Zusammenfassung Bericht über 14 Fälle von Pankreastraumen; in 7 Fällen lagen isolierte Pankreasverletzungen vor; weitere 7 Fälle wiesen Kombinationsverletzungen mit Pankreas, Leber, Milz, Nieren- und Nebennieren auf. Das typische freie Intervall zwischen Unfallereignis und Beginn der klinischen Symptomatik wird anhand von Kasuistiken veranschaulicht. Bei Korpus- und Schwanzverletzungen hat sich in der operativen Soforttherapie die termino-terminale Pankreatikojejunostomie bewährt. Bei Verletzungen des Pankreaskopfes empfiehlt sich zunächst lediglich die Drainage. Die chirurgische Therapie der Spätkomplikationen (Pseudocysten, Fisteln und Gangstrikturen) erfolgt nach den allgemein bekannten Regeln der Pankreaschirurgie.

Summary The authors report on 14 cases of trauma of the pancreas; in 7 cases isolated lesions of the pancreas were treated, while in the other 7 cases blunt trauma was followed by typical combinations of ruptures of the pancreas, liver, spleen, kidneys, and suprarenal glands. A study of the case histories revealed the typical interval between the injury and the onset of clinical symptoms. Emergency treatment consists in terminoterminal pancreaticojejunostomy if both the corpus and the tail of the pancreas are injured. When lesions of the head of the pancreas are present drainage only is indicated initially. Surgical treatment of posttraumatic complications such as pseudocysts, fistulas, and cicatricial strictures follows the general rules for pancreatic surgery.
