    Notes: Abstract Background: Advanced laparoscopic procedures are more commonly performed in elderly patients with cardiac disease. There has been limited data on the use of pulmonary artery catheters (PAC) and transesophageal echocardiography (TEE) to monitor hemodynamic changes. Methods: We prospectively studied eight patients undergoing laparoscopic assisted abdominal aortic aneurysm repair. All patients had a PAC and all but one had an intraoperative TEE. Data included heart rate (HR), temperature (temp), pulmonary artery systolic (PAS) and diastolic (PAD) pressures, mean arterial pressure (MAP), central venous pressure (CVP), pulmonary capillary wedge pressure (PCWP), cardiac index (CI), mixed venous oxygen saturation (MVO2), and oxygen extraction ratio (O2Ex) and was obtained prior to induction, during insufflation, after desufflation, during aortic cross-clamp, and at the end of the procedure. End diastolic area (EDA), a reflection of volume status, was measured on TEE. ANOVA was used for data analysis. Results: No changes were noted in HR, temp, PAS, PCWP, CI, MVO2, and O2Ex. PAD and CVP were greater during insufflation compared with baseline and aortic cross-clamp without associated changes in EDA. MAP was higher at baseline compared with all other times during the procedure. Conclusions: Insufflation increased PAD and CVP. However, volume status as suggested by EDA and PCWP did not change. These data question the reliability of hemodynamic measurements obtained from the PAC during pneumoperitoneum and suggest that TEE may be sufficient for evaluation of volume status along with the added benefit of timely detection of ventricular wall motion abnormalities.

    Notes: Abstract Background: Laparoscopic surgery decreases postoperative pain and length of hospital stay. Whether laparoscopically assisted abdominal aortic aneurysm (AAA) repair can be safely and reliably performed is unknown. This prospective study was designed to establish the feasibility of laparoscopically assisted AAA repair and its effects on intraoperative and postoperative variables. Methods: With IRB approval, 10 patients with infrarenal AAA requiring a tube graft underwent laparoscopically assisted AAA repair. The procedure consisted of laparoscopic dissection of the aneurysm neck and iliac vessels. Then, through an 8–11-cm minilaparotomy, a standard endoaneurysmorrhaphy was performed. Data included laparoscopic and total operative times, blood loss, fluid requirements, duration of nasogastric suction (NGT), and lengths of intensive care unit (ICU) and postoperative hospital stays. Results: Laparoscopically assisted AAA was completed in nine of 10 patients. The first patient was converted to a standard incision because the aneurysm neck could not be adequately dissected. Laparoscopic and total operative times were 1.8 ± 0.4 and 4.5 ± 0.7 h, respectively. Mean blood loss was 1 ± 0.6 l. Intraoperative fluid requirement was 6.6 ± 1.3 l. The duration of NGT suction was 1.8 ± 1.0 days. The ICU stay was 2.1 ± 0.8 days and hospital stay was 6.7 ± 2.5 days. There were two minor complications and no deaths. Conclusions: Laparoscopically assisted AAA repair is technically feasible with acceptable blood loss, operative time, morbidity, and mortality. Potential advantages may be early removal of the NGT and shorter ICU and hospital stays. Prospective randomized trials are needed to determine if laparoscopically assisted AAA repair is advantageous.

    Notes: Measurements of solids motion in a fluidized bed have been made in a computer-aided particle tracking facility. A radioactive tracer particle, dynamically identical to the solid particles to be studied, was mixed with the solids in the bed. The gamma radiation from the tracer was continuously monitored by a large number of scintillation detectors located around the bed, providing information on the tracer's instantaneous location. Prudent use was made of the purposely introduced redundant distance data to achieve improved accuracy. The recognition of the existence of secondary emission due to the interaction of the primary radiation and other system materials and the subsequent devising of a scheme to mitigate its effect contribute much to the success. Results for a bed with a uniform air distributor plate show the existence of two counter-rotating toroidal vortices whose relative sizes and strengths vary with the fluidizing velocity. Fluctuating motions at low frequencies ranging from 1.6 to 16 Hz have been observed at various locations in the bed.

    Notes: The theoretical equations for the flow of Newtonian fluids in a falling cylinder viscometer with eccentricity are developed. The error in fall velocity of the cylinder caused by eccentricity is determined and presented in tabular form. An approximate method to determine the error in fall velocity caused by eccentricity for non-Newtonian fluids of the power law type is also presented. For the Newtonian case, n=1, the approximate method is shown to give excellent agreement with the exact method.

    Notes: The dielectric behavior of glassy thermoplastic polymer in the microwave frequency range was investigated. Specifically, the relationship between energy absorption and temperature for several thermoplastic systems was examined to test the theoretical basis for heating under microwave irradiation. Irradiation under traveling and standing wave conditions were explored. The heating rate versus temperature data at a frequency of 2.45 GHz yielded a microwave calorimetry procedure for examination of the dielectric and relaxation behaviors. Correlations were drawn between (a) the apparent activation energy and the critical temperature, and (b) the shape of the dielectric spectra at 2.45 GHz and its shape in the kHz region. WLF relationships were examined for glassy thermoplastics to show the difference in changing activation energy with temperature.

    Notes: The objective of this effort has been to investigate the relationship between polymer structure and microwave absorptivity. Dielectric loss factor, ε″, loss tangent, tan δ, and oscillator strength, (εS -- ε∞), were used to evaluate potential material processability under applied microwave radiation. Numerous polymeric materials varying in chemical and physical structures were irradiated in a low power (≤ 100W) electric field at 2.45 GHz. Electromagnetic radiation was applied as either traveling or resonant wave modes in cylindrical and rectangular waveguides. In general, heatability was found to be a direct function of the dielectric loss dispersion dependence on temperature and frequency. The dielectric loss factor obtained at low frequency measurements was found to be directly proportional to the heatability of polymers. A WLF plot was used to predict the shift of dielectric loss maxima into or out of the microwave frequency range.

    Notes: The objective of the current paper was to extend the relationships between complex polymer structures and microwave absorptivity that were formulated in the first three papers of this series (1-3). The microwave processing of composite and polymeric blends via a cylindrical resonance wave cavity and a rectangular standing wave applicator is described. These polymeric materials were irradiated in a low power (〈 100W) electric field at 2.45 GHz. Graphite-epoxy laminates were processed in both standing and traveling wave applicators. Rapid heating and curing were achieved in both cases. An observation of significance was that, with proper tuning of the traveling wave device (the precursor of a protable repair tool), it proved to be highly effective in processing. Additionally, a compatible blend of poly(methyl methacrylate) and poly(vinylidene fluoride) was heated in and applicator and the rates of temperature rise were demonstrated to depend upon morphology.

    Notes: The overall objective of these studies was to investigate the relationship between polymer structure and microwave absorptivity. In this paper, the microwave processing of semicrystalline polymers such as poly(ether ether ketone) (PEEK), nylons, and poly(ethylene terephthalate) (PET), via a cylindrical resonance wave cavity and a rectangular standing wave applicator is described. These polymeric materials were irradiated in low power (〈 50W) electric fields at 2.45 GHz. Silicone flexible molds were necessary for improved processing of nylons and PEEK at temperatures below their Tc Rapid heating rates were observed between the glass transition temperature, Tg, and the melting temperature, Tm, for all these polymers provided that Tc was exceeded. Both dynamic mechanical thermal analysis (DMTA) and dielectric thermal analysis (DETA) spectra were utilized to predict the heating phenomena between amorphous and semicrystalline materials and to explain the rapid crystallizing rate of PEEK. above its glass transition temperature. Correlations were drawn between (a) the apparent activation energy and the critical temperature (Tc) and (b) the shape of the dielectric spectra at 2.45 GHz and its shape in kHz region.
