    Notes: Abstract Kaolin induced a clear and reproducible writhing reaction when intraperitoneally injected into mice. A simultaneous injection (i.p.) of soybean trypsin inhibitor (SBTI) significantly suppressed the kaolininduced writhing reaction. This writhing reaction was markedly potentiated by a simultaneous injection (i.p.) of captopril. In anin vitro experiment kaolin caused kinin-release in mouse plasma, possibly through the activation of prekallikrein. This activation of plasma prekallikrein and kinin-release were inhibited in the presence of SBTI. Some non-steroidal anti-inflammatory agents inhibited the kaolin-induced writhing reaction dose-dependently. These results suggest that kaolin-induced writhing reaction may be caused by the released bradykinin through activation of the plasma kallikrein-kinin system. This model is a novel and simple tool for assessment of analgesic agents.

    Description / Table of Contents: Résumé Huit patients (âge moyen : 10.1 ans) présentant une subluxation atloido-axoidienne avec symptômes neurologiques, relativement rare dans le syndrome de Down, ont été traités chirurgicalement. Dans tous les cas existaient diverses anomalies osseuses congénitales. Une importante myélopathie a été notée dans quatre cas avec ossiculum terminale, et une légère myélopathie dans un cas de spondyloschisis antéro-postérieur. Une arthrodèse postérieure réussie de C1 à C2 a été effectuée dans six cas, et de l'occiput à C2 dans les deux cas de spondyloschisis antéro-postérieur. Une excellente fusion osseuse et une amélioration de l'ensemble des symptômes ont été obtenues, avec un recul d'un an à cinq ans et six mois (en moyenne : trois ans et un mois après l'opération). La stabilisation chirurgicale précoce constitue le traitement de choix dans les cas de subluxation atloïdo-axoïdienne symptomatique, au cours du syndrome de Down, où il existe des anomalies osseuses congénitales, une laxité ligamentaire généralisée et où la moelle épinière elle-même est en danger.
    Notes: Summary Atlanto-axial subluxation, which was causing symptoms in eight patients with Down's syndrome, was treated by operation. In every case various congenital bony anomalies were found. Marked myelopathy was present in four cases of ossiculum terminale, and mild myelopathy in one with anteroposterior spondyloschisis. Successful posterior arthrodesis was performed from C1 to C2 in six cases, and from the occiput to C2 in the other two patients with anteroposterior spondyloschisis. Excellent bony union and an overall improvement in symptoms was achieved. The follow-up period was from one year to five years and six months. Early surgical stabilization is the treatment of choice in those cases of symptomatic atlanto-axial subluxation in Down's syndrome where there are congenital osseous anomalies and generalised ligamentous laxity, and where the spinal cord is at risk.

    Description / Table of Contents: Zusammenfassung Wir operierten 10 Patienten mit den folgenden seltenen Wirbelsäulentumoren: drei solitäre Plasmacytome, eine aneurysmale Knochenzyste, ein Chordom, einen Riesenzelltumor, ein Hämangiom, ein gutartiges Osteoblastom und ein Chondrosarkom. Diese Tumoren fanden sich in der Halswirbelsäule (ein Fall), in der Brustwirbelsäule (fünf Fälle), in der Lendenwirbelsäule (zwei Fälle) und im Sakrum (zwei Fälle). Es waren fünf männliche und fünf weibliche Patienten mit einem Durchschnittsalter von 43,1 Jahren. In sechs Fällen wurde Harrington-Instrumentation verwendet, in zwei Fällen Zielke-Instrumentation, eine Kombination von beiden in einem Fall und Kirschner-Draht für den letzten Fall. In zwei Fällen wurde nach totaler Resektion des Sakrum eine ilio-lumbale Verbindung mittels Harrington-Instrumentation durchgeführt. Die Fälle wurden durchschnittlich über fünf Jahre nachverfolgt. Der vorliegende Bericht beschreibt diese seltenen Fälle und betont die Wirksamkeit instrumenteller Chirurgie bei der Rekonstruktion der Wirbelsäule nach ausgedehnten Eingriffen.
    Notes: Summary We performed instrumentation surgery on ten patients with the following rare primary spinal tumors; three solitary plasmacytomas, an aneurysmal bone cyst, a chordoma, a giant-cell tumor, a hemangioma, a solitary bone cyst, a benign osteoblastoma, and a chondrosarcoma. These tumors were found in the cervical spine (one case), thoracic spine (five cases), lumbar spine (two cases), and sacrum (two cases). The patients consisted of five men and five women, and their average age was 43.1 years. Harrington instrumentation was used in six cases, Zielke instrumentation in two, a combination of both in one case, and a Kirshner wire in one. In two cases, iliolumbar fusion was performed with Harrington instrumentation, after total resection of the sacrum. Average duration of the follow-up period was 5 years. This report describes these rare cases and emphasizes the effectiveness of instrumentation surgery in the reconstruction of the spine after extensive surgery.
