Summary Sixty-seven prolonged intubated critically ill patients were examined by direct laryngoscopy between the 3. and 14. day of intubation. It could be manifested that already after 5 days the majority of the investigated patients, showed severe laryngeal damage. Preferential damage occurs on the vocal cord (laryngeal granuloma), on the arytenoid vocal process and on the cricoidal plates (ulcerations). We concluded that latest at the 5th day of intubation a direct laryngoscopy should be performed to determine the extent of the laryngeal damage. If necessary a secondary tracheotomy should be considered.
