Summary Report on 264 operated pituitary adenomas. 1. A report on 264 cases of pituitary adenoma operated in the years from 1933 to 1952. By inoperable tumors the mortality at the operation was 10,4% in all cases including 12 deaths after an exploratory trepanation; excluding the inoperable cases the percentage was 5,8%. 2. The AA. confirm the unfavorable prognosis in adenomas with disorders of the ocular muscles. It is also in direct relation to the decrease of the visus and lesion of the visual field. 3. The regeneration of the visus and visual field in the above mentioned cases corresponds to the data in literature as well as to the degree of recovered working-capacity. 4. The endocrinous deficiencies are indicated more precisely. In 40% of the cases there was a latent or evident diabetes. In these cases the hypotonia of the blood flow and the inclination towards a retention of liquid deserve a special observation. This was observed in 57% by chromophilic adenomas and in 86% by chromophobe adenomas. 5. Only one third of the metabolic disorders found relief in operation. As a substitution, a calf hypophysis was implanted. Its effect could be noticed in the hormonic secretion of the cortical substance of the suprarenal capsule in the urine. As yet nothing can be said about a lasting success regarding this implantation.

Résumé 1. Les A. analysent 264 cas des tumeurs hypophysaires opérées pendant 1933 à 1952. La mortalité postopératoire était 10,4%, en concernant aussi les cas inopérables. Sans ces cas la mortalité était 5,8%. 2. Les cas avec une lésion motrice oculaire ou une progression rapide d'une hémianopsie montrent une prognose moins favorable. 3. La régénération du visus et des champs visuels correspond aux constatations de la littérature mondiale, aussi la capacité de travail. 4. Les recherches sur les lésions endocrinologiques sont discutées exactement. 40% des adénomes hypophysaires montrent un diabète manifeste ou latent. En particulier, c'est d'une grande valeur l'a constatation d'une hypotonie circulatoire et la tendence de ces cas à une rétention de l'eau. On trouve ces conditions chez 57% des adénomes chromophiles et chez 86% des adénomes chromophobes. 5. Seulement 33% des lésions du métabolisme furent enlevées par l'intervention. Pour substitution furent implantées des hypophyses de veau. L'effet de cette thérapie s'est montré par l'apparition des hormones surrénales dans l'urine; il n'est pas encore possible communiquer les résultats éloignés de cette thérapie.

Zusammenfassung 1. Es wird über 264 operierte Hypophysenadenome aus den Jahren 1933 bis 1952 berichtet. Die Operationsmortalität betrug für alle Fälle einschließlich 12 Todesfällen nach Probetrepanation bei inoperablen Geschwülsten 10,4%, ohne die inoperablen 5,8%. 2. Die ungünstige Prognose der Adenome mit Augenmuskelstörungen wird bestätigt. Desgleichen läßt die Prognose ein Parallelgehen mit der Zunahme der Visus- wie Gesichtsausfälle erkennen. 3. Auch die Regeneration von Visus und Gesichtsfeldern entspricht den Angaben des Schrifttums. Desgleichen der Grad der wiedererlangten Arbeitsfähigkeit. 4. Ausführlicher werden die inkretorischen Ausfälle dargestellt. Manifeste und latente diabetische Stoffwechsellage zeigte sich in 40%. Besondere Beachtung verdient die Kreislaufhypotonie und die Neigung dieser Fälle zur Wasserretention. Sie fand sich bei chromophilen Adenomen in 57%, bei chromophoben in 86%. 5. Nur ein Drittel der Stoffwechselstörungen ließ sich durch die Operation beseitigen. Zur Substitution wurden Kalbshypophysen implantiert. Ihre Wirksamkeit ließ sich an den im Urin ausgeschiedenen Nebennierenrindenhormonen feststellen. Über Dauererfolge dieser Substitution kann noch nicht berichtet werden.
