Zusammenfassung Das Aspergillom als Sonderform der Aspergillose ist chirurgisch von Bedeutung. Wenn auch röntgenologisch oft charakteristisch als gut abgegrenzte Solitärläsion mit Luftsichel imponierend und damit wertvolle diagnostische Hinweise gebend, reichen diese Symptome für die Differentialdiagnose nicht immer aus. Prä- und postoperative immunbiologische Untersuchungen an 70 Fällen vermitteln nicht nur die Überzeugung einer hohen diagnostischen Treffsicherheit (85 %), sondern erlauben auch eine kritische Verlaufsbeobachtung der Erkrankung nach Entfernung des Aspergilloms.

Summary Aspergilloma as a special form of aspergillosis is surgically relevant. X-ray investigation often reveals the typical pattern: a well-circumscribed solitary lesion featuring the characteristic light crescent. But these findings, though significant, are not always sufficient for differential diagnosis. Pre- and postoperative immunobiological intigations in 70 cases show a very high degree of diagnostic accuracy (85 %) and they also allowed critical observation of the condition's development after surgical removal of the aspergilloma.
