    Description / Table of Contents: Summary The author presents a report on eight female patients with various hematological diseases (primary and secondary thrombopenia, thrombasthenia) in whom it was possible to control severe meno- and metrorrhagia by ovulation inhibitors (the combination drugs Anovlar or Lyndiol). The treatment may be continued for a long period of time without detrimental effects on the patient. The effect of the drug is readily reversible.
    Notes: Zusammenfassung Es wird über 8 Patientinnen mit verschiedenen hämatologischen Erkrankungen (primäre und sekundäre Thrombopenic, Thrombasthenie) berichtet, bei denen schwere Meno- und Metrorrhagien durch Ovulationshemmer (Kombinationspräparate Anovlar oder Lyndiol) unterdrückt werden konnten. Die Behandlung kann ohne Schaden für die Patienten über einen langen Zeitraum fortgesetzt werden. Die Wirkung der Präparate ist prompt reversibel.

    Description / Table of Contents: Summary 1. 16 patients with myocardial infarction who showed no overt diabetes mellitus were investigated. 2. On the basis of the tolbutamide test, half of the patients showed a latent diabetes while another third was highly suspicious. 3. Although in all cases the fasting blood sugar was less than 105 mg-%, the mean in the patients with myocardial infarction ( $$\bar x$$ =79 mg-%) was higher than that of a control group ( $$\bar x$$ =62,4 mg-%). 4. The means of the triglycerides, cholesterol and free fatty acids in patients with myocardial infarction were higher when compared with a control group, but did not differ from those in patients with treated diabetes. 5. On the basis of these investigations one can assume that latent diabetes might play an important role in the pathogenesis of myocardial infarction.     Notes: Zusammenfassung 1. Wir untersuchten 16 Patienten mit durchgemachtem Myokardinfarkt, die keinen manifesten Diabetes mellitus aufwiesen. 2. Auf Grund des Tolbutamidtestes bestand bei der Hälfte der Patienten ein latenter Diabetes, bei einem weiteren Drittel war eine latent diabetische Stoffwechsellage wahrscheinlich. 3. Obwohl der Nüchternblutzuckerspiegel in allen Fällen unter 105 mg-% lag, war der Mittelwert bei Patienten mit Myokardinfarkt ( $$\bar x$$ =79 mg-%) höher als bei einer Kontrollgruppe ( $$\bar x$$ =62,4 mg-%). 4. Triglyceride, Cholesterin, freie Fettsäuren waren bei Infarktpatienten im Mittel gegenüber einer Kontrollgruppe erhöht; gegenüber Diabetikern, die unter Behandlung standen, fand sich kein Unterschied. 5. Auf Grund dieser Untersuchungen ist anzunehmen, daß der latent diabetischen Stoffwechsellage eine außerordentliche Bedeutung als Ursache oder Auslösung des Myokardinfarktes zukommt.

By constant infusion of 9,10-H3-palmitic acid as an albumin complex and by separation of the main lipid fractions by means of thin layer chromatography the turnover rates of free fatty acids (FFA) and triglycerides (TG) in plasma, the plasma TG clearence and the plasma TG turnover time were estimated in 9 normal persons, 5 diabetics, and 6 patients with essential hyperlipemia. 2. A positive correlation was found between the concentration of FFA in plasma and TG influx (p〈0,01). This was found both in normal persons and in patients with symptomatic as well as essential hyperlipemia. In patients with essential hyperlipemia the concentration of FFA was elevated (p〈0,02) and the incorporation of FFA into plasma TG was increased (p〈0,05), but did not differ from diabetics with the same elevation of plasma FFA. The results show, that in patients with essential hyperlipemia the increased influx of TG cannot be responsible for their extremely elevated TG level. 3. In normals and patients with diabetes a positive correlation was found between TG influx and the log of TG concentration in plasma. This does not apply, however, to patients with essential hyperlipemia. In these patients TG clearence is reduced to about one fifth and plasma TG turnover time is prolonged about tenfold. It is suggested that an impaired efflux of plasma TG is responsible for the high TG level in patients with carbohydrate induced hyperlipemia.

Durch konstante Infusion von 9,10-H3-Palmitinsäure und Auftrennung der Hauptlipidfraktionen durch Dünnschichtchromatographie wurden Umsatzraten der freien Fettsäuren und der Plasmatriglyceride sowie Plasmatriglycerid-Clearence und Plasmatriglycerid-Umsatzzeit bei Stoffwechselgesunden und Patienten mit essentieller Hyperlipämie untersucht. 2. Bei Patienten mit essentieller Hyperlipämie ist die Einbaurate der freien Fettsäuren in Plasmatriglyceride im nüchternen Zustand im Mittel gegenüber Stoffwechselgesunden leicht erhöht. Diese Erhöhung wird vermutlich durch die erhöhte Konzentration an freien Fettsäuren im Plasma dieser Patienten verursacht. Die Triglycerid-Clearence ist bei diesen Patienten aber auf das Fünffache erniedrigt und die Triglycerid-Umsatzzeit entsprechend auf das Zehnfache erhöht. 3. Auf Grund dieser Ergebnisse ist eine Störung im Abtransport der Plasmatriglyceride als Ursache des extrem erhöhten Triglyceridspiegels bei Patienten mit essentieller Hyperlipämie anzusehen.

Free fatty acids, triglycerides, total cholesterol, and phospholipids were estimated in the plasma of diabetic patients without ketoacidosis and without treatment, of well controlled tolbutamide treated patients, of well controlled insulin treated patients, as well as of diabetic patients with ketoacidosis. 2. Compared with age-matched normal persons, the most striking differences were found in the concentration of triglycerides and free fatty acids. 3. The level of triglycerides was elevated in all groups of diabetics. The highest concentration of free fatty acids was found in patients with ketoacidosis. In patients with well controlled maturity onset diabetes the increase in the level of free fatty acids could not be prooved statistically. 4. An increase of the concentration of total cholesterol or phospholipids could be demonstrated only in well controlled insulin treated diabetics. 5. There is a good positive correlation between relative body weight and the concentration of triglycerides in plasma of normal persons and well controlled diabetics. Whereas in normal persons an overweight of 10 kg is linked with an increase of 8 mg-% triglycerides, in tolbutamide treated patients the corresponding increase is 28 mg-% and in insulin treated patients 39 mg-%.

Die einzelnen Lipidfraktionen (Cholesterin, Triglyceride, Phospholipide und freie Fettsäuren) im Plasma von frisch entdeckten unbehandelten Diabetikern, von gut eingestellten mit Tolbutamid bzw. Insulin behandelten Diabetikern und schließlich von Diabetikern, die zum Zeitpunkt der Untersuchung eine Ketonurie aufwiesen, wurden bestimmt. 2. Die ausgeprägtesten Veränderungen gegenüber entsprechend ausgewählten Normalpersonen fanden sich in der Konzentration der Triglyceride und der freien Fettsäuren. 3. Der Triglyceridspiegel war in allen untersuchten Gruppen signifikant erhöht. Die höchste Konzentration an freien Fettsäuren wiesen Patienten mit Ketose auf; bei gut eingestelltem gutartigen Altersdiabetes war die Erhöhung der freien Fettsäuren statistisch nicht mehr zu sichern. 4. Eine geringe, jedoch statistisch signifikante Vermehrung des Gesamtcholesterins fand sich nur in der Gruppe der insulinbehandelten Diabetiker. Dasselbe gilt für die Phospholipide. 5. Eine gute positive Korrelation zwischen relativem Körpergewicht und Triglyceridspiegel wurde im Plasma von Normalpersonen und gut eingestellten Diabetikern nachgewiesen. Während bei einem Übergewicht von 10 kg bei Normalpersonen eine Erhöhung der Triglyceride um 8 mg-% gefunden wurde, betrug die Erhöhung bei tolbutamidbehandelten Diabetikern 28 mg-%, bei insulinbehandelten sogar 39 mg-%. 6. Die klinische Bedeutung dieser Ergebnisse, vor allem in Zusammenhang mit artherosklerotischen Spätkomplikationen, wurde diskutiert.
