    Description / Table of Contents: Zusammenfassung Bei 12 Patienten (1–4 Monate nach akutem Myocardinfarkt) wurde das Verhalten hämodynamischer Parameter in Ruhe und unter Belastungsbedingungen vor und nach i.v. Injektion von Furosemid und Ethacrynsäure untersucht. Herzzeitvolumen, Herzfrequenz und arterieller Druck zeigen nach Applikation von Furosemid und Ethacrynsäure keine signifikanten Veränderungen. Während nach Injektion von Ethacrynsäure (trotz gleich starker Diurese wie nach Furosemid) eine gerichtete Veränderung des diastolischen Pulmonalarteriendruckes (PA d) und rechten Vorhofdruckes (RA) nicht zu beobachten ist, bewirkt Furosemid bereits wenige Minuten nach Applikation eine signifikante Senkung des linksventriculären Füllungsdruckes (PA d) und des rechten Vorhofdrucks (RA). Diese hämodynamischen Veränderungen sind als Folge eines direkten dilatorischen Effektes von Furosemid auf die Kapazitätsgefäße aufzufassen. Eine entsprechende direkte Gefäßwirkung von Ethacrynsäure ist nicht nachzuweisen.
    Notes: Summary The haemodynamic effects of intravenous injections of furosemide and ethacrynic acid were studied in 12 patients during rest and excercise. Cardiac output, heart rate and arterial blood pressure showed no significant changes after injection of furosemide or ethacrynic acid. Few minutes after injection of furosemide there were significant falls of pulmonary artery diastolic pressure and right atrial pressure, whereas no changes were seen after injection of ethacrynic acid. This initial effect of furosemide apparently is of primarily vascular origin, causing an increased peripheral venous capacitance. Ethacrynic acid failed to exhibit any direct vascular effect

    Notes: Summary On the basis of a three stage grading system we report 23 stage one recurrent oligodendrogliomas (O 1), and 29 stage two recurrent oligodendrogliomas (O 2). In the O 1 group after the first interval 15 became O 2 and 2 became glioblastomas. Twenty tumours of the O 2 group after the first interval were not changed, three became oligodendroglioma-astrocytomas stage 2, and six became glioblastomas. The time relation for the recurrent phase in the primary O 1 group is calculated as 42 months, and in the primary O 2 group as 22 months, but this is without significance. For the development of malignancy, especially for the change to glioblastoma, a prominent participation by transformed local astrocytes seems to be essential. Postoperative irradiation most probably does not favour malignant change. A prolongation of the expectation of life by radiotherapy is not noticed.

    Notes: Summary On the basis of our previous investigation regarding histological transformation of recurrent astrocytomas and oligodendrogliomas we report the clinical results of reoperations. The present observations deal with 121 cases: grade 1 astrocytomas (45), oligodendrogliomas (12), grade 2 astrocytomas (47), and oligodendrogliomas (17), respectively. In all these a second reoperation was performed in 14 cases. Operative mortality was relatively high, but without significant difference in the two groups of malignancy: 24.5% and 29.6%. Survival times in both groups showed striking individual differences. We achieved post-reoperation survivals exceeding one year in 18 and 14 cases, respectively. The results of second reoperations were generally poor. We find no remarkable differences in survival time following reoperations in primary grade 1 and grade 2 tumours. Similarly, malignant change proved to be not decisive for survival times. On the contrary, most of our patients with unchanged grade 1 astrocytomas, reoperated on after a longer interval, achieved a short second survival only. Prolongation of life expectancy by radiotherapy is doubtful. Irradiation seems to be necessary with malignantly transformed tumours if the patient did not have any treatment before reoperation.

    Notes: Summary We report 137 recurrent supratentorial astrocytomas. The primary tumours diagnosed on the basis of a grading system with three stages were 72 astrocytomas I and 65 astrocytomas II. In the first group 14% of the recurrences were not changed, 55.5% became astrocytomas II, and 30.5% became glioblastomas. In the second group 55.4% were unchanged, and 44.6% became glioblastomas. The postoperative intervals until reintervention or death were statistically examined. It seems that the recurrence time chiefly depends on the nature of the primary tumour. The transformation of an astrocytoma I to a glioblastoma takes longer than the transformation of an astrocytoma II into a glioblastoma. In about two thirds of all astrocytomas an increase of malignancy is to be expected. From the histological picture it is not possible in an individual case to predict the likelihood or speed of malignant change. With regard to the effect of irradiation the authors conclude that radiotherapy most probably does not produce malignancy.

    Notes: Summary A case report of Tangier disease (Anlipoproteinemia). The symptom is the orange yellow discolouration of hyperplastic tonsills. Further typical but not always present findings are orange yellow changes of the oropharynx and nasopharynx and enlargement of lymph nodes. The clinical, histological and electron-microscop ical findings are described. Being acquainted withe the disease the ENT specialist can establish an early diagnosis.

    Notes: Summary 26 mongrel dogs were given a single dose of 0.03 mg/kg tritium-labelled digoxin, β-methyldigoxin, digitoxin or ouabain 2 hrs or 95 hrs following experimental coronary occlusion. Examination of the epicardial ECG was performed by moving from intact to ischemic or necrotic zones. 60 min after glycoside administration the animals were sacrificed and tissue samples from the marked heart muscles areas and from the skeletal muscle were analysed for glycoside content. The early glycoside uptake in acute ischemic or necrotic myocardium was diminished independently of the physicochemical properties of the glycoside. Significantly higher glycoside concentrations (ng/g wet weight) were measured in the injured myocardium 3 hrs after coronary occlusion than 96 hrs afterward (p〈0.005). The values in acute ischemic myocardium varied considerably. This non-homogeneity of glycoside uptake in the acute ischemic heart muscle may partly explain the increased sensitivity to glycosides in myocardial infarction. The decline of glycoside concentration correlates with the alterations in the epicardial ECG. The cardiac effects of cardenolides 60 min after intravenous administration was caused by the unchanged glycoside. In contrast to the myocardium, glycoside accumulation could not be found in the skeletal muscle. The concentrations of digoxin, β-methyldigoxin and digitoxin in the skeletal muscle were significantly higher than the concentration of ouabain, which was rapidly eliminated via the urine.

    Notes: Summary In order to induce a serous otitis media in cats, the pharyngeal opening of the Eustachian tube was cauterized and obstructed on one side only through transpalatal approach; the other side remaind intact and served as control. On tympanic membrane examination and puncture 11 months later, there was fluid in the middle ear in three ears, no fluid in two ears, a tympanic membrane perforation in one ear and a red tympanic membrane in one ear. The middle ear was then ventilated through a plastic tube in two ears with and without fluid respectively. In all other ears, no tube was inserted. After another 7 months, the animals were intravitally perfused, and the middle ear mucosa biopsyed. The histological results of the light and electron microscopy showed chronic changes of the middle ear mucosa following experimental tubal obstruction.

    Description / Table of Contents: Zusammenfassung Es wird über die ersten in Deutschland entdeckten Patienten mit einer Hypo-α-Lipoproteinämie — Tangier disease — berichtet. Diese Fettstoffwechselstörung zeichnet sich durch eine Verminderung der HDL-Lipoproteine im Serum sowie durch Cholesterinspeicherung in Histiocyten aus. Hervorstechendes Merkmal ist eine orange-gelbliche Verfärbung des lymphatischen Rachenringes; die weiteren Symptome können vielgestaltig sein, der Verlauf der Erkrankung ist wenig typisch. Als Ausdruck der wahrscheinlich autosomal rezessiv vererbten Fettstoffwechselstörung werden hauptsächlich Cholesterinester im Reticuloendothelialen System (z. T. in kristalliner Form mit Doppelbrechung) gespeichert, in geringerem Maße auch in glatten Muskelzellen, Pericyten und Schwannschen Zellen. Der Morbus Tangier kann durch laborchemische und ultrastrukturelle Kriterien von anderen Fettstoffwechselstörungen abgegrenzt werden. Eine causale Therapie ist nicht bekannt, der HNO-Arzt kann die sicher häufiger vorkommende Krankheit frühzeitig erkennen.
    Notes: Summary The report deals with the first three cases of Tangier disease (Hypo-α-Lipoproteinaemia) observed in Germany. This rare metabolic disorder is distinguished by a diminution of HDL-Lipoproteins in serum and a lipid storage in histiocytes. The prominent symptom is the orange yellow discolouration of hyperplastic tonsills and adenoids; further symptoms of the disease generally are various, the development is little typical. The morphological substrat of the deranged fat metabolism is cholesteryl ester storage in the reticuloendothelial system, smooth muscle cells, pericytes and Schwann cells of peripheral nerves. M. Tangier can be differentiated from other lysosomal defects by ultrastructural and chemical parameters, it seems to have a autosomal recessiv inheritance. It is the ENT-specialist, who can establish an early diagnosis of this disease, provided he is aware of its clinical features.
