    Notes: Abstract We present finite difference forward models of elastic wave propagation through laterally heterogeneous upper oceanic crust. The finite difference formulation is a 2-D solution to the elastic wave equation for heterogeneous media and implicitly calculatesP andSV propagation, compressional to shear conversion, interference effects and interface phenomena. Random velocity perturbations with Gaussian and self-similar autocorrelation functions and different correlation lengths (a) are presented which show different characteristics of secondary scattering. Heterogeneities scatter primary energy into secondary body waves and secondary Stoneley waves along the water-solid interface. The presence of a water-solid interface in the model allows for the existence of secondary Stoneley waves which account for much of the seafloor ‘noise’ seen in the synthetic seismograms for the laterally heterogeneous models. ‘Random’ incoherent secondary scattering generally increases aska (wavenumber,k, and correlation length,a) approaches one. Deterministic secondary scattering from larger heterogeneities is the dominant effect in the models aska increases above one. Secondary scattering also shows up as incoherence in the primary traces of the seisograms when compared to the laterally homogeneous case. Cross-correlation analysis of the initialP-diving wave arrival shows that, in general, the correlation between traces decreases aska approaches one. Also, because many different wave types exist for these marine models, the correlation between traces is range dependent, even for the laterally homogeneous case.

    Notes: Abstract Two patients are described in whom large gallstones, up to 4 cm in diameter, were removed percutaneously. The technique has some relevance to high risk patients with surgical cholecystotomies in whom elective cholecystectomy may be deferred if all the calculi can be removed. It may also be relevant if “percutaneous cholecystectomy” is ever developed clinically.

    Notes: Abstract Class III gene rearrangements have been examined in Thai/Chinese individuals with supratypes bearing defective or null C4 alleles. Genomic DNA from C4 null supratypes was probed with an almost full-length 21-OH probe following digestion with Taq I and Kpn I. The HLA-B17 C4A3 BQO BfS DR3 Thai/Chinese supratypes (which may be associated with insulin-dependent diabetes mellitus in Orientals) lacks a 3.2 kb Taq I and a 3.9 kb Kpn I fragment hybridizing with the 21-OH probe. Similar gene rearrangements are found in Caucasoid diabetogenic supratypes HLA-B18 C4A3 BQO BfF1 DR3 and HLA-B8 C4AQ0 BI BfS DR3. Interethnic comparisons suggest that class II and class III interactions may be important in disease susceptibility. By contrast, neither of two Thai/Chinese supratypes with C4AQ0 appear to have major class III gene rearrangements; disease association studies will determine the significance of C4deficiency per se. As in Caucasoids, the electrophoretically fast C4 allele, C4A6, in Orientals has been shown to correlate with a 12 kb Bg1 II fragment hybridizing with a C4 probe. It is likely that the HLA-B17 C4A6 BI BfS DR7 supratype marks a highly conserved MHC chromosomal segment.

    Notes: A kinetic model is used to predict the temperature evolution of a monatomic ideal gas undergoing an adiabatic expansion or compression at a constant finite rate, and it is then generalized to treat real gases. The effects of interatomic forces are considered, using as examples the gas with the square-well potential and the van der Waals gas. The model is integrated into a Carnot cycle operating at a finite rate to compare the efficiency's rate-dependent behavior with the reversible result. Limitations of the model, rate penalties, and their importance are discussed.

    Notes: If nurse practitioners are to achieve their goal of serving as the principal providers of primary health care and having a major voice in health policy decisons, they must increase their political effectiveness. Of all the problems that nurse practitioners face today, this is one of the most serious. This article identifies some of the reasons why nurse practitioners have failed to achieve maximum political effectiveness, such as lack of national stancardization of the role, titling confusion, and the lack of research to demonstrate that the nurse practitioner is the most appropriate and cost effective primary health care provider. Next, the consequences of the above are explored. Finally, some solutions are proposed, the most important of which is the establishment of a strong national nurse practitioner organization to serve as the voice of all nurse practitioners.
