    Notes: Summary An immunohistochemical study was made of ten cases of asymptomatic and three cases of symptomatic Rathke's cleft cyst. The cysts in the asymptomatic cases had monolayer columnar or cuboidal epithelium. Within the epithelium, cells which were positive for at least one of the pituitary hormones were found. The rate of positive reactions for these six pituitary hormones was between 70% and 100%. In contrast, the cysts in the symptomatic cases had an epithelium which was partly stratified squamous and partly squamous epithelium, and none of the pituitary hormones were found in them. Therefore, when a Rathke's cleft cyst enlarges to the extent that clinical symptoms are produced, we consider that changes have already occurred in structure and function of the cyst epithelium. In addition, we believe there is a tendency for monolayer epithelia to undergo squamous metaplasia and for cells which are positive for pituitary hormones to change into non-granulated cells.

    Notes: Summary We review the surgical results in 372 cases of multiple intracranial aneurysms over a 25-year period in which one of us (JS) performed 2,000 direct operations for aneurysms. All patients were classified into four groups according to the location of the aneurysm: Group 1: multiple aneurysms including anterior communicating artery aneurysm (157 cases); Group 2: multiple aneurysms of unilateral anterior circulation (72 cases); Group 3: multiple aneurysms of bilateral anterior circulation (110 cases); Group 4: multiple aneurysms including vertebro-basilar artery aneurysms (33 cases). In multiple aneurysm cases, our policy has been to treat all aneurysms, ruptured and unruptured, in a one-stage operation whenever possible. About 90% of patients in both Group 1 and 2 were treated by one-stage operations, while 60% of patients in Group 3 and 42% of patients in Group 4 were operated on in the same manner. Excellent and good results in from 73% to 81% of cases were obtained in patients in Group 1, Group 2 and Group 3. Morbidity was 14–19% and mortality was 6–8%. These results were comparable to the results with a single aneurysm of the anterior circulation. On the other hand, the surgical results in Group 4 were poor with a mortality of 27%. Poor results were attributable to the postoperative rebleeding from the untreated vertebro-basilar aneurysms, which were thought to be unruptured aneurysms preoperatively. Furthermore, it was clarified that the results of early one-stage operations (within one week from onset) in patients with multiple aneurysms were satisfactory. In this group, there was good recovery in 84% of patients, 7% were disabled and 9% died. The morbidity was notably lower in patients operated on within one week than in those operated on after 8 days. Based on these results, the one-stage operation in the acute period is recommended for patients with multiple aneurysms.

    Notes: Summary The flow threshold and time threshold for reversibility of cerebral ischemia were studied using a canine model of cerebral ischemia regulated by controlled perfusion of cerebral blood flow (CBF). CBF was continuously monitored with a laser Doppler flow meter, the brain was brought to a constant level of ischemia for a defined period of time, after which recirculation was instituted. The electroencephalogram (EEG) and somatosensory evoked potentials (SEP) were monitored and used as an index of brain function. No recovery of brain function was found following recirculation if the CBF was reduced to a level below 20% of the normal state for more than one hour. When residual blood flow was 30% of the normal level, however, recirculation after one hour of ischemia resulted in nearly complete recovery of brain function. Significant functional recovery was not seen after two or more hours of such ischemia. Nearly complete recovery was also seen following reperfusion within three hours with 40% of normal CBF. It was demonstrated that the reversibility in the ischemic brain was critically correlated to the level of blood flow and its duration. If these results can be applied to the human brain, emergency cerebral revascularization for ischemic stroke should be attempted when critical flow and time thresholds have not been crossed, namely, in less than 1 hour and 3 hours of insult when the residual blood flow is reduced to 30 and 40% of the normal state, respectively. In clinical situations, this “critical time” may be too short for acute revascularization unless cerebral protective measures are applied pre-operatively to prolong the viability period of ischemic cerebral tissue.

    Notes: Summary The authors report their experience with the surgical treatment of carotid-ophthalmic aneurysms in 29 cases, and describe their surgical technique. The technique can be summarized as follows. When dissecting the aneurysm, temporary vascular occlusion of the common carotid artery and external carotid artery is done in the neck under the administration of cerebral protective substances. Through a bifrontal craniotomy, wide dissection of the Sylvian fissures and the interhemispheric fissure is performed. When necessary, the anterior clinoid process and the roof of the optic canal are removed. This approach allows for observation of the neck of the aneurysm from various angles, thus facilitating clipping of the neck. There have been no previous reports of direct surgery on carotidophthalmic aneurysms using an interhemispheric approach, but this approach provides a much larger operative field and a better exposure of the aneurysm than other surgical approaches.

    Notes: Summary To study the therapeutic effect of phenytoin on cerebral ischaemia and confirm whether or not the effectiveness of phenytoin could be enhanced by combined administration with free radical scavengers, twenty-five dogs were subjected to ischaemia, using the “canine model of the completely ischaemic brain regulated with a perfusion method”. Five animals served as untreated controls, fifteen received treatment with several doses of phenytoin and five were treated with 10mg/kg phenytoin, 2g/kg mannitol and 30mg/kg vitamin E. These drugs were administered prior to the production of ischaemia. After one hour ischaemia, cerebral blood flow was restored and the recovery of electrical activity of the brain and the degree of brain swelling were observed for three hours. With regard to the recovery of the EEG, the higher the administered dosage, the better was the degree of recovery of the EEG. And the group which was treated with a combination of phenytoin, mannitol and vitamin E exhibited remarkable recovery of the EEG. With regard to the degree of brain swelling, a similar doserelated suppressive effect was seen in the phenytoin-treated groups. Furthermore, in the combination therapy group, brain swelling was attenuated significantly. Based on these results, it is concluded that phenytoin has a protective effect in cerebral ischaemia and it shows its most remarkable effect when given together with radical scavengers, such as mannitol and vitamin E.

    Notes: Summary Lipase fromPseudomonas fragi modified with polyethylene glycol was soluble and active in organic solvents such as benzene and chlorinated hydrocarbons. Using the modified lipase, terpene alcohol esters were synthesized with various combinations of terpene alcohols (citronellol, geraniol, farnesol and phytol) and carboxylic acids (acetic-, propionic-, n-butyric-, and valeric acids) in benzene at 25°C. The yield was generally very high.
