen Resektion des tumortragenden Knochenabschnittes und einer von 60 auf 36 Gy reduzierten Röntgennachbestrahlung, die strahlenbedingte Spätschäden weitgehend vermeidet und zur Rezidivprophylaxe ausreicht. Eine osteoplastische Wiederherstellung ist ebenfalls möglich. Mehrere nach diesen Vorgehen behandelte Beispiele belegen diese Aussagen. Auch bei den prognostisch ungünstigeren zentralen Tumorlokalisationen sollte ein derartiges Vorgehen erwogen werden.
    Notes: Summary Since 1981, in the University of Mainz clinics, treatment of tumors of the peripheral extremities has consisted of polychemotherapy and irradiation in combination with local resection of the tumor-infiltrated part of the bone. This procedure permits the radiation dose applied to be reduced from 60 to 36 Gy to avoid late complications in the soft tissue or osteonecrosis without increasing patient risk. Osteoplastic reconstruction of bone is possible if it is necessary for function. A few cases are reported to demonstrate the success of the procedure in tumors of the distal extremities. A similar procedure should also be discussed for less-favorable proximal or central tumor locations.
