Zusammenfassung Aus einer Gesamtzahl von 3850 Operationen an Colon und Rectum (1964-1980) wurden 52 Handnaht-Anastomosen nach anteriorer Rectumresektion (Resektionsrate 47 %) ohne Stoma mit 52 Maschinennaht-Anastomosen verglichen. Eine Ausweitung der Operationsindikation ergab sich nicht. Die Komplikationsraten waren vergleichbar, Anastomoseninsuffizienzen verliefen nach Maschinennaht leichter (Handnaht: 4, Maschine: 3). - Die Maschine bringt dem erfahrenen Chirurgen Vorteile, jedoch sollte nur der sie gebrauchen, der die schwierige Versorgung technischer Komplikationen beherrscht.

Summary Of a total 3850 operations on the colon and rectum from 1964 to 1980, a comparison was made of 52 hand-sutured anastomoses after anterior rectum resection (9/79-8/80) (resection quota 47%) without intestinal fistula and 52 anastomoses by machine (EEA). Indication for surgery was not extended by use of the machine. Complications like anastomosic insufficiency occurred in a similar range (4 hand, 3 machine) but were easier to manage after machine suture; the number of other complications was comparable. Use of the machine gives advantages to the surgeon of high experience, but it should be used only by those who are able to master the technical complications.
