Abstract The most important aim in acute care of complex severe hand injuries, besides the conservation and reconstruction of functional structures, is adequate closure of defects, to achieve early mobilization and functional restoration of the grasping function. The first step in this is denridement extending into healthy tissue, if necessary with the use of microsurgical methods. The next step, after the reconstruction of injured structures, is adequate and tension-free closure of the wound to ensure adequate cover of the soft tissues. The secondary care of large defects by plastic surgical techniques should be accomplished within the first 72 h after the initial debridement whenever possible (differentiated urgency). In addition to excellent specialist staff and availability of the instrumentation needed, the care of severe injuries of the hand demands a wide-ranging knowledge of surgical techniques appropriate to the hand and of plastic surgery.

Zusammenfassung Wichtigstes Ziel bei der Akutversorgung komplexer schwerer Handverletzungen ist neben der Erhaltung und Rekonstruktion funktioneller Strukturen der adäquate Defektverschluß, um eine frühzeitige Mobilisation und funktionelle Wiederherstellung der Greiffunktion zu erreichen. Als erster Schritt gehört hierzu ein ausgedehntes Débridement bis ins Gesunde u. U. auch mit mikrochirurgischen Methoden. Der nächste Schritt ist nach der Rekonstruktion verletzter Strukturen der adäquate spannungsfreie Wundverschluß, der eine suffiziente Weichteilbedeckung gewährleistet. Die sekundäre Versorgung großer Defekte durch plastisch-chirurgische Maßnahmen sollte nach Möglichkeit innerhalb der ersten 72 h nach dem ersten Débridement erfolgen (urgence différé). Die Versorgung schwerer Handverletzungen erfordert neben einer guten fachpersonellen und instrumentellen Ausstattung ein breites chirurgisches Spektrum in der Hand- und Plastischen Chirurgie.
