Summary 1. With the aid of surgically secured material of diseases of the thoracic spine, it was possible to show that functional disturbances of the inner organs, which correspond to the thoracic segments, may represent a very frequent and often initial symptom of these diseases. Their mechanism is to be conceived, in the first place, in such a way that the process in the thoracic vertebrae leads to an irritation of the corresponding posterior radices, their vessels, and frequently also of their shares of the sympathetic trunk, at least, however, to a disturbance of the nutrition reflex of the corresponding inner organ which then leads to functional disturbances of the organ. 2. We believe that precisely the irritation of the surrounding vessels of the radix, which may frequently be observed on the occasion of operations, plays an important part in this functional process, since it may be released and maintained by climatic influences, psychical emotions, vasomotor irritations, hormonal influences and other factors. This assumption is substantiated by the fact, too, that it may frequently be reduced by blocking the intervertebral space concerned with novocain. Internists, surgeons and neurologists will have these facts in view in the future with regard to both diagnostics and therapeutics.

Résumé 1. À l'aide d'un matériel opératif de lésions de la colonne vertébrale thoracique on pouvait montrer que des troubles fonctionnels des visires correspondants aux segments thoraciques peuvent représenter un symptôme fréquent et souvent initial de ces maladies. Leur mécanisme doit être compri surtout de telle manière que le procès dans les vertèbres thoraciques mène à une irritation des racines postérieures correspondantes, de leur vaisseaux et fréquemment aussi de leur part du grand nerf sympathique, au moins, pourtant, à un trouble du reflet de nutrition de l'organe intérieur correspondant qui, après, mène à un trouble fonctionnel de l'organe. 2. Nous croyons que précisément l'irritation des vaisseaux voisins de la racine que l'on peut fréquemment observer à l'occasion d'opérations joue un rôle important dans ce procès fonctionnel car elle peut être provoquée et maintenue par des influences climatiques, par des émotions psychiques, par des irritations vasomotrices, par des influences hormonales et par d'autres facteurs. Cette supposition est soutenue aussi par le fait que l'on réussit fréquemment à la couper à l'aide d'un blocage de l'espace intervertébral en question avec de la novocaïne. À l'avenir, l'interniste, le chirurgien et le neurologiste devront tenir compte de ces faits non seulement à l'égard du diagnostic mais encore à l'égard de la thérapeutique.

Zusammenfassung 1. An Hand eines zum großen Teil operativ sichergestellten Materials von Brustwirbelsäulenerkrankungen konnte gezeigt werden, daß funktionelle Störungen der inneren Organe, die den thorakalen Segmenten zugeordnet sind, ein sehr häufiges und oft initiales Symptom dieser Erkrankungen darstellen können. Dabei ist der Mechanismus in erster Linie so aufzufassen, daß es durch den betreffenden Brustwirbelprozeß zu einer Irritation der zugehörigen hinteren Wurzeln, ihrer Gefäße und oft auch der Grenzstranganteile, zumindest aber der sympathischen Gefäßanteile, und dadurch zu einer Störung des Nutritionsreflexes des zugehörigen inneren Organes kommt, was dann zu funktionellen Störungen am Organ führt. 2. Wir glauben, daß gerade die immer wieder bei Operationen feststellbare Irritation der Nachbargefäße der Wurzel eine ganz besondere Rolle bei diesen funktionalen Geschehen spielt, da sowohl Klimaeinflüsse wie psychische Erregungen, vasomotorische Reize, hormonale Einflüsse und anderes mehr diese auslösen und unterhalten können. Auch die Tatsache, daß man sie häufig durch Novocainblockade des betreffenden Intervertebralraumes kupieren kann, spricht in gleichem Sinne. Sowohl der Internist wie der Neurologe und Chirurg werden also in Zukunft ihr Augenmerk vielmehr auf diese Tatsachen sowohl in der Diagnostik wie in der Therapie zu richten haben.
