Summary The blood clotting factors prothrombin, factor V, factor VII-complex, and thromboplastin-generation test have been investigated in the contents of Cantharid blisters and, for purposes of comparison, in venous blood. The question to be answered was whether patients with disturbed blood clotting or vascular permeability demonstrate a difference, as compared to healthy controls, in the extravascular behavior of blood clotting factors. In hemophilia A, the values for prothrombin, factor VII-complex, and for the reduced factor VIII in Cantharid blister contents are the same as in venous blood, while the activity of these factors in the extravascular fluid is significantly lower in normal individuals. By means of the thromboplastin-generation test, no difference can be seen between the so-called tissue fluid of a hemophilic A and an individual with normal clotting. Patients with thrombocytopenia or with a probably increased vascular permeability also did not show any differences between the intra- and extravascular activity of prothrombin. Factor V was significantly less intensely reduced in the extravascular fluid of all three groups of patients examined than in the control group. These results point to an increased vascular permeability for plasma clotting factors in hemophilia and in thrombocytopenia. The clotting potential in the interstitial space is, accordingly, above normal in the disease groups studied here.

Zusammenfassung Im Inhalt einer Cantharidenblase und vergleichend im Venenblut wurde die Blutgerinnung untersucht (Prothrombin, Faktor V, Faktor VII-Komplex, Thromboplastinbildungstest). Es sollte die Frage beantwortet werden, ob bei Störungen der Blutgerinnung oder der Gefäßpermeabilität Unterschiede im extravasalen Verhalten der Blutgerinnung gegenüber gesunden Kontrollen vorkommen. Bei Hämophilie A waren die Werte für Prothrombin, Faktor VII-Komplex und für den verminderten Faktor VIII im Cantharidenblaseninhalt gleich wie im Venenblut, während bei Gesunden die Aktivität der Faktoren in der extravasalen Flüssigkeit signifikant geringer ist. Mit dem Thromboplastinbildungstest kann man im sog. Gewebssaft eine Hämophilie A nicht von Gerinnungsgesunden unterscheiden. Bei Kranken mit Thrombocytopenie oder mit wahrscheinlich gesteigerter Gefäßpermeabilität fanden sich für Prothrombin ebenfalls keine Differenzen zwischen intra- und extravasaler Aktivität. Faktor V war in allen drei untersuchten Krankheitsgruppen in der extravasalen Flüssigkeit signifikant weniger stark vermindert als bei den Kontrollen. Die Befunde weisen auf eine gesteigerte Gefäßdurchlässigkeit für plasmatische Gerinnungsfaktoren bei Hämophilie und Thrombocytopenie hin. Das Gerinnungspotential im interstitiellen Raum ist demnach bei den untersuchten Krankheitsgruppen gegenüber der Norm erhöht.
