    Description / Table of Contents: Zusammenfassung Alle „arteriellen” Lebermetastasen sind praktisch inoperabel und haben eine ausgesprochen schlechte Prognose. Chirurgisch angehbar sind nur synchrone bzw. metachrone portale" Lebermetastasen von relativ wenig bösartigen Colon- und Rectumtumoren. Bei gesichertem Solitärbefall ist die linke Leber zu resezieren, rechtsseitig die Exstirpation des Tumors anzustreben. Als zusätzliche chirurgische Maßnahme kommen in Betracht die Ligatur der A. hepatica bzw. von Ästen der Vena portae, Dauerinfusionen von Cytostatica und Radioisotopen über die A. hepatica oder die Vena umbilicalis sowie eine kryochirurgische Therapie.
    Notes: Summary All arterial metastases of the liver are virtually inoperable and have a poor prognosis. However, if portal metastases from low-grade malignancies of the colon or rectum are noted at the same time as or after the tumor is removed, surgical treatment is possible. If a single metastasis is found in the left lobe, the entire lobe must be removed, while if it is situated in the right lobe extirpation of the actual lesion may be sufficient. Additional surgical measures to be considered are ligation of the hepatic artery or of branches of the portal vein, infusion of cytotoxic agents and radioisotopes via the hepatic artery or the umbilical vein, and cryosurgical therapy.
